A flurry of activity in Washington and beyond leaves home health care in a unique, but still

challenging and exciting position as we move forward into 2016. Along with the continuation of the Alliance’s Future of Home Health Project, 2015 saw organizational opportunities to build partnerships with peers across the care continuum on a variety of educational, research, and regulatory initiatives.

In late 2015, the Alliance announced a partnership with the Visiting Nurse Associations of America (VNAA) in an effort to advance the missions of both organizations by aligning research and educational initiatives, sharing office space (which the Alliance moved into in early 2016), and providing reciprocal membership benefits. This exciting opportunity will strengthen efforts to gain increased recognition for the value of home health care in our nation’s health care system.

The Alliance also welcomed new board leadership. In November, Kathleen Gilmartin, President and CEO of Interim HealthCare, was elected chairman of the Alliance’s Board of Directors. The Alliance would like to thank Dr. Steven Landers for his incredible dedication and leadership as the Alliance’s Board chairman for three years. Dr. Landers will remain on the Alliance’s Executive Committee and Board where he was elected Vice-Chairman for the upcoming term. Marcia Reissig, CEO of Sutter Care at Home, will stay on as Secretary, and J. Mark Baiada, Founder and President of BAYADA Home Health Care, will remain in his role as Treasurer. The Alliance would also like to welcome Keith Myers, CEO of LHC Group to the Executive Committee. Mr. Myers was elected to the Alliance’s Board of Directors and Executive Committee in late 2015.

Included in the 2015 Annual Report is further information about the Alliance’s leadership, member organizations, financial performance, accomplishments and goals for 2016. Within the Future of Home Health Project and beyond, the Alliance established partnerships with a wide variety of organizations and individuals integral to the successful delivery of quality home health care in the future. Connections were made through Future of Home Health Project symposia, educational outreach on webinars, and via social media and the monthly #HomeHealthChat on Twitter, on topics ranging from caregiving to health information technology, new models such as Accountable Care Organizations and bundled payments, quality metrics, and more.

As the Alliance looks ahead, the future of home health care will remain at the forefront of Alliance activity, both within the project and in additional endeavors. Planned 2016 initiatives include a webinar series on value-based purchasing jointly-sponsored with the VNAA, data analysis relating to the Comprehensive Care for Joint Replacement Model, state-focused data analyses and dissemination, and the release of the final Future of Home Health Project strategic framework accompanied by more regional events and a briefing in Washington.

Thank you all for your support of the Alliance, our mission, and our endeavors in 2015. We are excited to continue the conversation on the future of home health care, the changing health care landscape, and ways to improve person-centered care, as well as quality and efficiency for the health care system.

FUTURE OF HOME HEALTH PROJECTIn 2015, the Future of Home Health Project with three symposia on the future of home health,

and the beginning of work to construct the research-based strategic framework.

NATIONAL SYMPOSIUMIn January, the Alliance hosted the Future of

Home Health Symposium: Tackling Critical

Issues for the Future of Home and Community

Based Care. Held in Washington, DC, the

symposium focused on key issues that need

to be tackled in order to achieve a health

care system shift towards community and

home-based care.. Speakers from across the

care continuum, including those from the

United Hospital Fund, naviHealth, and more

touched on the ways to bring care to the home

and community in the future. Brian Griffin,

President & CEO of Empire BlueCross BlueShield

delivered the keynote on perspectives

from a commercial payer on the future.

REGIONAL SYMPOSIAFollowing the success of the national

symposium in Washington, DC, the

Alliance hosted two regional symposia on

the Future of Home Health Care in New

Orleans and New York City. Both events

featured an overview of the IOM workshop,

with additional content and speakers.

At the Southwest Regional symposia in

New Orleans, speakers from Amedisys,

Kindred, VCU Medical Center, and PACCR

spoke on new models of care delivery and

preparing for the future. In New York,

speakers from Geisinger, The Hartford

Foundation, the Eldercare Workforce

Alliance, and more spoke to policy and

research needs, critical factors in shifting

care to the home and community, and

alternative models of health care delivery.

VNAA PARTNERSHIPLate in 2015, the Alliance announced a partnership with the Visiting Nurse

Associations of America to align and advance their respective missions, improve

policymaker and public understanding of home health care and community-based care,

and invest in research and data analysis to support quality and innovation in health care.

By sharing resources and knowledge, the VNAA and the Alliance will enhance their efforts

to ensure that the role of home health care is recognized in our nation’s health care system

and appropriately utilized so that patients are receiving the right care in the right setting.

ALLIANCE LEARNING COLLABORATIVE SERIESFive new Learning Collaborative webinars

were held in 2015, ranging in topic from legal

and regulatory developments to clinical

decision-making and more.

Health Project, spotlight the vital importance of

home health care. The Alliance’s work helps to both

highlight the ways in which home health is currently

utilized, and how it benefits the entire health care

CHARTBOOK UPDATEIn addition to updating the Home Health Chartbook for 2015, the Alliance worked

with Avalere Health to provide data compilations on: Post-Discharge Destinations,

by State; and 30-Day Readmissions from Home Health, by State.

STATE-LEVEL DATAThe Alliance continued to work with state

home care associations in 2015 to identify

areas for research and data development.

These conversations led to three new state-

level Chartbook supplements, updated state

one pagers, and state data packets, and

paved the way for work in 2016.

In 2016, the Alliance will look to wrap up the Future

of Home Health Project with the publication of the

research-based strategic framework on the future of

home health care.

Further collaboration with VNAA will look at aligning

the two organizations to provide reciprocal membership

benefits, organizational efficiencies, and an expanded

scope of programming.

Continued support of research and education through

expanded data analyses, one pagers, further Chartbook

updates for 2016, and programming including webinars

and events focused on vital policy, technology,

and payment updates.
