1.0 Description of the Project / The Site & Surroun ding Land Use

The Site The proposed location is Churchfield Primary School, Latymer Road, Enfield, Middlesex N9 9PL. The school was originally built in the mid 1970s and was designed as separate infant and junior schools. It was amalgamated in the 1980s to form a single primary school and has grown by accretion and provision of temporary classrooms to its current size. The site is located off Latymer Road with a single narrow access shared by pedestrians and vehicles. It is bordered on the north and east boundaries by private residential property, to the west by a recreation ground and to the south by Salmon’s Brook. The actual site boundaries are indicated on the contract drawings and these are to be secured by the Principal Contractor and maintained as secure throughout the course of the contract. It is anticipated that access for the second stage of the works (new extension and eco classroom) will be via the private access way between 123 and 125 Latymer Road, subject to terms and conditions to be agreed between LBE and the owners. Surrounding Land Use Adjacent buildings are residential housing and will remain in occupation and use throughout the works. All Contractors’ vehicles must be parked to avoid obstruction at all times. Loading and offloading of materials must take place within the Principal Contractor’s designated site area.

The works The works comprise the consolidation of pupil numbers on the site to 3 form entry, i.e. increasing from 574 to 630 pupils (Reception to Year 6).

Six classes are currently housed in three double temporary (mobile) classrooms. The intention is to provide six new classrooms plus additional teaching and non teaching space. The works will provide approximately 1280m2 of new build accommodation together with some remodelling and modernisation, additional hard play area and car parking.

The scheme proposes a two storey extension to the north west of the main school building and linked to it as Contract 2 and a new standalone building as an eco classroom. Contract 1 (by others) requires the extension of the main school reception and relocation of the admin offices from Easter to the end of the summer break 2011. There is expected to be some overlap between Contracts 1 and 2.



1.1 Introduction Kind & Company (Builders) Limited, as the Principal Contractor for this project has formulated this Construction Phase Plan in accordance with the Construction (Design & Management) Regulations 2007 and the Managing Health & Safety in Construction Approved Code of Practices 2007, to enable the Company and others associated with the project to improve planning and arrangements of the health and safety requirements for the project. Designated Safety Advisor: TBA Tel No: Tba 2.2 Responsibilities and Duties of Management Kind & Company (Builders) Limited will maintain the duties and responsibilities as the Principal Contractor for the duration of the works. The Project Manager will be overall responsible for health and safety for this contract (refer to the Company Health & Safety Policy, Part 1). Specialist Package Contractors will be appointed who will be responsible for complying with all Health & Safety requirements related to their works, including adequate supervision. At no time will the site be operational without competent supervision.



2.3 Health & Safety goals for the Project To prevent, insofar as is reasonably practicable, risks and any unplanned events that results in injury, ill health to people, or damage to property, plant materials, or the environment or a loss of business opportunity and to complete the project on time and within budget. 2.4 Arrangements for monitoring and reviewing Healt h & Safety performance The Project Management Team plan to carry out regular Health & Safety monitoring to ensure that Health & Safety standards are of an acceptable level throughout the duration of the project. Kind & Company (Builders) Limited have arranged initially for fortnightly monitoring of the works to be carried out by their Health & Safety Advisor to ensure compliance with Statutory Legislation, Approved Codes of Practice and Safe Systems of Work. If safety standards are not being met satisfactorily, the safety monitoring and Safety Inspections will be increased to assist where improvements may be necessary. Kind & Company (Builders) Limited require any specialist Package or Trade Contractors to carry out their own monitoring and reviewing of Health & Safety on site and report to Kind & Company (Builders) Limited’s Project Manager as soon as possible after each review / inspection. Health & Safety will be reviewed by all persons associated with the project during Site Progress Meetings. 2.5 Liaison between parties on site Sufficient liaison with the Client and the Client’s Project Team will be held with Contractors prior to the Contract starting and throughout the duration of the project. Regular liaison and meetings will be held with all parties involved as circumstances dictate. Safety issues are planned to be discussed during monthly Progress Meetings. 2.6 Consultation with the workforce Kind & Company (Builders) Limited Project Manager will ensure provisions are in place and maintained for consultation with the workforce throughout the duration of the project. Management will liaise with workforce representatives and the workforce generally on a regular basis to ensure that any Health & Safety issues raised will be considered so that Health & Safety can be developed and improved where necessary. In addition, Kind & Company (Builders) Limited will maintain pro-active open channels of communication with the workforce, via formal or informal meetings, written communications or by Management having general discussions with the workforce. 2.7 The exchange of design information between the Client, Designer, CDM Co-ordinator and Contractors The Contract Administrator will ensure that the exchange and circulation of information in relation to design changes occurs as required and that records are updated accordingly. The media for exchange of information will be agreed in advance with all parties on the project. The Contract Administrator will ensure that significant design information regarding the project is incorporated into the Health & Safety File.



