台灣失智症現況

摘 要
台灣社區流行病學調查發現失智症於老年人盛行率是1.7%到4.4%,
而以阿茲海默氏病為最常見的類型。低盛行率可能和華人老人生活較單
純,死亡率相對較高和華人地區APOE4基因盛行率較低有關。台灣失智
病人的行為和精神心理症狀之盛行率和表現與西方國家相似,這也表示
失智病人的行為和精神心理症狀主要是由神經生化因素所決定者。但是
因文化上之差異,對於激動和淡漠的行為解釋上可能會有不同。對於大
多數的台灣失智症病患是由配偶、媳婦和其他家族成年成員提供照顧,
因此照顧者負擔是台灣一逐漸浮現的問題。

前言
聯合國對所謂「高齡化國家」的定義
是老年人口佔全國人口的7%以上即屬之
,台灣在1993年即已跨過7%的門檻,成
為高齡化國家。台灣近年來人口老化速
度非常快,根據內政部截至2007年年底
的最新統計 [1],台灣65歲以上的老年人
口數已高達234萬人,佔全台人口總數的
10.2%。在60歲以上,每增加5歲失智症
盛行率便倍增 [2],因此失智症已成為台
灣重要的社會議題。
台灣目前平均餘命是78歲,其中男性
為75歲,女性為81歲 [1]。於2007年,77%
的年男性是有配偶的,但是只有49%的老
年女性是有配偶的。47%的老年女性是喪

偶的,她們的經濟來源很多需仰賴子女
,因此老年女性需要更多的關注。
一、失智症的流行病學研究
(一)盛行率和發生率
台灣多起社區流行病學調查發現台
灣失智症盛行率於65歲以上的人口分別
為1.7%到4.4%不等(表1) [3-8]。所有這些
研究都發現失智症的盛行率隨著年齡的
增加而遞增,而且女性高於男性。這樣
的盛行率和對岸中國的1.1%到4.6%的數
字接近[9-12],但是比起歐美的5%到10%
的數字為低 [13-17]。不過需注意的是這
些研究都是10幾到20幾年前所作的研究
,和目前實際的盛行率可能已有差距。
至於長期照護機構內的老人中,失智症
盛行率則偏高,曾有養老院27%、安養中
心62%和護理之家65%之調查報告[18]。
對於東西方失智症盛行率的差異,有
以下幾個可能:(1)種族因素,目前已知
APOE4是一重要失智症危險因子,而華
人APOE4比例約為4.8-11.0% [19-22],較
西方國家的9.0% 到16.5%為低 [23-27]
。但是台灣兩起APOE4的研究也同時發
現,APOE4只和阿茲海默氏病有關,而
和其他類型的失智症無關 [28, 29];(2)
華人社區未受教育的老人較多,因此生
活較單純,不易符合DSM-IV中需影響社
交或是工作能力條件[30];(3)台灣地區兩
起失智症社區調查發現兩年死亡率高達
32%和48% [6,31],於醫院的調查則發現
,參與研究的病患於研究起始算起,平
均存活為4.5年[32]。但是65歲以上台灣老
人的失智症發生率為1.3% [30],和中國
的1.2% [33]差不多,也和西方國家1.3%-
2.5% [34-38] 相去不遠。因此盛行率偏低
也可能與死亡率偏高有關。
對於診斷為「可疑失智症」
(questionable dementia) 的病患,每年轉
變成阿茲海默氏病的機會是19.9% [39]
。具APOE4基因、定向和短期記憶得分
較低和焦慮轉變成阿茲海默氏病的機會
較大。
(二)失智症類型
和西方國家相同,台灣的失智症社區
研究發現阿茲海默氏病是當中最常見的
類型 [6-8]。但是兩起醫院的調查卻發現
醫院中求診的病患,血管性失智症的比
例高於阿茲海默氏病 [40, 41]。有一台灣
之社區研究發現女性以阿茲海默氏病居

