Children’s Zippered Outerwear By Old Navy

Date of the recall: June 8, 2004

This recall concerns children’s outerwear

garments with a clear, oval-shaped zipper pull.

They were sold in sizes of 6 months up to 4T.

The style numbers are printed on a white label

sewn into the garment.

The label is located under the garment-care tag. The garments were sold at Old Navy Stores

and at their website from November 2003 through May 2004 from between $14 and $34.

The plastic zipper pull can come off, creating a

choking hazard.

There have been 13 reports of the zipper pulls

coming off, including one report of a child having it in

her mouth. No injuries have been reported.

What should you do?

Return the garments to any Old Navy store for a

refund and complimentary $5 gift card.

Chamois Blankets by Pottery Barn Kids

Date of the recall: January 24, 2005

These blankets are made of 100% polyester. They were sold in yellow, pink, blue, and green

and the reverse side of the blanket is always white.

Decorative yarn in a coordinating color is stitched around the blanket’s edge. The blanket

measures 30 inches wide by 40 inches long and is labeled for stroller use. A label on the

blanket includes the phrases “pottery barn kids” and “Made in Korea”.

The blankets were sold at Pottery Barn Kids’ stores nationwide, through the catalog and online

from December 2002 through December 2004 for about $30.

Reason for the recall:

The decorative stitching around the edge can come

loose, allowing a child to become entangled with is a

strangulation hazard.

There have been four reports of the stitching

coming loose, no injuries have been reported.

What should you do?

Remove the decorative stitching.

Outerwear by Lands’ End

Date of the recall: March 1, 2005

The recalled outerwear has a rubber tab loca ted at the end of the fabric

zipper pull located on the front of the jacket. Those being recalled are listed

below. The style number is written on a label in the left side seam of the

garment.

They were sold through the Lands’ End catalog and online from August 2004

through December 21, 2004 for between $54.50 and $79.50.

The rubber zipper pull-tab can be bitten off, creating a choking hazard. There have been four reports of the tab being chewed off, all without injury.

Arizona Boys’ Cardigan Sweater by JC Penny

Date of the recall: March 4, 2005

The recalled sweaters were sold in toddler sizes (2T to 5T)

and infant sizes (12M to 24M). They are sold in the colors

“red barn” and “navy”.

They have an Arizona Jean Company tag sewn in the

neckline, which includes the phrases “RN Number 93677”

and “Made in Israel”. They were sold in JC Penny stores

nationwide from September 2004 through January 2005

for $15 to $17.

Reason for the recall:

The zipper-pull can come off of the zipper, creating a choking hazard.

There has been one report of a toddler removing the zipper-pull and placing it in his

mouth. No injuries have been reported.

What should you do?

Stop using the recalled bodysuits immediately.

Return them to a JC Penny store for a refund.

Okie Dokie Bodysuits by JC Penny

Date of the recall: April 27, 2005

These multicolored bodysuits are 100% cotton and

have a heat-sealed appliqué on the front and

attached to the side. They were sold in newborn

sizes (0-9 months) and infant sizes (12-24 months).

The phrase “Okie Dokie” is sewn in the neckline and

“Made in China”.

They were sold at JC Penny stores and online from

October 2004 through February 2005 for about $6.

Reason for the recall:

The appliqué can fall off of the garment, creating a choking hazard.

There have been three reports of the appliqué detaching, but no injuries.

What should you do?

Stop using the recalled bodysuits immediately.

Return them to a JC Penny store for a refund.

Denim and Knit Jackets by H&M

Date of recall: July 26, 2005

The recalled jackets are light blue denim with two breast pockets,

a gray knit hood and gray knit sleeves. The buttons have the

phrase “BABY” written on them. A red and white “A” patch is

sewn on the front, a blue “3” is on one sleeve and a red football is

on the other.

This recall involves those jackets in sizes 2 months through 18

months. Style number 432740 is written on the laundry care tag

inside the jacket. “H&M” is written on the collar label.

The metal buttons can come loose and detach from the garment, creating a choking

hazard.

There has been one report of the metal buttons detaching, but no injuries.

What should you do?

Caroline Flower Baby Sandals by Gymboree

Date of the recall: August 12, 2005

The recalled sandal has a SKU number of 140002069 and were sold

in sizes 1 to 8. The SKU number is printed on the price tag above the

bar code. The sandal has a white upper sold and cork covered outer

sole.