2.8 Handling design changes during the Project Design changes agreed by the Project Team are to be co-ordinated by the Contract Administrator. Any risks involved in design change are to be evaluated by the Project Team and suitable control measures relayed to the workforce in advance of the work. 2.9 The selection and control of Sub-Contractors Package Contractors and Trade Contractors engaged by Kind & Company (Builders) Limited will be subject to investigation and vetting procedures instituted by Management. Only competent and suitably resourced Contractors will be engaged on the project. The Vetting Form required to be completed by Contractors is contained in the Company Health & Safety Policy, Part 3. The Project Management Team will adequately induct, supervise and monitor Contractors Health & Safety performances to ensure compliance with all appropriate Health & Safety Standards and arrangements. Non-conformance by Contractors will be thoroughly investigated and additional control measures will be implemented to prevent reoccurrence. 2.10 The exchange of Health & Safety information be tween Contractors The exchange of information between Contractors on this project will be via the Project Manager. 2.11 Site Security Hoarding, access gates and signage is to be maintained throughout the duration of the project by Site Management and the workforce. “No Unauthorised Persons Beyond This Point”, “Danger – Keep Out” signs are to be displayed around the perimeter of the site for the duration of the project. All Site Personnel will be required to comply with Kind & Company (Builders) Limited security arrangements for the project as outlined below:- Access to the project site will be strictly controlled by Site Management. All Site Personnel, Contractors and Visitors will be required to report to Site Management upon arrival and on departure from site. Sub-Contractors are to supply Kind & Company (Builders) Limited with names, addresses and dates of birth of all persons who will be required to work on site. Sub-Contractors are to advise Kind & Company (Builders) Limited in advance of any changes of site operatives due to sickness or absenteeism etc., and will supply this information before persons commence work. All site personnel are to co-operate with security arrangements by informing Site Management of any suspicious persons or occurrences that they witness.



2.12 Site Induction All personnel required to work on site must receive Site Safety Induction prior to being allowed to work on site. Kind & Company (Builders) Limited Site Management will provide Site Safety Induction to Contractors and main groups of workers. A register of persons receiving Site Safety Induction will be kept on site. Contractors are to ensure that all their Site Personnel have received Company Induction. If Site Personnel miss Kind & Company (Builders) Limited Induction courses, the Contractors Management Team will be responsible for ensuring they give Site Safety Induction to the minimum standard of Kind & Company (Builders) Limited Health & Safety Policy and provide proof to Kind & Company (Builders) Limited Site Management that Induction has taken place. Site Safety Induction is to follow the minimum format laid out in Kind & Company (Builders) Limited Health & Safety Policy, Part 3. 2.13 Site Safety Training Site personnel will be required to demonstrate both Health & Safety and Trade competence prior to attending Site Induction. Copies of Site Personnel’s competency Training Certificates must be provided to Kind & Company (Builders) Limited Site Management Team, which will be kept on site in the Site Safety File which will be available for inspection. Kind & Company (Builders) Limited’s Designated Site Safety Advisor and Site Management will perform Safety Tool Box Talks to remind Site Personnel of general safety arrangements. Contractors will be required to give regular Safety Tool Box Talks to their own workforce. Proof of Tool Box Talks given is to be submitted to Kind & Company (Builders) Limited Site Management in the form of Tool Box Talk Registers which will be kept in the Site Safety file which will be available for inspection.



2.14 Welfare facilities and First Aid Kind & Company (Builders) Limited will set-up all main site welfare facilities in accordance with the Site Set Up Plan. This will include the following:- 2.14.1 A toilet and hand basin will be provided. Paper hand towels and soap will also be provided for general use. These facilities will be maintained by Kind & Company (Builders) Limited. All site personnel are required to keep the facilities in clean order after use. 2.14.2 A canteen facility will be provided by Kind & Company (Builders) Limited, which will include the following:- � Seating (chairs with backs - no benches) for a minimum of 20 persons should be

provided or a managed system to stagger rest breaks must be put in place; � A sink connected to the sewer and freshwater supply; � Hot running water; � Electricity supply; � The facility for heating food and water; � Refrigerator; � Flip top bin; � Basic supply of cutlery and crockery; � Impervious flooring and worktops; � Heater; � Health & Safety Notice Board. The canteen facility will be arranged to be cleaned on a regular basis by Kind & Company (Builders) Limited. All Site Personnel are required to keep these facilities in clean order after use, particularly with regard to food hygiene, keeping surfaces and cutlery clean and disposing of waste food. 2.14.3 All Contractors will be required to carry out a PPE Assessment for their own Site Personnel and provide all appropriate personal protective equipment to their workforce. The standard issue to all Site Personnel is required to be provided by the Contractors to their own workforce, which is as follows:- � Safety helmets; � Safety footwear; � Fluorescent tops, i.e., Jackets, T-shirts, Vests, etc.

Contractors are to ensure their Site Personnel wear the standard issue PPE at all times whilst on site, except where it may be designated a ‘safe area’ by Kind & Company (Builders) Limited Site Management, such as the canteen and the site office.



2.14.4 A drying room and changing facilities will be provided by Kind & Company (Builders) Limited in the compound area. These facilities will consist of the following:- � A bench; � Wall hooks; � Heater. These facilities will be maintained by Kind & Company (Builders) Limited i.e. regularly cleaned to prevent a build-up of discarded PPE and Contractors should advise their workforce of this procedure so that personal belongings will not be lost. Kind & Company (Builders) Limited will not be responsible for the security of personal belongings and advise that Contractors should make their own arrangements for their workforces valuables during the working day. All site personnel are required to keep the facilities in clean order after use. 2.14.5 Kind & Company (Builders) Limited will ensure that an adequate number of First Aiders are provided for the duration of the works including appropriate First Aid Kit. The First Aid Kit will be located in the Site Managers office. Contractors are required to carry out their own First Aid Risk Assessment for the type of works they will be involved in on site and ensure they have sufficient First Aid provisions where required for their own specific needs. 2.14.6 Shared welfare arrangements are to be agreed in writing with all Contractors and a record kept on site available for inspection. Any additional welfare required by Contractors for their workforce must be fully expressed and agreed with Kind & Company (Builders) Limited Project Team in advance of working on site.

