BOARD OF DIRECTORS
A MESSAGE FROM OUR CEO
CathySwanson,MPHChiefExecutiveOfficer
Latelastyear,CaldwellHospiceand
PalliativeCare(CHPC)markeda
milestoneinourservicetothecommu-
nity.Weservedour10,000thpatient.
Thatfigurerepresentsatremendous
numberofindividualsandfamily
memberswhohavebeentouchedby
theworkandserviceofCaldwell
County’smostexperiencedandonly
local,not-for-profithospice.Italso
servesastestamenttoanincredible
levelofsupportfromacommunity
thathasbeenwithussincethevery
beginningnearly35yearsago.
CHPCbeganasalocal,not-for-profit,
community-basedhospiceprovider
andhasremainedsothroughthe
decades.Despitechangesinthe
healthcaremarketthathaveresulted
inmorefor-profithospicesandamove
awayfromcommunity-drivenwork,
CHPCiscommittedtostayingtrueto
itsoriginalmissionofservinganyone
whoneedsanddesiresourcare.
CaldwellHospicecouldnotexist
withoutyou—thepeoplewhohave
sogenerouslysupporteduswith
monetarycontributionsandvolunteer
hoursaswellasawealthofgoodwill
andkindnessthroughtheyears.
In2015-16,yourcontributions
helpedCHPCprovide$730,339in
caretothoseinourcommunitywho
neededend-of-lifecareandservices
buthadnowaytopayforthem.
Duringthatsameyear,volunteers
gavealmost12,500hoursofservice
toourhospice.Wearehumbledand
honoredbyyourgiving.
It is important for you to know we are
beinggoodstewardsoftheworkand
supportyouhaveentrustedwithus.
Westrivedailytoprovidethebest
possiblecaretoourpatientsandthe
peoplewholovethem,addressingthe
physical,emotionalandspiritualneeds
uniquetoend-of-lifecare.Weseekto
givebacktothecommunitythrough
theofferingofbereavementservicesto
anyoneinthecommunitywhoneeds
them,notjusthospicepatientsand
theirfamilies,aswellaseducational
opportunitiestothecommunity.And,
weareworkingtokeepourhospiceon
astrongfootinginanever-changing
healthcaremarketplace.
Thestaffandboardremaindiligentin
theiroversighttoensurethatCaldwell
Hospiceremainsfinanciallystrongand
steadyaswemeetthechallengesof
thisevolvingera.Amidstitall,CHPCis
committedtothecommunityitserves,
qualitypatientcare,andthenot-for-
profitmodelthatallowsittoprovide
caretoallpatients,regardlessof
reimbursement.
Whileothercommunitiesaroundthe
countryarelosingtheirnonprofit
hospices,CHPCremainssolid,another
testamenttothesupportandfaithof
thecommunityweserve—your
community,ourcommunity.

oundationsmatter.Theyarethebedrock,thestructurethat
supportsallthatistocome,anditisafoundationofministryandservicethatdistinguishesCaldwellHospiceandPalliativeCare(CHPC)andtheworkitdoeseveryday.
CHPCbegannearly35yearsagowithanunderpinningofservingothers—afoundationandpromisethatremainssteadyandstrongalltheseyearslaterandstilldrivestheworkofthecounty’sonlynot-for-profitcommunityhospice.
Whilefor-profitcorporationshavegottenintothehospicebusiness—infact,70%oftoday’shospiceprovidersintheU.S.arefor-profit—thenot-for-profitcommunityhospicemodelisbuiltnotonafinancialmotive,butcarefirst,amissionthatisexemplifieddailybythemanywhoworkandvolunteerforCaldwellHospice.Theideawasandisasimpleone—neighborshelpingneighborsthroughthedifficultdaysofend-of-life,acommunitycaringforitsown,servingindividualsandfamiliesintheirtimeofneed,personalizedcarethattreatspatientsasawholeperson,addressingtheirphysical,emotionalandspiritualneedsandbeingasupporttothosewholovethem.
Itseemslikeacommonideanow,butintheearly1980s,itwasarevolutionaryapproachthatwasfueledbyatruegrassrootsmovementinCaldwellCountytoestablishthecounty’sfirsthospice.
“Ourstruly isajointeffort—Caldwell Hospiceandthecommunitycomingtogethertocareforoneanother,”saysCathySwanson,chiefexecutiveofficer.
F Nowservingabout800patientsayear,CaldwellHospiceisaleaderamongnot-for-profithospicesstate-wide.ItoperatesNorthCarolina’soldestpatientcareunitwithitsKirkwoodfacilityinLenoir.Whilethemajorityofpatientsreceivecareathome,CaldwellHospicealsohasasecondpatientcareunitattheRobbinsCenterinHudson.
CHPCprovidesphysicianandnursingservices,socialworkservices,spiritualandbereavementcounseling,anddietarycounseling.Medicalsupplies,medicationsforsymptomcontrolandpainrelief,homehealthaideandhomemakerservices,volunteerservices,andphysical,occupationalandspeechtherapiesarealsoofferedaswellasenhancedservicesincludingcardiacandpulmonarycareathomeforpatientswithheart/lungdisease,adementiacareprogramandafullcomplementofcommunitybereave-mentservices,whichareopentoanyonewhoneedsthem,notjusthospicepatientsandfamilies.
Inaddition,CaldwellHospiceoffersapalliativecareprogramcalledAIM:AdvancedIllnessManagement—forthosewhoneedassistancemanagingachronic,potentiallylife-limitingillnessthatisnotterminal.
Today,hospicecareiscoveredbyMedicare,Medicaidandmostprivateinsurances.Still,therearemanywhohavenowaytopayfortheircare,andthatiswherethecommunityshinesinCaldwellHospicethroughdonations,volunteerserviceandothercontributions.
CaldwellHospiceneverturnsawayapatientbecauseheorshecannotpay,andthatpatientreceivesthesamelevelofcareasanyoneelse.
“Wecareforanyonewhoneedscareandcompassionastheyandtheirlovedonesfacetheendoflife.Asanon-profit,thatisourwork,”saysSwanson.“Servicetoourfellowmanhasalwaysbeenandremainsourmission.Itisthefoundationofwhoweare.”
Selectingamedicalproviderforalovedoneisanimportantdecision,andhospiceisnoexception.Withfor-profitcompetitors,suchasPruittHealth,CommunityHomeCareandHospice,andMediHome,andnon-profitcompetitorsservingresidentsinourcommunity,itisimportanttolearnabouteachhospiceproviderbeforemakingachoice.
By asking meaningful questions, such as the ones below, you can know what eachhospiceprovideroffersandwhichproviderisrightforyouoryourlovedone.

Until There Is a Cure...
Take the Tour®
Quality care for people dealing
with Alzheimer’s disease and
related dementias begins with
family members and caregivers
who are sensitive, patient, and
kind. But how can people be
empathetic when they have
never experienced firsthand the
physical and mental challenges
facing those with dementia?
Now there is a way.
TheVirtual Dementia Tour®
(VDT)—ascientificallyproven
methodofbuildingagreater
understandingofdementia
throughtheuseofpatented
sensorytoolsandinstruction—
isYourWindowintoTheirWorld.
The VDT was created by P.K. Beville, an
award-winning geriatric specialist and founder
of Second Wind Dreams®.
