Concierge Program

We assist those transitioning to home by

providing immediate one-on-one care and

assisting with patient and family needs.

The journey back home can be a seamless

transition, and HBH is here to help.

Our Concierge Program provides clients and their

families with ccoonnffiiddeennccee and ppeeaaccee ooff mmiinndd while

transitioning back to Home.

How We Can Help:

Immediate one-on-one care, emergency assistance

Patient advocacy

Companionship

Rest and respite for client’s family

Assist with client’s needs (eating, drinking, toileting, etc.)

Assist with planning transition to home

Provide transportation to client upon discharge from facility

Transition with client to home upon discharge

Provide Private Duty and/or Home Health Services upon arrival at

home (see next page for more information on these services)

Work with client’s physician and family to carry out plan of care

A transition usually happens when a client’s

condition and/or care needs change. When

this happens, Home Bound Healthcare is

here to assist with all matters related to the

transition.

Whether assisting in the hospital,

coordinating transportation after discharge,

helping at home after leaving the

Emergency Room, or all of the above –

Home Bound Healthcare is here to help.

Transition to Home Checklist – We Have You Covered

Private Duty/Home Care Services: Transportation to and from medical

appointments, pharmacy, and grocery store, etc. Meal preparation, laundry

assistance, light housekeeping, companionship, assistance with activities of daily

living (bathing, dressing, toileting), medication management

Home Health Services: In-home nursing, physical/occupational/speech therapy,

wound care, monitoring, medication management – all covered 100% by Medicare.

DME: Medical equipment such as oxygen, walkers, wheelchairs, hospital beds, etc.

We have packages and rates available to accommodate your unique situation.

Transportation home from medical facility (hospital, emergency room,

outpatient clinics, nursing homes, cardiac units, oncology units, etc.)

Emergency Assistance Packages

1 or 3 day live-in packages or ongoing live-in assistance

Intermittent Service Plans: a few hours a day, a few hours a week – we can

help develop a plan to suit your needs

Home Health Services – Medicare Covered

Our Home Health Services include in-home Nursing, Physical Therapy, Occupational

Therapy, Speech Therapy, Psychiatric Nursing, TeleHealth, Social Work, Home Health

Aides, Wound Care, Bathing Assistance, and Medication Management.

The Home Health Services received will be based upon what the client’s physician

orders. Medicare requires clients to be homebound in order to receive services.

Medicare’s definition of homebound states that it must require a taxing effort for the

client to leave the home.

Home Bound Healthcare (HBH) is a home

care agency that was founded with love by

two Registered Nurses in 2004. We offer a

broad spectrum of services to our clients

including Home Health, Private Duty, DME,

Hospice, Psychiatric Nursing, and TeleHealth.

With a mission that is dedicated to the

wellness of our clients, we have grown to

become a successful company that employs

over 500 employees and has serviced

thousands of clients and their families.

We are “A +” Rated Members of the Better

Business Bureau The Better Business Bureau grades each of its

members according to its letter grading

system. The BBB letter grades represent the

BBB’s opinion of the business. BBB assigns

letter grades from A+ (highest) to F (lowest).

The grades are awarded based upon the

following following: business’ complaint history with BBB, type of business, length of time in

business, background information, honoring commitments to BBB, licensing and

government actions, etc. Home Bound Healthcare is pleased to have an “A +” rating!