2.15 The reporting and investigating of accidents a nd incidents, including near

misses Kind & Company (Builders) Limited operates a comprehensive incident reporting and investigation system as part of their Health & Safety Management System. All accidents, incidents and near misses, irrespective of their origin or result, are required to be reported to Kind & Company (Builders) Limited Site Management as soon as possible, who in turn will report to the Company’s Designated Safety Advisor for further advice. If any accidents occur, they are to be logged in the Site Accident Record Book / Form. All accidents will be investigated by Kind & Company (Builders) Limited Site Management and those considered significant, including RIDDOR reportable will be further investigated by Senior Management with the assistance of the Company’s independent Health & Safety Consultants, Richardson-Hill Limited. RIDDOR is to be fully complied with and Contractors are required to provide a copy of any RIDDOR reportable reports to Kind & Company (Builders) Limited Senior Management and a copy is required to be kept in the Site Safety File.

2.16 The production and approval of Risk Assessment s and written Systems of work The Design Team for the project have produced Risk Assessments and control measures to be considered to minimise risks and hazards for this project. These documents have been considered by Kind & Company (Builders) Limited Project Management Team in the planning of the project.

Kind & Company (Builders) Limited Project Management Team members have undertaken a Project Specific Risk Assessment exercise and have produced Action Required Method Statements for the main safety controls for the project. This document is included in the Appendices section 5.2 of this Construction Phase Plan.

Contractors are to carefully consider the Design Team and the Project Management Team’s Risk Assessments and control measures in advance of works to ensure the control measures are suitable and can be accommodated by the Contractors works on site. Any areas of concern or conflict with regard to Health & Safety must be communicated to Kind & Company (Builders) Limited Project Management Team for further consideration and any necessary additional safety measures.

Further specific detailed Risk Assessments and Safe Method of Work Statements are to be carried out by Contractors where necessary and agreed with Kind & Company (Builders) Limited Project Management Team, prior to commencement of Contractors operations on site. This should be carried out with sufficient time for arrangements to be agreed and accommodated. Contractors Risk Assessments and Safe Method of Work Statements are to be supplied to the Site Management Team, copies of which will be kept in the Site Safety File, available for reference purposes and inspection. E.g:-

• Work on an occupied school site/premises. • Demolition works. • Access and egress to/from local estate roads. • Maintaining means of escape at all times. • Protection of school children, school staff, visitors, pedestrians, local

residents and private property. • Prevention of damage to Highway Authority assets (e.g. footways,

carriageways and highways infrastructure, etc. • Tracing for hidden or buried existing services within the confines of the

project works. • Works creating dust, noise and vibration. • Work at height. • Lifting operations. • Manual handling. • Construction traffic movements. • Lifting operations. • Asbestos Removal • Any hot works • Gas works • Electrical supply works, disconnections and reinstatements; • Movement and use of plant, equipment and materials; • Working at height and use of access equipment;



These will be examined and approved by Kind & Company (Builders) Limited Project Team prior to work being allowed to commence. Sub-Contractors will be required to provide evidence that the contents of such documents have been conveyed to their operatives prior to such activities commencing. Site Management for the project are to ensure that site personnel are made aware of all necessary safety precautions in connection with their work prior to commencement of such activities. All necessary information will be conveyed to the workforce and a record kept. 2.17 Site rules, including drugs and alcohol policy Kind & Company (Builders) Limited General Safety Precautions and Site Rules will be complied with. These arrangements must be conveyed during Site Induction to all Site Personnel involved in the project. Kind & Company (Builders) Limited Site Management and Contractors supervision will monitor compliance with these and other arrangements specified in Kind & Company (Builders) Limited Health & Safety Policy, the Contractors Policy, Site Risk Assessments and Safe Method of Work Statements on a continuous basis throughout the duration of the project. 2.17.1 General Safety Precautions and Site Rules: REFER TO APPENDIX 5.8 � All Site Personnel to sign in and out of the site and make their presence

known on site to Management and wear ID Badges � Standard PPE, safety footwear, safety helmets and fluorescent tops will be

worn at all times. A reasonable dress code shall be followed � No contact with the school, school children or staff is permitted at any time � General public safety to be maintained at all times. � All temporary electrics are to be 110 Volt supply and to conform to the HSE

publication HS(G) 141 'Electrical Safety on Construction Sites'. � Use the right tools for the job. � Plant and equipment to be kept in good order and not misused. � Report any safety defects to Site Management. � No smoking, no naked flames in areas at risk from fire and explosions. � Check the working environment for hazards before working. � Ensure instructions are fully understood before working. � Maintain personal hygiene and look after welfare facilities provided. � Be aware of others safety - do not create hazards. � Always take precautions to prevent materials and tools etc., from falling. � Ensure that debris does not accumulate unsafely. � Do not obstruct accessways unsafely. � Do not carry loads that cannot be managed comfortably to prevent the risk of

injury. � Report all accidents and dangerous occurrences immediately to Site

Management. � The consumption of, or being under the influence of alcohol is not permitted

on site. � No gambling allowed on site. � No radios allowed on site.



� No burning of rubbish on site. � A Permit to Work System is required for Confined Spaces, Work on or

adjacent to Live Electrical Services, Hot Works, Hazardous Materials, Asbestos Removal, Use of Disc Cutters, Work at Height involving the use of fall arrest equipment, Excavations

� Smoking in designated area only � The use of illegal drugs is not permitted on site. � No on site parking is permitted. Parking is restricted to surrounding roads. � Violent or abusive behaviour is not permitted on site. � Waste: Remove rubbish, debris, surplus material regularly and keep the site

and Works clean and tidy. You are responsible for removal of your waste/packages to skips.

� Any potential hazards, incidents or accidents must be reported to the Site Agent immediately.

� Storage of LPG canisters or cylinders: remove from site daily or store to comply with the Codes of Practice for the keeping of LPG in cylinders and similar containers.