A white plastic flower is attached to the top of the front strap. The

front and back straps have adjustable touch fasteners. The phrase

“Gymboree” is printed in aqua on the sole. They were sold at

Gymboree stores and online from May 2005 through July 2005 for

between $22 and $28 each.

Reason for the recall:

The plastic flower can be pulled or snapped off,

creating a choking hazard.

There have been four reports of the flower coming

off. One child pulled the flower off, placed it in her

mouth and started to choke. Another child was

reported to have bitten a flower off of the sandal.

What should you do?

Take the sandals away immediately.

Return them to the store for a full refund or contact Gymboree (877-449-6932) for

return instructions.

Infant Booties by LL Bean

Date of the recall: February 7, 2007

These booties are sold in infant sizes 1-6. The booties

were sold in a tan or pink suede. They have rubber soles

and zipper closure at the ank e. They are lined with

crème-colored faux shearling acrylic/polyester fleece.

The phrase “L.L. Bean” is printed in white letters on the

sol es of the booties. The size and stock number (BNK1)

are printed on a tag inside.

The booties were sold through the L.L. Bean catalog,

online, and retail stores in Maine, New Hampshire, Massachusetts, New Jersey, Pennsylvania,

Maryland and Virginia from July 2006 through December 2006 for about $16.50.

Reason for the recall:

The small metal zipper tab can detach and be choked on.

There have been six reports of the tabs coming off. No injuries have been reported.

What should you do?

Take the booties away immediately.

Lil’ Snugglers Children’s Blankets

Date of the recall: May 20, 2008

These recalled blankets are 14 inches square with an animal head sewn in the middle. The satin

border around the outside edge is about one inch wide. Only blankets with a PO number of

1330 or below are being recalled. You can find the style number and PO number (which follows

the style number) on a white tab sewn into the blankets behind the Douglas red label.

The blankets were sold at specialty stores nationwide and online from April 2005 through

December 2007 for about $12.

Reason for the recall:

The blanket’s satin edge can come loose, creating a strangulation hazard.

There have been 18 reports of the satin separating from the blanket, but no injuries.

Camouflage Pajama Sets by The Children’s Place

Date of the recall: June 10, 2008

These recalled pajama sets involve both long and short-sleeved

pajamas. They have a blue shirt with a red screen print that says

“Athletics 90” with matching camouflage pants. They were sold in

boys’ sizes XXS (2/3) to XL (14).

They were sold exclusively at The Children’s Place from December

2006 until January 2008 for between $15 and $17 each.

Reason for the recall:

The screen print on the front of the shirt has too much lead. If it were to crack and be

ingested by a child it could seriously hurt their development and brain functions.

What should you do?

Immediately take the sets back to any Children’s Place store for a full refund.

Overalls by Sara Lynn Togs

Date of the recall: June 12, 2008

This recall involves Infant and Toddler Gingham and

Flannel Shortalls. Shortalls are overalls with shorts

instead of long pant legs. They have a collegiate

appliqué or embroidery on the front. The sizes

affected are 6/9 month, 12 month, 18 month, 2T, 3T,

and 4T.

The overalls were sold at college bookstores, fan

stores, children’s boutiques and other gift shops

from March 2007 through February 2008. The Gingham Shortall sold for around $35 and the

Flannel Shortall for nearly $24.

This recall involves Cadence-Lea and Trio-Lea Girl’s Sandals.

They are designed for little girls, sizes 5-12. However, they

are included in this report because the detaching pieces are

a danger for any infants and babies in the home.

The sandals are leather with attached leather flowers. The

Cadence-Lea sandals are white, light green, or silver. The Trio-Lea sandals are white or silver.

“Nordstrom” is printed on the sandals. T hey were sold at Nordstrom stores nationwide from

March 2008 through May 2008 for about $30 each.

Reason for the recall:

The flower embellishments on the sandals can

come off, posing a choking hazard for young

children.

What should you do?

Take the sandals away immediately and return

them to any Nordstrom store for a full refund.

Date of the recall: June 27, 2008

The recalled jackets and hoodies have a drawstring through the hood. They were sold in sizes 2

through 12. The jackets come in tan, stone, bayou blue, crisp blue, white, berry, and citrus.