多,而男性以血管性失智症為主 [8]。社
區調查和醫院調查不同的原因目前仍不
清楚,可能是社區研究沒有將每位診斷
為失智症的病患都進行影像檢查,因此
低估了血管性失智症的盛行率。另外,
也可能是血管性失智症死亡率較高,也
因合併有其他疾病如高血壓、糖尿病等
,而求醫機會較高。
(三)危險因子
和其他國家研究相同,台灣的流行病
學研究發現年齡低教育程度是失智症的
危險因子 [7]。年齡越高,罹患失智症的
機會愈高。未受教育或是低教育程度者
,罹患失智症的機會高。低教育程度是
失智症的危險因子的病因,有兩種學說
。一是因為低教育程度的人可能因生活
型態和環境等不利因素造成他們得病的
機會較高,也有學說認為是因教育可增
加腦子儲存量(brain reserve),而延後或減
少失智症的產生。
除APOE4基因外,台灣阿茲海默氏
病的危險因子調查發現少量的吸菸是保
護因子,而天天吸菸則是危險因子 [42]
。而一近期的流行病學研究發現,中年
時期身體質量指數(body mass index)和將
來失智症之間的關係成J型 [43]。以中年
身體質量指數為20.5到22.9為基準,身體
質量指數太高或是太低都顯現失智症增
加。中年肥胖(身體質量指數≧25.5)於吸
菸的人是阿茲海默氏病和血管性失智症
的危險因子,但是於非吸菸的人,中年
肥胖則和失智症沒有關係。有中風病史
會增加血管性失智症的機會。
二、常用的失智症評估工具
目前台灣所用的失智症評估工具都
來自西方,有些會根據國情不同做了一
些修正,有些則否。一份合乎台灣當地
文化的譯版評估工具對於失智症的診斷
和評估十分重要。過去台灣的老年人,
特別是鄉村地區很多沒有受過教育,但
是自從1943年實施6年國教,1968年實施
9年國教後,目前已大幅減少。根據截至
2007年的最新統計 [1],目前不識字的65
歲以上的老年人是15.6%,其中男性為
4.1%,女性為26.7%。因為一般智能測驗
常受教育程度影響,因此於評估時需對
這些未受教育的老人的結果有不同考量
。以下簡介台灣目前常用的失智症評估
工具:
(一)簡易智能狀態檢查(Mini-Mental State Examination)
在台灣,如同世界很多國家,簡易智
能狀態檢查(MMSE)是最常使用的失智
症篩檢問卷[44]。全球有非常多的語言版
本,而有幾個中文版已經有效度驗證
[45-47]。他們和原始的英文版本十分接
近,但是都做了一些小的修正。如於一
台灣的版本 [46],7個字的中文成語取代
了英文版的「no ifs, ands, or buts」,英文
版的3個步驟中的最後一個動作“把它
交還給我”,用“把它放在地上”取代。中
國的版本[47]則用著名的繞口令「44隻石

獅子」取代「no ifs, ands, or buts」,而用
說一個句子,取代寫一個句子。表面上
好像只有讀寫和教育程度有關,事實上
除了命名和登錄外,簡易智能狀態檢查
上所有項目都和教育有關 [48]。
(二) 老年人認知功能減退知情者問卷 (The Informant Questionnaire on Cognitive Decline in the Elderly, IQCODE)
老年人認知功能減退知情者問卷是
一份26題的問卷 [49, 50],它是詢問受訪
者的親屬他最近10年來的智能變化情況
。如同簡易智能狀態檢查,老年人認知
功能減退知情者問卷也有許多不同的語
言版本,包括台灣版 [50]。此問卷最大
的好處是不受教育程度、性別或是之前
職業的影響,在筆者之前的研究發現
[50],針對教育程度差異甚大的族群,
IQCODE比MMSE更為合適作為一篩檢
工具。
(三) 阿茲海默氏病量表(Alzheimer's Disease Assessment Scale)
阿氏量表是目前美國食品藥物安全
管理局(FDA)對於輕到中度阿茲海默氏
病藥物試驗的標準量表 [51]。台灣版的
阿茲海默氏病量表 [52]將原版中的語言
記憶,改成視覺記憶,使得沒受過教育
的老年人也可以使用。其信效度測驗結
果良好,測試再測試信度可達0.96,而且
其記憶項目在相當早期的失智症即有變
化,而語言項目則可用來追蹤病情變化。
(四) 智能篩檢測驗 (Cognitive Abilities Screening Instrument, CASI)
智能篩檢測驗是結合了簡易智能狀
態檢查 [44]和日本常用的長谷川氏量表
(Hasegawa Scale) [55]所發展出的失智症
量表[53,54]。智能篩檢測驗滿分為100分
,也可分別算出簡易智能狀態檢查和長
谷川氏量表的分數,因此適合作跨文化
研究用。它包含10個認知功能的細項,
分別是長期記憶、短期記憶、定向感、
注意力、心智操作和集中力、語言流利
度、語言能力、抽象思考能力、判斷力
、和繪圖能力,涵蓋的認知領域較廣。
(五) 畫時鐘測驗 (clock drawing test, CDT)
畫時鐘測驗主要是檢視視覺空間結
構(visuospatial construction)和執行功
能,和上述四個全面性檢測認知功能的
測驗有所不同。它的另一個好處是沒有
語言上的隔閡。台灣目前有兩起畫時鐘
測驗的相關研究 [56,57],發現此測驗的
鑑別力中等(敏感性73%,特異性66%)
,也不易區分可疑失智症和輕度失智症
病患。因此要做為大規模失智症篩檢問
卷並不合適。
三、失智症行為和精神心理症狀 (behavioural and psychological symptoms of dementia, BPSD)
失智症病人除了認知功能障礙外,也
常有行為和精神症狀。根據筆者回顧
1990年1月至2005年11月所有台灣有關