� All local speed limits shall be strictly complied with. � Anyone who does not understand their responsibilities regarding Health and

Safety must discuss this with the Site Agent, Kind & Co will provide any assistance required which will include offering subcontractors and subcontract operatives the guidance of Kind & Co’s Safety Consultant.

2.18 Fire & Emergency Procedures An appropriate outlined Fire & Emergency Plan has been formulated and included in the Risk Assessment / Action Required Method Statement contained in Appendices section 5.2 of this Construction Phase Plan. This outlined Plan will be developed by the Kind & Company (Builders) Limited Site Management Team and Contractors as works progress, where appropriate. Site Personnel will be informed of fire precautions during Site Safety Induction prior to them commencing work on site. Site Management will monitor the escape routes to ensure they remain free from any unsafe obstructions. Escape routes will not be permitted to be used for work activities or storage areas. The site will be a designated ‘No Smoking’ area. Temporary signage will be displayed where necessary with regard to Fire Points and emergency evacuation routes / exits etc. Temporary fire points will be provided by Kind & Company (Builders) Limited for this project. The fire points will be located at strategic positions on the site and are to comprise of 9 litre water and CO2 fire extinguishers as a minimum. All potential hot works will be carried out under the strict control of Hot Works Permits issued and managed by Kind & Company (Builders) Limited Site Management.



Kind & Company (Builders) Limited have contained in their Health & Safety Policy, a general Fire Policy and Fire Policy Checklist which must be conformed to where appropriate by all Site Personnel. Contractors are to appoint competent members of their workforce in writing as Fire Wardens to assist in the maintenance of fire precautions and to report any defects to Kind & Company (Builders) Limited Site Management Team. 3.0 Arrangements for controlling significant site r isks 3.1 Delivery and removal of materials, waste and wo rk equipment etc., taking into account any risks to the general public, neigh bours and site personnel, traffic routes and segregation of vehicles and pede strians REFER TO APPENDX 5.15 – Site Delivery Traffic Management Statement There will be no on site parking. Parking will be on main road and surrounding streets. It is important that site activities and deliveries, etc., do not put the general public, neighbours and site pedestrians at risk. Site personnel are to comply with all Local Authority requirements, The Road Traffic Act for the roads, in particular with regard to parking and any speed restrictions.

Existing vehicular, pedestrian access routes and rights of way are to be safely maintained throughout the duration of the project. 3.2 Dealing with services, water, gas, electricity, including overhead power lines and temporary services Kind & Company (Builders) Limited will arrange to have all of the live gas and electric services isolated in the public highway (for buildings to be demolished) prior to any demolition of building works commence. To the existing building to be refurbished the electricity supply will be downgraded to 110v using transformers. Refer to section 1.5 for existing survey records. Contractors are to study this information in advance of their works. Work on existing site services will be carried out under a Permit to Work. Where circuits are disconnected at power source, provisions will be in place to ensure the following:- a) That there will be no possibility of accidental re-connection. b) Identification and warnings against re-connection are displayed. c) Records of Permits are to be kept on site throughout the contract.



Contractors are to exercise extreme caution with the testing and subsequent reinstatement of any service supplies, in particular electric and gas. The Project Team will ensure they obtain “as installed” drawings clearly showing the routes of all hidden services. Live high voltage wiring and gas services necessary to be left live will be identified in all areas that could be affected by the works as ‘live’. Live services in the vicinity of the works must be adequately safeguarded and protected from damage. Wiring disconnected will be identified in all areas as ‘dead’. Electrically powered tools and equipment are required to be 110v or battery operated wherever possible. Any specialised equipment that requires a higher voltage will only be permitted if there is no alternative and after an application has been made in writing and agreed by the Project Team. This type of equipment will have power cable runs kept as short as possible and suitably protected from potential damage and will be connected to Residual Circuit Devices (RCD’s) Only competent and approved Engineers working to current standards and industry guidance will be allowed to carry out work on gas and electrical systems. Gas and electrical works will be appropriately tested and certificated. Contractors are to provide Test Certificates and data information to Kind & Company (Builders) Limited Site Management Team who will retain copies on site in the Site Safety File. Refer to Project Specific Risk Assessment contained in the Appendices Section 5.2. 3.3 Accommodating adjacent land use The proposed location is Churchfield Primary School, Latymer Road, Enfield, Middlesex, N9 9PL The school was originally built in the mid 1970s and was designed as separate infant and junior schools. It was amalgamated in the 1980s to form a single primary school and has grown by accretion and provision of temporary classrooms to its current size. The site is located off Latymer Road with a single narrow access shared by pedestrians and vehicles. It is bordered on the north and east boundaries by private residential property, to the west by a recreation ground and to the south by Salmon’s Brook. The actual site boundaries are indicated on the contract drawings and these are to be secured by the Principal Contractor and maintained as secure throughout the course of the contract. It is anticipated that access for the second stage of the works (new extension and eco classroom) will be via the private access way between 123 and 125 Latymer Road, subject to terms and conditions to be agreed between LBE and the owners.