The hoodies were sold in teal, kiwi, mandarin, aloe green, paprika, hibiscus, smokey blue,

white, sherbet, cobalt and stone.

The Hoodies were sold at Coolibar’s catalog and online store from March 2005 until May 2008.

The jackets were sold from February 2008 until May 2008. They were sold between $25 and

$40.

Reason for the recall:

The drawstring in the hood is a strangulation hazard to children.

What should you do?

Immediately remove the drawstring from the jacket or hoodie to eliminate the hazard.

Two Taggies Sleep’n Play style garments are being recalled. They were sold in purple or blue

They were sold in sizes 0-3 months, 3-6 months, and 6-9 months. RN #27829 has been printed

on the care label sewn into the garment. The style numbers have been printed on the hangtag

The shoes are leather with attached leather flowers and were sold under the name “Kids Feet”. They

were either white or white with pink soles. They were sold in girls sizes 5 through 12 at Rack Room

Stores nationwide from February 2008 through June 2008 for around $25.

Reason for the recall:

The ornamental flowers can fall off, creating a choking hazard for younger children. There has

been one report of a flower coming off, but no injuries.

Warning About Carter’s Fall 2007 Clothing

Date of the recall: October 24th

, 2008

The Consumer Product Safety Commission is warning parents that there have been a few reports of

infants and babies developing rashes on their backs after wearing Carter’s clothing with heat-transferred

or “tag-less” labels. In other words, the writing that would be printed on a label, is printed on the

clothing itself instead.

The advisory is regarding the Fall 2007 line of Carters clothing. In this line, there is a label on the inside

back of the garment that has a raised surface with a solid, rather than a stenciled background.

What should you do?

If your child develops a rash after wearing these garments, stop using them immediately. If the

rash persists, contact your pediatrician.

Young Colors Children’s Hooded Jackets

Date of the recall: November 19, 2008

These jackets have drawstrings through the hood and at the waist. They were sold in sizes 12 months

through size 10 in a variety of solid colors and prints. The phrase “Young Colors” is sewn into the jacket

tag at the neck. They were sold at specialty children’s stores nationwide from April 2004 until October

2007 for about $45.

Reason for the recall:

Children can get entangled in the drawstrings that can catch on playground equipment, fences,

or tree branches.

From 1985 to June 2008 there have been 27 deaths and 70 non-fatal accidents involving the

entanglement of children’s clothing drawstrings.

What should you do?

Immediately remove the drawstrings from the jacket.

Arizona Newborn and Infant Pants

Date of the recall: November 25th

, 2008

The recalled pants are Arizona brand newborn and infant girl’s and boy’s denim pants. They were sold

in sizes 0-3 months, 3-6 months, and 6-9 months. Only those pants made in Bangladesh are being

recalled. Pants made in China are NOT being recalled. You can find the country of origin on a label sewn

into the waistband.

These pants were sold at JCPenny stores nationwide and online from July 2008 until November 2008 for

about $15.

Date of the recall: January 12, 2009

This is an expansion of an earlier recall for Taggies Sleep ‘n Play Infant garments. That recall has been

expanded to include the Dinosaur Applique and the Pink Toss print styles. These garments are one-

piece footed coveralls with snaps down the front. They were sold in sizes 0-3 months, 3-6 months, and

6-9 months. The number “RN #27829” is printed on the care label sewn into the side of the garment.

The style numbers below are printed on the hangtag at the neck.

Children’s Flip Flops from Havainas

Date of the recall: March 3rd

, 2009

The recalled flip flops involved the Havaianas brand and were sold under the following model

names: Baby Estampas, Baby Pets, Kids Apple, Kids Fairy, Kids Flores, Kids Lighthouse, Kids

Monsters, Kids Surf, Baby Letrinhas, Kids Sports, Kids Candies, Kids Fun, Kids Love, Kids Sereias,

Kids Speed, Kids Lucky Bug, Kids Pets, Kids Wonder Woman, Kids Small Flowers, Kids Tropical

w/Kit.

Those Havainas brand name flip flops that do not contain decorative paint are not included in

this recall. The flip flops were sold at department and speciality stores nationwide from

November 2006 through February 2009 for between $15 and $24 a pair.

