1). On or Before Tuesday May 20, 2014 Please email the ENTRY FORM to let us know in what division you like to participate and how many teams.

2). Coaches Meeting Registration fee/Guidelines/Groupings/ Date: May 21, 2014 Time: 1:00 pm Place: ASCOM Grandstand

Registration and payment of fees will be accepted in this meeting. First come first serve basis.

3). REGISTRATION FEES: YOUTH DIVISIONS : Php 2,500 per team MEN’s AND LADIES OPEN : Php 3,000 per team

Note: Acceptance to the festival depends on the availability of the slots and playing fields.

4). Deadline of Entries and Registration fees of teams will be on May 21, 2014. You can deposit Bank of Philippines Islands. Savings account # 8369- 2997-36.

WAIVER FORM

In consideration of accepting this entry, I hereby waive all claims against the organizers of Summer Football festival, ASCOM football field NCOIC, and all officials of the tournaments, from any liability in connection with this activity and for any injury that may occur while competing or watching in this event.

Further, we grant full permission for Sponsors and organizers to include Pictures and Videos of my team and players in any legitimate account in promotion thereof.
