FEEDBACK MECHANISM:COMMENDATION:

1 Write or call the customer to validate the deficiency/ies or flaws2-a. Analyze, study and make the necessary recommendations for corrective measures to address the defficiency/ies or flaws.

2-b. In case of personnel deficiency, refer the matter to the Administrative Officer for corrective actions.

3-a. Make recommendatons to the Head of Office for his action on the proposed of corrective measures to be undertaken.

3-b. Administrative Officer may write or call the complainant as to the corrective measures taken.

Administrative officer shall make a report to the Head of Office as to the actions taken.

4 Head of Office shall issue the necessary office order to address the defficiency/ies or flaws.

VIII.b. PROCEDURES IN FILING A COMPLAINT (against erring NTC personnel)1) File a written complaint2) Have the letter formally received3) Forward immediately the written complaint to the office of the Regional Director4) The Regional Director shall forward the complaint to the grievance committee.5) The grievance committee shall schedule a formal hearing to determine probable cause of whether the complaint is meritorious or not.6) The erring personnel shall be required to submit a written answer to the complaint.

TIME FRAME:1) Receiving Time - 5 minutes2) Investigation of Complaint - 1 to 30 days depending on the severity