The decorative paint used on the sole of the flip flop has too much lead, which is toxic to

young children.

What should you do?

Immediately take the flip flops away.

Date of the recall: April 9, 2009

T his particular recall involves the Buster Brown &

Co. “CARS” fleece clog children’s shoes. The shoe is

a red plastic molded clog with fleece lining and is

designed to look like a toy car. The recalled shoes

were sold in infant sizes 4-6 and toddler sizes 7-11.

The infant-sized shoes have a strap in the back to wrap

around the heel. Each side of the shoe has two red and

white black decorative wheels and the number “95”. The

phrases “Buster Brown & Co” and “Disney” are printed on

a tag sewn inside the shoe.

The shoes were sold exclusively at Wal-Mart in the United States and Canada from September

2008 through March 2009 for between $6 and $10 (in US) and around $13 (in Canada).

Reason for the recall:

The four decorative wheels on the shoe can come off, creating a choking hazard for

young children.

What should you do?

Take the shoes away immediately.

Return them to the closest Wal-Mart for a full refund.

Baby Gap Children’s Coats

Date of the recall: April 30th

, 2009

This particular recall involves three different styles of children’s coats sold at Baby Gap. These styles are

the Rose Toggle Hoodie (style 600298), Pink Duffle Coat (style 600344), and Blue Plaid and Mojave

Toggle Coat (style 600942). The toggle coats were sold in infant sizes up to 24 months and consist of

cotton, polyester, or polyester/acrylic. The toggles are light tan plastic and about 1.75 inches long.

The coats were sold at babyGap, GapKids, and Gap stores nationwide and online at www.gap.com from

October 2008 through April 2009 for between $20 and $50.

Reason for the recall:

The toggle fasteners can break and detach from the coat, creating a choking hazard.

What should you do?

Beanie Children’s Knitted Ski Hats

The Erwin Beanie is mostly green and white yarn knitted

to look like a frog with white eyeballs and a red smile.

The Poseidon Beanie comes in several colors of yarn and

has hand sewn turtles, whales, and an octopus attached

to the beanie. The firm’s logos “Ambler” or “A” is printed

on a fabric tag. The hats were sold in specialty and ski

shops nationwide from September 2008 through

November 2008 for about $30.

Reason for the recall:

The eyeballs on the Erwin Beanie and the octopus

legs on the Poseidon Beanie come become loose,

potentially creating a choking hazard.

There has been one report of the octopus’ legs

coming off, with no injuries reported.

What should you do?

Immediately stop using the beanies.

Return them to Ambler Mountain for a free

replacement product or cut off the frog’s eyes

and the octopus’ legs and throw them away.

These loungewear sets are described as “Lounge-About-Housewear” or “Short sets”. They

were sold in a variety of styles, colors, and prints. The sizes varied from S (1-2 years), M (2-3

years), L (3-4 years), XL (4-5 years), 6 years, 7 years, and 8 years.

All the garments have label sewn on the inside that says “These garments do NOT comply with

the Federal Flammability Standards and are therefore not suitable for sleepwear.”

Reason for the recall:

The garments fail the federal flammability standards for

children’s sleepwear. These standards require sleepwear

The recalled clogs are plastic molded EVA “water” clogs and are designed to resemble a car.

The shoes come in red, brown, blue, yellow, and pink. Most bear the phrase “CARS”, “T

ransformers Animated,” or “Barbie”.

“Buster Brown & Co.” is printed on the shoe, inside the heel, on a sewn-in tag inside the fleece-

lined shoe, as well as on the original hang tag. The model number can also be found on these

tags. (See pictures below for model numbers.) They were sold in sizes 5 – 13 and youth sizes 1

– 3.

The shoes were sold at major retailers like Bealls’, Famous Footwear, JC Penny, Meijer, Rack

Room, Sears, Shoe Show, Target, and Wal-Mart from August 2007through June 2009 for

between $10 and $25.

This recall is an expansion of the “CARS” shoes previously recalled in April and sold at Wal-Mart.

Nearly 1.4 million pairs are being recalled.

Reason for the recall:

The shoe has decorative wheels that can come off and create a choking hazard.

Two faux pom poms are attached to the end of a drawstring that hang from the cape. The