Surrounding Land Use Adjacent buildings are residential housing and will remain in occupation and use throughout the works. All Contractors’ vehicles must be parked to avoid obstruction at all times. Loading and offloading of materials must take place within the Principal Contractor’s designated site area. The Contractor must ensure that clear access to the adjoining parts of the school site and buildings are maintained and suitably segregated from the works and vehicles movements at all times, and school users are not impeded by Contractor’s vehicles, materials and/or plant. All public and private highways and access points must be kept free of obstructions during the works. The perimeter of the site has a combination of existing timber fencing, brick walling and trees bordering neighbouring residential properties. All site personnel will be required to work diligently to safeguard neighbours and their visitors, as well as their properties, giving consideration with special regard to safe access and egress, protection of services and environmental issues, such as dust and noise. (Refer to Project Specific Risk Assessment contained in the Appendices Section 5.2 and 5.16 Environmental Policy) 3.4 Stability of construction whilst carrying out c onstruction work, including temporary structures and existing unstable structur es The Package Contractors and other Contractors are to fully consider the stability of the existing building to be refurbished while demolition, refurbishment and the new structure is being built including access equipment such as scaffolding and plant etc. The Designers of the new structure are to take full account of ensuring the stability of permanent structure. Any alteration or process / conditions which could affect the stability of such structures are to be fully considered and communicated to Kind & Company (Builders) Limited Project Team. Appropriate safety information regarding stability must be calculated and detailed. This information must be available on site in the Site Safety File. 3.5 Preventing falls Working at height will be planned in line with The Work At Height Regulations 2005. Wherever possible, working at height will be eliminated or minimised. 3.5.1 Preventing persons from falling It is anticipated that the main risk from falling will be in connection with high level works, i.e:- � Falling into excavations; � Working on erecting, maintaining and dismantling scaffolding; � Working from scaffolding; � Work in connection with constructing floors on the new structure; � Edge protection on floor edges and stairwells, etc;



� Climbing and descending access ladders; � Working with lifting appliances; � Working on roofs; � Works on staircases; � Tripping and slipping; � Working from the back of lorries. Working at height and the potential of falling will be continuously assessed by the Project Team and Contractors. Risk Assessments / Safe Method of Work Statements are to be formulated and appropriate provisions provided prior to starting work where it could be possible for persons to fall. Sufficient measures are to taken to prevent persons from falling at all times. Site personnel working at height are required to work from the most suitably safe and adequate access equipment (such as scaffold, podium steps or similar fitted with guardrails) in order to minimise risks Risk Assessment and Method Statement Induction is required to be conveyed to persons required to work at height prior to allowing them to work. Only competent persons will be allowed to work at height. Working at height is required to be supervised by a competent person to ensure that all safety controls are in place to prevent falls. Safety Tool Box Talks will be given by Kind & Company (Builders) Limited Site Management and Contractors Management to persons working at height. This is required to be carried out regularly throughout the project to site personnel of the risks associated with working at height. Working at height areas will be required to be inspected by Contractors before each shift by Contactors competent persons prior to allowing works to continue.

3.5.2 Preventing Materials and Equipment etc., fro m falling The working area will be sufficiently fenced or cordoned-off to prevent any persons from venturing into areas where there could be a risk of materials, etc., from falling. Provisions will be put in place to prevent materials and equipment etc., falling from height, such as:- � Monaflex / debris netting and brickguards; � Sandwiching plastic sheeting within double boarding of the 1st lift; � Scaffold fans. Materials and equipment which could be caught by the wind will be adequately secured. Materials such as plywood, scaffold boards, etc., will be laid down flat to prevent them from being blown over by the wind, or knocked over accidentally. 3.5.3 The Use of Ladders Ladders will only be permitted if there is no other reasonably practicable alternative. Persons who are required to use ladders and step-ladders will need to demonstrate their competence and a checking procedure is required to be in place to ensure that ladders are safe and suitable prior to use by a competent person.



3.5.4 Erecting, maintaining & dismantling scaffoldi ng Scaffolding is required to be erected in accordance with current Industry Codes of Practice, Manufacturers / Suppliers specifications and relevant Regulations to ensure suitability and stability. Scaffolders are to submit evidence of their competency prior to carrying out scaffold works. The scaffolding works will supervised by an advanced Scaffolder and overseen by the Site Management Team. Copies of Handover Certificates will be provided to the Site Management Team prior to the scaffolding being used, copies of which are required to be kept on site for the duration of the project. Risk Assessments and Safe Method of Work Statements for scaffolding works will be required prior to scaffolding works being carried out on site. Scaffold inspections will be carried out by a competent person on a weekly basis and after any alterations or bad weather conditions. Records of inspections will be made and kept on site utilising the appropriate inspection report forms. Contractors supplying their own scaffolding, which includes tower and system scaffolds, must make arrangements for a competent person to carry out the legally required inspections. Copies of Inspection Reports are to be submitted weekly to Kind & Company (Builders) Limited Site Management Team. It is envisaged that scaffolds will be required for the following tasks:-

� To gain access to building elevations; � To store small quantities of building equipment and materials; � To provide protection to accessways and working areas around the site; � Access in stairwells; � To prevent the need for ladders when working internally on high ceilings etc; � For Surveys and Inspections, etc. 3.6 Work with or nearby fragile materials All appropriate precautions will be required to be taken by Contractors and Site Personnel when working near fragile materials, such as:- � Glass; � Temporary decking; � Plasterboarded ceilings.