失智症行為和精神心理症狀的文章,發
現共有23篇相關文獻 [58]。台灣阿茲海
默氏病的病人約有30%到63% 出現妄想
、幻覺較少,約有21% 到26%。35%到
76%的病人有焦慮,22%到55%的病人有
憂鬱症狀,26%到61%則有睡眠問題。
上述這些盛行率往往因為在不同的
部門,如神經科或是精神科、門診或是
病房、使用的評估工具不同,而有不少
差異。但是整體而言,東西方國家失智
症行為和精神心理症狀盛行率差距不大
。世界各國的研究都報告「狂喜」是失
智病人最少見的症狀,這表示失智症行
為和精神心理症狀是一個神經生物學上
的變化所決定的症狀。而這些症狀也可
能受到基因的影響,如帶有APOE4的病
人較易出現幻覺和妄想 [59]。
然而,少數失智症行為和精神心理症
狀的確因為文化的差異而有所不同。在
一跨文化比較美國、台灣和香港的失智
症病人行為和精神心理症狀研究中發現
,華裔失智病患比美國有較多的焦慮和
妄想症狀,但是美國的失智症病人有較
多的淡漠症狀 [60]。筆者於台灣所進行
的失智症病人行為和精神心理症狀研究
發現,台灣失智病人有淡漠症狀的有44%
,而美國的研究則有72%失智病人有此症
狀 [61, 62]。這可能是不同文化間對於淡
漠所持的標準不一的關係。台灣文化比
較不認為老年人較為安靜或是退縮是一
項非常負面的行為。因此不同的地區可
能對於有些精神心理症狀的標準不同,
造成盛行率不同。筆者所進行的一項跨
文化比較台灣、美國和義大利的失智症
病人行為和精神心理症狀研究發現,雖
這三個族群的失智症病患激動症狀的盛
行率都很高,但是於台灣失智病人激動
症狀和幻想顯著相關,但是於義大利病
人則和淡漠相關,美國病人和暴躁易怒
有關 [63]。這顯示表面上看來一樣是激
動的症狀徵候,在不同的文化下造成的
原因常常不同,因此文化和環境因素也
於失智症病人行為和精神心理症狀扮演
一重要角色。
有些人認為東方社會對老年人記憶
力減退較傾向認為這是一件自然的事情
,比較不認為是疾病。是否因此台灣地
區的失智症病患求醫的時間會比較晚?
或是只有精神心理症狀出現時,他們才
會求醫?根據前述的台灣香港和美國的
跨文化研究發現 [60],三個地區失智病
人求醫時疾病嚴重程度並無太大差異。
四、失智症的治療
目前美國食品藥物安全管理局核定
有三種乙醯膽鹼酶抑制劑藥物:
donepezil (Aricept®,愛憶欣),1998年經
台灣核准使用; rivastigmine (Exelon®
,憶思能) 2000年核准使用;
galantamine (Reminyl®,利憶靈) 2002年
和一種NMDA受體拮抗劑,memantine
(Ebixa®,憶必佳,Witgen®威智) 2004年
均於台灣上市。目前衛生署所核定的適
應症,三種乙醯膽鹼酶抑制劑藥物均為
針對輕到中度阿茲海默氏病症,而
NMDA受體拮抗劑則為中到重度阿茲海
默氏病症。健保局目前的藥品給付規定

