A MESSAGE FROM OUR CEO

Cathy�Swanson,�MPHChief�Executive�Officer

Late�last�year,�Caldwell�Hospice�and

Palliative�Care�(CHPC)�marked�a

milestone�in�our�service�to�the�commu-

nity.�We�served�our�10,000th�patient.

That�figure�represents�a�tremendous

number�of�individuals�and�family

members�who�have�been�touched�by

the�work�and�service�of�Caldwell

County’s�most�experienced�and�only

local,�not-for-profit�hospice.�It�also

serves�as�testament�to�an�incredible

level�of�support�from�a�community�

that�has�been�with�us�since�the�very

beginning�nearly�35�years�ago.

CHPC�began�as�a�local,�not-for-profit,

community-based�hospice�provider

and�has�remained�so�through�the

decades.�Despite�changes�in�the

healthcare�market�that�have�resulted

in�more�for-profit�hospices�and�a�move

away�from�community-driven�work,

CHPC�is�committed�to�staying�true�to

its�original�mission�of�serving�anyone

who�needs�and�desires�our�care.

Caldwell�Hospice�could�not�exist

without�you—the�people�who�have�

so�generously�supported�us�with

monetary�contributions�and�volunteer

hours�as�well�as�a�wealth�of�good�will

and�kindness�through�the�years.

In�2015-16,�your�contributions�

helped�CHPC�provide�$730,339�in�

care�to�those�in�our�community�who

needed�end-of-life�care�and�services

but�had�no�way�to�pay�for�them.

During�that�same�year,�volunteers

gave�almost�12,500�hours�of�service�

to�our�hospice.�We�are�humbled�and�

honored�by�your�giving.

It is important for you to know we are

being�good�stewards�of�the�work�and

support�you�have�entrusted�with�us.

We�strive�daily�to�provide�the�best

possible�care�to�our�patients�and�the

people�who�love�them,�addressing�the

physical,�emotional�and�spiritual�needs

unique�to�end-of-life�care.�We�seek�to

give�back�to�the�community�through

the�offering�of�bereavement�services�to

anyone�in�the�community�who�needs

them,�not�just�hospice�patients�and

their�families,�as�well�as�educational

opportunities�to�the�community.�And,

we�are�working�to�keep�our�hospice�on

a�strong�footing�in�an�ever-changing

healthcare�marketplace.

The�staff�and�board�remain�diligent�in

their�oversight�to�ensure�that�Caldwell

Hospice�remains�financially�strong�and

steady�as�we�meet�the�challenges�of

this�evolving�era.�Amidst�it�all,�CHPC�is

committed�to�the�community�it�serves,

quality�patient�care,�and�the�not-for-

profit�model�that�allows�it�to�provide

care�to�all�patients,�regardless�of

reimbursement.

While�other�communities�around�the

country�are�losing�their�nonprofit

hospices,�CHPC�remains�solid,�another

testament�to�the�support�and�faith�of

the�community�we�serve—your

community,�our�community.

Service to our fellowman has

mission. It is the foundation

of who we are.

oundations�matter.�They�are�thebedrock,�the�structure�that

supports�all�that�is�to�come,�and�itis�a�foundation�of�ministry�andservice�that�distinguishes�CaldwellHospice�and�Palliative�Care�(CHPC)and�the�work�it�does�every�day.

CHPC�began�nearly�35�years�agowith�an�underpinning�of�servingothers—a�foundation�and�promisethat�remains�steady�and�strong�allthese�years�later�and�still�drives�the�work�of�the�county’s�only�not-for-profit�community�hospice.

While�for-profit�corporations�havegotten�into�the�hospice�business—in�fact,�70%�of�today’s�hospiceproviders�in�the�U.S.�are�for-profit—the�not-for-profit�community�hospicemodel�is�built�not�on�a�financialmotive,�but�care�first,�a�mission�that�is�exemplified�daily�by�the�many�who�work�and�volunteer�forCaldwell�Hospice.�The�idea�was�andis�a�simple�one—neighbors�helpingneighbors�through�the�difficult�daysof�end-of-life,�a�community�caring�for�its�own,�serving�individuals�and�families�in�their�time�of�need,personalized�care�that�treatspatients�as�a�whole�person,addressing�their�physical,�emotionaland�spiritual�needs�and�being�asupport�to�those�who�love�them.

It�seems�like�a�common�idea�now,but�in�the�early�1980s,�it�was�arevolutionary�approach�that�wasfueled�by�a�true�grassrootsmovement�in�Caldwell�County�toestablish�the�county’s�first�hospice.

“Ours�truly is�a�joint�effort—Caldwell Hospice�and�the�community�coming�together�to�care�for�oneanother,”�says�Cathy�Swanson,�chief�executive�officer.�

F Now�serving�about�800�patients�ayear,�Caldwell�Hospice�is�a�leaderamong�not-for-profit�hospices�state-wide.�It�operates�North�Carolina’soldest�patient�care�unit�with�itsKirkwood�facility�in�Lenoir.�While�themajority�of�patients�receive�care�athome,�Caldwell�Hospice�also�has�asecond�patient�care�unit�at�theRobbins�Center�in�Hudson.

CHPC�provides�physician�andnursing�services,�social�workservices,�spiritual�and�bereavementcounseling,�and�dietary�counseling.Medical�supplies,�medications�forsymptom�control�and�pain�relief,home�health�aide�and�homemakerservices,�volunteer�services,�andphysical,�occupational�and�speechtherapies�are�also�offered�as�well�asenhanced�services�including�cardiacand�pulmonary�care�at�home�forpatients�with�heart/lung�disease,�adementia�care�program�and�a�fullcomplement�of�community�bereave-ment�services,�which�are�open�toanyone�who�needs�them,�not�justhospice�patients�and�families.