3.7 Control of Lifting Operations Lifting operations will be carried out in accordance with The Lifting Operations and Lifting Equipment Regulations 1998 (LOLER) and The Work At Height Regulations 2005. Contractors works must be fully considered within the Safe Working Load capacity of any mobile crane or other lifting appliances that may need to be utilised on site. Competent persons must be appointed to supervise and manage lifting operations. Only competent persons are allowed to be involved in lifting operations. Details of operators who have received training and are authorised to use lifting equipment are to be included in a Lifting Plan, along with the safe operating systems and precautions. Safety Tool Box Talks are to be given to persons working at height regularly throughout the project to remind them of the risks associated with working at high levels with regard to the potential of materials etc., falling onto persons below and neighbouring properties. Proof of thorough examination, testing and inspecting records are required to be retained on site in the Site Safety File. Refer to Project Specific Risk Assessment contained in the Appendices Section 5.2 and Manufacturers / Suppliers Specifications. Refer to Kind & Company (Builders) Limited Company Health & Safety Policy Procedures, Part 2. 3.8 The maintenance of plant and equipment All plant and equipment to be used on site is required to be selected for its suitability and safety. All plant and equipment supplied to site must comply with current Legislation. Appropriate Test Certificates and maintenance records are required for plant and equipment prior to them being used on site. Appropriate and regular checks are required to be made by Contractors for plant and equipment to ensure it is properly used and maintained. Plant and equipment operators will be competent and have competency certification where required, i.e. C.I.T.B. All electrical tools will be either battery operated or 110v supply and will be PAT tested as appropriate. Competency Certificates and Test Certificates for equipment will be kept on site in the Site Safety File. All defective plant or equipment will be taken out of use, isolated, clearly marked ‘Out of Order’ and removed from site or suitably repaired as soon as possible.



Particular attention will be given to the possible hazards associated with using the following plant:- � Electrical equipment � Angle grinders � Compressed air equipment � Ladders

� Mixers � Vibration and noise from plant or hand held tools � Any lifting equipment and lifting gear � Scaffolding

All plant & equipment, whether provided by Kind & Company (Builders) Limited or its Contractors will be properly maintained and inspected on a regular basis and according to the regime established by the Kind & Company (Builders) Limited Health & Safety Management System, PUWER & LOLER. Refer to Project Specific Risk Assessment contained in the Appendices Section 5.2 and Manufacturers / Suppliers Specifications. 3.9 Work on excavations and work where ground condi tions are poor Not anticipated on this project 3.10 Work on wells, underground earthworks and tunn els Not anticipated on this project. 3.11 Work on or near water where there is a risk of drowning Not Applicable 3.12 Work involving diving Not applicable to this project. 3.13 Work in a caisson or compressed air working Not applicable to this project. 3.14 Work involving explosives Not applicable to this project.



3.15 Storage of materials (particularly hazardous m aterials) and work equipment Package Contractors and Contractors / Sub-Contractors are to store their general materials and equipment in the designated storage area on site. See Appendix 5.6 Site Set Up Plan. Hazardous storage must be agreed with Kind & Company (Builders) Limited Site Manager prior to them delivered to site. Storage requirements are to be planned to minimise associated risks such as:- � Unsafe obstructions; � Overloading; � Stability; � Unnecessary handling; � Difficulties in slinging and lifting. Special consideration must be given to potentially hazardous substances such as:- � Explosive gases – LPG and Acetylene; � Fuel for plant; � Highly flammable substances such as solvents and paints; � Combustible waste such as timber and packaging.

Contractors are required to provide Site Management with Manufacturers Data Sheet information, COSHH Assessments and detail their storage arrangements for the substances of special consideration. 3.16 The removal of asbestos Refer to Type 3 Asbestos Survey Report carried out by Tersus on 13 December 2008; reference L802/NPS1/1. Refer to the Method of Analysis and Conclusion contained in Appendix 2 of the report. Refer to Appendix 5.12 3.17 Dealing with contaminated land Refer to Site Investigation Report; Report reference No.C11584. 3.18 Manual handling Contractors are required to minimise manual handling wherever possible. Any essential manual handling is required to be restricted to weights and material sizes capable of being handled safely. Where materials are required to be manually handled, weights are required to be restricted to 20kg or less and distances for carrying manually, minimised. Equipment is to be provided to minimise the need for manual handling. Persons required to carry out manual handling are to be suitably fit, trained and competent. 3.19 The use of hazardous substances, particularly where there is a need for health monitoring The use of hazardous substances is required to be kept to the minimum. All potentially hazardous substances to be used on this project are to be subject to a COSHH Assessment and such assessment information along with risk reduction and control measures are to be conveyed to site personnel prior to use. A complete list of all potentially hazardous materials / substances / operations and waste etc., for the project is to be accompanied by COSHH Assessments, Manufacturers Data Sheets



and Method Statements to ensure appropriate control measures will be in place. This information is to be submitted to Kind & Company (Builders) Limited Project Team in advance of being brought onto the site and used, etc. Note : The provision of Manufacturers Data Sheets will not substitute for Employer’s COSHH Assessments. The following list of substances, which is not exhaustive, have been considered as having the potential of being hazardous and Contractors are therefore required to consider this list and any other potentially hazardous substance carefully and provide all necessary safety measures to protect the workforce and others on this project:- Materials requiring particular precautions with wor k on existing buildings

• Any asbestos containing materials, including rainwater goods,

cowls, board, spray applied materials, coatings, cement based sheet products, floor tiles, bituminous materials/adhesives.

• Insulation to pipes and lagging, ropes and seals, etc. • Existing painted surfaces containing lead. • Lead sheet materials (e.g. flashing). • Lead pipes. • Lead sash window counter-weights. • Brick/cement/concrete and general dusts. • Existing plaster boards. • Existing insulation (e.g. glassfibre and/or Rockwool). • Existing areas of damp/mould (mould spores). • Lath and plaster materials (particularly those containing animal

hair as a constituent of the plaster mix). • Biological materials and detritus deposits remaining foul and

surface water pumping stations, manholes, catch-pits, chambers, drainage pipes, disused plumbing fixtures, etc.

• Glass and glazing.

In addition, the following materials may present health hazards and where used in the works the Contractor should obtain the relevant Manufacturer’s Safety Data Sheet (MSDS), ensure suitable and sufficient COSHH assessments are carried out and the contents made known to all persons involved in the works who may be exposed to risks associated with the use of the products.