,對於初次使用的病患,神經科或精神
科醫師需事前申請經健保局核准後方得
使用此類藥品;而對於續用的病患,則
需在使用後每6個月重新評估,由專科醫
師以簡易智能測驗或臨床失智量表
(Clinical Dementia Rating Scale, CDR)追
蹤病患的智能狀態,若測驗顯示病患的
智能較起始治療時退步,則應停用藥品
。若申請案被健保局駁回,亦即不同意
給付使用該藥品,不服核定結果的申請
人可將此爭議案上呈至「全民健康保險
爭議審議委員會(爭審會)」審議。申請
初次使用案件之核准率約為61%,而申請
繼續使用案件則為86.9% [64]。被健保局
核定不同意給付使用的案件上呈至爭審
會申請審議高達九成被駁回,根據成功
大學依據健保資料庫所做的分析發現,
這些爭議的原因都是由於申請人與審查
人對於藥品給付規定的解釋缺乏共識所
造成 [64]。而根據另一起研究則發現,
於現行健保規範下只有9.6%的失智病人
能持續用藥超過3年,而平均用藥時間則
為14個月 [65]。
恩主公醫院曾於89位輕到中度阿茲
海默氏病患身上進行一隨機雙盲安慰劑
對照試驗,發現於乙醯膽鹼酶抑制劑藥
物外加上500mg維他命B12和多種維他命
26週,雖然可以降低同半胱胺酸
(homocysteine)濃度,但是其智能和日常
生活功能卻沒有顯著與對照組不同 [66]
。台北榮總的另一項50名女性阿茲海默
氏病患的隨機雙盲安慰劑對照雌激素藥
物試驗,則未能證實使用雌激素12週可
帶來臨床療效[67]。
五、失智症照顧
西方國家,失智症病人約有50%住在
長期照護機構 [68],但是台灣的失智症
病人有很高的比例是於家中照顧。傳統
文化強調孝順的美德,這個觀念使得老
年人得到較好的照顧,但是也使得照顧
者,特別是家中的媳婦,有著沉重的負
擔 [69]。根據筆者過去的研究發現,媳
婦是所有照顧者中憂鬱量表分數最高的
一群,配偶照顧者次之 [70]。不同身分
的照顧者分別面對不同難題。
在西方,照顧者對於失智病人的憂鬱
症狀最為苦惱 [71],但是於台灣照顧者
最擔心病人的破壞行為 [72]。照顧者的
負擔會隨著時間而加重 [73]。教育程度
低的照顧者不知道如何尋找社會資源幫
助,往往負擔較重 [73]。自1993年長期
照護機構立法以後,台灣的照顧者對於
長期照護機構的接受度和西方相當,有
28%的照顧者想把病人送到長期照護機
構 [74],特別是於都會區 [75]。目前已
有不少民間社團和政府的措施來協助減
輕失智的照顧負擔。
六、結論
隨著人口的老化,失智症已成為台灣
醫學界一重要問題,也進行了不少研究
。我們需要用合乎本地文化的評估工具
,也需要發展出本地的照顧模式。對於
藥物治療的規範,醫界仍需努力達到共
識。

Dementia in Taiwan: Current Status

Abstract
Studies on Taiwan population have indicated that the prevalence of
dementia is approximately 1.7~4.4% among elderly people, and Alzheimer’s
disease (AD) has been identified as the most common type of dementia. The
lower prevalence rate might be due to the simple lifestyles of
Chinese/Taiwanese elderly, higher selective mortality, and lower prevalence
of the APOE4 allele in the Chinese/Taiwanese population. The prevalence
and clinical manifestations of behavioral and psychological symptoms of
dementia (BPSD) in Taiwan are similar to those presented in Western reports,
suggesting that most of BPSD are neurobiologically determined. Owing to
different cultural backgrounds, the interpretation of agitation and apathy
might differ. Dementia patients in Taiwan are mostly taken care by their
spouses, daughters-in-law, or other adult family members, and this burden of
care may become a serious problem in Taiwan.
