Certified Dementia Care Planner (CDCP) Course offered by Institute of Alzheimer’s Education of Hong Kong Alzheimer’s Disease Association CDCP Course is a part-time course, which comprises 4 modules involving 60 hours of lecture and 20 hours of practicum, case discussion and continuous assessment. The maximum study period is one year. The course normally runs quarterly. Through these 80 hours of training, students will be equipped as Certified Dementia Care Planners. I welcome you to the Course on “Certi�ed Dementia Care Planner”, organized by the Institute of Alzheimer’s Education, which is a training facility under the Hong Kong Alzheimer’s Disease Association. The Institute was inaugurated at the Alzheimer’s Disease International (ADI) Regional Conference in Hong Kong in December, 2013.

One of the major training initiatives is an intensive course on planning care for persons with dementia with their family members and di�erent service providers, in order to meet the needs of the individual and the family at di�erent stages of the disease, including crisis management. This is in line with the emphasis of the ADI report 2013 on Advance Care Planning, which should encompass the entire trajectory of the person with dementia (major neurocognitive disorder, DSM-5) including end-of-life preparations.

Hence, the 80-hour course will cover all aspects of care from biological, medical, social, legal, ethical to cultural aspects of care. Participants will receive structured training from renowned and experienced multi-professions in the �eld and will be engaged in practicum work.

A certi�ed dementia care planner will be able to manage directly cases and supervise relevant disciplines in care planning in di�erent care settings including daycare, home care and residential care.

Dementia is the greatest health challenge of our generation which demands concerted e�orts in �ghting the upcoming epidemic. Hong Kong Alzheimer’s Disease Association (HKADA) has therefore established the Institute of Alzheimer’s Education (IAE). IAE is guided by an Advisory Committee comprising multi-disciplinary experts in dementia care. We aim to address the pressing needs for community-wide capacity-building. Through di�erent comprehensive training and educational courses for medical practitioners, healthcare professionals, formal and informal caregivers and the younger generation, IAE seeks to promote standardized quality of dementia care and build a dementia-friendly community.

A Certi�ed Dementia Care Planner is a professional equipped with comprehensive skills and knowledge about dementia care, from understanding the pathology and treatment, assessment and care planning to advance care planning. The Certi�ed Dementia Care Planner can then provide continuity of care walking through the caring journey with families living with dementia, acting as coordinator with di�erent dementia care teams in community, residential and hospital settings.

About the courseCo-developed with the Alzheimer’s Disease International (ADI) and in collaboration with Sau Po Centre on Ageing, The University of Hong Kong, the Certi�ed Dementia Care Planner (CDCP) Course is designed to provide versatile training for the award of the title of Certi�ed Dementia Care Planner. The Course consists of lecture, case study and discussion, practicum and continuous assessment, which ensure the skills transfer from lecture to practice.

Teaching teamMulti-disciplinary professionals including geriatrician/ psychogeriatrician/ psychiatrist, psychologist, lawyer, occupational therapist, nurse and social worker.

Mode of teachingLectures are conducted in Cantonese supplemented by English. Prescribed work will be in the form of assignments, service attachment and group presentation.

1. Enrolment optionsInterested parties may choose to enrol full course or one or more modules tailoring to his/her interests and needs.

2. Entry requirements* Eligible participants in speci�c module(s) or the full course must hold a relevant bachelor’s degree in medicine, nursing,

occupational therapy, physiotherapy, social work, psychology or other health care related courses.* For associate degree and diploma holders, completion of bridging course is required.

3. Assessment, graduation requirements and award of certi�cateIn addition to continuous assessments based on performance in course assignments, participants must complete practicum and exit examination at the end of each module. Practicum arrangements are subject to speci�c module requirements. For the award of the title of Certi�ed Dementia Care Planner, participants must achieve 80% or above for both class and practicum attendance and an overall average mark of at least 50% in the course assignments, practicum, written examinations in all FOUR modules and the exit examination of the course. The same thresholds apply to participants enrolling in speci�c modules for the award of a modular certi�cate of attendance.