• Any cementitious materials, screeds, fillers, mortars and

concrete,etc. • Plaster and lime powders, dusts and mixes. • Any solvent based adhesives. • Spray applied paints and adhesives. • Insulation fibres (e.g. Rockwool). • Solvent based paints/finishes. • Epoxy type resins and compounds. • Pastes and fluxes associated with hot works/soldering, including

fumes given off by hot work processes. • Sealants. • Wood dusts and fibres (particularly MDF).



All safety measures necessary to safeguard the workforce and others must be complied with, including the provision for appropriate training, instruction, supervision, health monitoring and where required, PPE Assessments and record keeping. Contractors are to provide Kind & Company (Builders) Limited Project Team with appropriate information regarding potentially hazardous substances which were not eliminated which have residual risks for the purpose of inclusion into the project Safety File. 3.20 Reducing noise and vibration Noise and vibration is required to be eliminated wherever possible, or kept as low as reasonably practicable throughout the duration of the project. Where the operational risk levels illustrated within The Control of Noise At Work Regulations 2005 could be exceeded, the precautions set out to eliminate or reduce noise levels are to be implemented. Details of maximum exposure times are to be conveyed to the relevant Site Personnel and strictly adhered to. Records of required monitoring should be kept on site in the Safety File, available for inspection. Works generating noise and vibration are to be carried out at times considered reasonable for a residential and commercial area and as may be specified by the Local Authority with regard to noise and by the Project Teams Engineer with regard to limitations on vibration. Contractors are required to give consideration to nearby neighbours and keep the Kind & Company (Builders) Limited Site Manager informed in advance of potentially noisy operations which cannot be avoided, to ensure that suitable measures are taken to suppress noise. Note: This means, in most cases, noise suppression techniques will be expected, or alternative solutions are to be employed. Noisy operations which cannot be avoided are to be planned to be carried out during periods of the day to minimise inconvenience to neighbours. Every consideration should be given not to start any potentially noisy operation before 8.30am or after 5.30pm during weekdays, with a ‘quiet time’ between the hours of 1.00pm to 2.00pm. No noisy operation should be planned to be carried out during weekends, bearing in mind that no work is permitted after 1.00pm on Saturdays. The main activities which are anticipated could involve the potential of high noise levels to neighbours or Site Personnel, which will need to be accounted for and suitable control measures put in place if it is not possible to eliminate, are as follows:- � Plant operations, excavators, generators and compressors; � Power tool operations, breakers, hammer drills, angle grinders, Hilti fixings, etc; � Hand tool operations, hammers; � Scaffold works; � Loading skips; � Lorry deliveries.

The main activities which are anticipated could involve the potential of substantial vibration to structures or to Site Personnel which will need to be accounted for and suitable control measures put in place if it is not possible to eliminate, are as follows:- � Plant operations and excavators;



� Power tool operations, breakers, hammer drills, angle grinders, Hilti fixings, circular saws, jigsaws, chop saws, etc;

� Hand tool operations, hammering, chiselling; � Loading skips; � Lorry deliveries. 3.21 Work with ionising radiation Not anticipated to be an issue on this project. 3.22 Exposure to UV radiation (from sunlight) Contractors are required to advise all their personnel who are required to work on site of the potential health problems associated with UV radiation. Site Personnel will be required to wear suitable clothing at all times when exposed to sunlight. Skin exposed to sunlight should be protected with suitable UV protection sun cream, which Contractors should ensure is available to be used by their Site Personnel. Contractors should monitor these protective measures and ensure that their Site Personnel are safeguarded. 3.23 Hot Works All hot works will be subject to a Hot Works Permit. These will detail all important safety issues to safeguard against fire, such as the person(s) undertaking the task and the type of fire extinguisher(s) that must be present during the task, etc. All hot works will cease one hour before the end of the working shift and the person who carried out the hot works will remain in the vicinity checking regularly to ensure that no fires will occur. Hot works are to be signed-off by the person who carried out the hot works and the competent supervising person. Records of Hot Works Permits are to be kept on site available for inspection. 4.0 The Health & Safety File 4.1 Layout and format Kind & Company (Builders) Limited Project Team will ensure that the layout and format for the Health & Safety File conforms to the CDM 2007 Code of Practice. Contractors are required to provide all appropriate Health & Safety information to Kind & Company (Builders) Limited Project Team that will be needed to allow future construction work, including cleaning, maintenance, alteration, refurbishment and demolition to be carried out safely. This information should alert those carrying out such works to risks so that it helps them decide how to work safely. 4.1.1 Contractors are to provide accurate information wherever possible in advance of their works and on an on-going basis with completion of accurate information no later than 7 days after their works are complete on site. 4.1.2 Designers and Contractors are required to inform the CDM Co-ordinator in advance, in writing, of the information that will be prepared and handed over for the Safety File, specifically for this project. 4.2 Arrangements for collecting and gathering infor mation The arrangements for collecting information for the Health & Safety File from the Designers and Contractors, etc., are to be agreed with Kind & Company (Builders) Limited Project Team. The Project Team will arrange with the CDM Co-ordinator



appropriate arrangements to monitor and review the status of information and provide assistance in amending or adding information as the project progresses. 4.2.1 Contractors are required to compile all appropriate information to Kind & Company (Builders) Limited Project Team’s Contract Administrator for inclusion in the Health & Safety File. 4.2.2 The CDM Health & Safety File is planned to be submitted to the CDM Co-ordinator at the end of the project. 4.2.3 The Health & Safety File will be located with Kind & Company (Builders) Limited head office. 4.2.4 The content of the Health & Safety File must contain the following information which Contractors must submit where it relates to their works:-

The Health & Safety File The Contents of the Health & Safety File: - The purpose of the Health and Safety File is to provide information needed to allow future construction work, including cleaning, maintenance, alterations, refurbishment and demolition to be carried out safely.

