All participants must …Fill out ALL areas of the form (name, base school, hours, check mark for banking and SIGN it.)

Presenter must…1.Sign ALL certificates.2.PRESENTER KEEPS PINK COPY ONLY and gives yellow and white copy to participant.

Engagement AssessmentAll participants must …Fill out the Engagement Assessment at the end of your session.

Presenter must…1.Keep the white copy for yourself.2.Put the yellow copy in the presenter envelope.