All members of the project team are to provide relevant information to the CDM Coordinator for inclusion in the Health and Safety File. This information should include: -

a) a brief description of the work carried out;

b) any residual hazards which remain and how they have been dealt with (e.g. surveys or other information concerning asbestos; contaminated land; water bearing strata; buried services etc);

c) key structural principles (e.g., bracing, sources of substantial stored energy – including pre and post-tensioned members) and safe working loads for floors and roofs, particularly where these may preclude placing scaffolding or heavy machinery there;

d) hazardous materials used (e.g. lead paint; pesticides; special coatings which should not be burnt off etc);

e) information regarding the removal or dismantling of installed plant and equipment (eg any special arrangements for lifting, order or other special instructions for dismantling etc); f) Health and safety information about equipment provided for cleaning or maintaining the structure; g) The nature, location and markings of significant services, including underground cables; gas supply equipment; fire-fighting services etc;



h) Information and as-built drawings of the structure, its plant and equipment (e.g., the means of safe access to and from service voids, fire doors and compartmentalisation etc).

The file does not need to include things that will be of no help when planning future Construction work, for example: a) The pre-construction information, or construction phase health and safety plan; b) Construction phase risk assessments, written systems of work and COSHH assessments; c) Details about the normal operation of the completed structure; d) Construction phase accident statistics; e) Details of all the contractors and designers involved in the project (though it may be useful to include details of the Principal Contractor and CDM Co-ordinator); f) Contractual documents; g) Information about structures, or parts of structures, that have been demolished – unless there are any implications for remaining or future structures, e.g., voids; h) Information contained in other documents, but relevant cross-references should be included.

4.2.4.1 Refer to the CDM Code of Practice with regard to further Safety File Information details. 4.3 Storage of information Kind & Company (Builders) Limited standard method of information storage is to use modern information technology where this is practicable. Contractors should submit information to the Contract Administrator via e-mail wherever possible, in Microsoft Word, Excel or PDF format. If this is not possible, hard copies can be sent by post, but Designers and Contractors must retain copies until the Contract Administrator has confirmed receipt. 4.4 Handing over information at the end of the Proj ect to the Client Kind & Company (Builders) Limited Project Team will hand over the Health & Safety File to the CDM Co-ordinator.

If, due to unforeseen circumstances, this becomes necessary, check the requirements of the Local Authority with regard to permission and Licences and ensure that storage areas on roads and pavements are safely cordoned-off and appropriate signs and lights are in place in accordance with The Road Traffic Act and Local Authority requirements. GENERAL NOTE: No persons are allowed to work on the Company work sites until they have received Site Safety Induction by Kind & Company (Builders) Limited Management. A record should be kept of all persons who have received Induction. Induction to include the following:- Company Health and Safety Policy: Remind personnel of the Policy and its contents; Inform personnel where a copy of the Policy is displayed on site. THE HEALTH AND SAFETY FILE The Principal Contractor shall comply with his duties under the Construction (Design and Management) Regulations 2007 and obligations set out in the contract documents to ensure that the CDM Co-ordinator receives all the information required to complete the Health and Safety File within four weeks of the issue of the Certificate of Practical Completion by the Architect/Lead Consultant.

The Client requires a four ring binder containing originals or good copies of all documents as detailed plus a CD of the same. The contents of the CD shall be pdf, Word or Excel file format with any scanned documents separately identified. A CD with the entire contents of the Health and Safety File/Operating and Maintenance Manuals as a single document will be unacceptable.

Operation and Maintenance Manuals must be available before the building is handed over to the Client.

The information compiled by the Principal Contractor for the Health and Safety File needs to include any items that will assist anyone carrying out construction work, maintenance to the structure after completion of the current project and/or demolition of the structure at the end of its life and may include some of all of the following:-

o Commencement and completion dates o A brief description of the work carried out.

o Names and addresses of sub-contractors and suppliers and the relevant

work packages undertaken.

o General details of the construction methods and materials used.

o Key structural principles (for example, bracing, sources of substantial stored energy - including pre- or post-tensioned members) and safe working loads for floors and roofs, particularly where these may preclude placing scaffolding or heavy machinery there.

o Any residual hazards which remain and how they have been dealt with (for

example surveys or other information concerning asbestos, contaminated land, water bearing strata, buried services, etc).

o Hazardous materials used (for example lead paint, pesticides, special

coatings which should not be burnt off, etc).

o Information regarding the removal or dismantling of installed plant and equipment (for example any special arrangements for lifting, order or other special instructions for dismantling, etc).



o Health and safety information about equipment provided for cleaning or maintaining the structure.

o Details of the structures internal equipment and maintenance facilities.

o Manuals produced by specialist sub-contractors and suppliers which outline

operating and maintenance procedures and schedules for plant and equipment installed as part of the structure.

o The nature, location and markings of significant utilities services, including

underground cables, gas supply equipment, fire-fighting services, etc. and details of any new or existing incoming services.

o Record or and as-built drawings of the structure, its plant and equipment

(for example, the means of safe access to and from service voids, fire doors and compartmentalisation, etc.) produced throughout the construction process along with the related design criteria.

o Inspection, installation and test certificates and commissioning test results.

o The CDM Co-ordinator may ask for permission to audit this material and

system at any time during the construction phase and such permission is not to be unreasonably withheld.

o On larger and more complex projects, this information is to be forwarded to

the CDM Co-ordinator on completion of the work stages or elements.