In�addition,�Caldwell�Hospice�offers�apalliative�care�program�called�AIM:Advanced�Illness�Management—forthose�who�need�assistance�managinga�chronic,�potentially�life-limitingillness�that�is�not�terminal.

Today,�hospice�care�is�covered�byMedicare,�Medicaid�and�most�privateinsurances.�Still,�there�are�many�whohave�no�way�to�pay�for�their�care,and�that�is�where�the�communityshines�in�Caldwell�Hospice�throughdonations,�volunteer�service�andother�contributions.

Caldwell�Hospice�never�turns�away�apatient�because�he�or�she�cannot�pay,and�that�patient�receives�the�samelevel�of�care�as�anyone�else.

“We�care�for�anyone�who�needs�careand�compassion�as�they�and�theirloved�ones�face�the�end�of�life.�As�anon-profit,�that�is�our�work,”�saysSwanson.�“Service�to�our�fellowmanhas�always�been�and�remains�ourmission.�It�is�the�foundation�of�whowe�are.”

Selecting�a�medical�provider�for�a�loved�one�is�an�important�decision,�and�hospice�is�no�exception.�With�for-profit�competitors,�such�as�PruittHealth,Community�Home�Care�and�Hospice,�and�Medi�Home,�and�non-profit�competitors�serving�residents�in�our�community,�it�is�important�to�learn�about�each�hospice�provider�before�making�a�choice.

By asking meaningful questions, such as the ones below, you can know what eachhospice�provider�offers�and�which�provider�is�right�for�you�or�your�loved�one.

l What�is�the�hospice’s�philosophy�of�care?l What�is�the�hospice’s�mission�statement?l How�long�has�the�hospice�been�in�business?l Is�the�provider�a�for-profit�or�a�non-profit,�community-based�hospice?l Is�the�hospice�accredited?l How�are�calls�and�patient�visits�handled�after�regular�business�hours?l How�quickly�does�the�hospice�respond�to�referrals?l How�experienced�is�the�staff?l Does�the�hospice�have�an�inpatient�unit?l What�volunteer�services�are�offered?l Does�the�hospice�offer�any�specialized�services?l What�services�does�the�hospice�offer�to�families�following�a�death?l What�is�the�satisfaction�rate�from�patients�and�families?l Where�is�care�provided?l Who�pays�for�services?

other�winners,�was�honored�at�theBest�of�Ashe�Awards�Luncheon�atWest�Jefferson�United�MethodistChurch�in�January.

“In�nursing�school,�my�plan�was�togo�into�obstetrics,”�said�Barker,�“butafter�working�with�patients�at�theend�of�life,�I�felt�so�pulled�in�thatdirection.�I�discovered�that�the�endof�life�deserves�just�as�much�love,care�and�respect�as�the�beginning�of�life.�That’s�why�I�am�a�hospicenurse.”

Built on a foundation of nearly 35 years of experience in patient-centered end-of-life care,�the�Caldwell�Hospice�High�Country�teamcontinues�to�grow�as�it�provides�compassionate,�quality�end-of-life�care�toresidents�of�Ashe,�Avery�and�Watauga�counties.�Each�member�of�our�HighCountry�team�is�an�area�resident�with�a�desire�to�serve�patients�and�familiesin�the�community�they�call�home�regardless�of�one’s�ability�to�pay.�With�apermanent�work�station�in�Boone,�the�High�Country�care�team�made�nearly8,000�visits�to�156�hospice�patients�in�the�three-county�area�in�2015-16.Another�56�patients�in�the�AIM�(Advanced�Illness�Management)�programwere�served�with�755�staff�visits.

Caldwell Hospice�welcomed�ten�new�volunteers�who�completed�training�in�October�and�December�2016.�

The�new�volunteers�are:�Cynthia�Alley,�Rachel�Boston,�Kendal�Bray,�Lily�Calderon,�Marcus�Dolphens,�Vickie�Gragg,�Karen�St.�John,�Barbara�Stark,�Tammy�Whicker,�and�Pam�Penttinen�with�therapy�dog�Galen.

Caldwell�Hospice�in�the�High�Country�welcomed�five�new�volunteers�who�completed�training�in�October�2016.

These�new�volunteers�are:�Peyton�Cook,�Murry�Haber,�Richard�Hall,�Beverley�Lawry,�and�Gordon�Lawry.

Adult�volunteer�training�is�offered�several�times�each�year.�The�next�training�is�scheduled�for�June5,�7,�and�10,�2017 in�Caldwell�County.�Other�volunteer�opportunities includeVolunTEENs,�for teens in�high school, and Ambassadors,�liaisons�betweenfaith�communities�and�Caldwell�Hospice.�Caldwell�Hospice’s�need�for�volunteers�is�ongoing. Volunteer�coordinators�will�match�volunteer’s�skillswith�the�organization’s�needs.�

Caldwell�Hospice�and�Palliative�Care,along�with�other�national,�state�andcommunity�organizations,�areleading�a�massive�effort�to�highlightthe�importance�of�advance�health-care�decision-making—an�effort�that�has�culminated�in�the�formaldesignation�of�April�16�as�NationalHealthcare�Decisions�Day�(NHDD).As�a�participating�organization,Caldwell�Hospice�is�providing�infor-mation�and�tools�for�the�public�totalk�about�their�wishes�with�family,friends and healthcare providers,�andexecute�written�advance�directives(healthcare�power�of�attorney�andliving�will)�in�accordance�of�NorthCarolina�state�laws.�These�resourcesare�available�from�Caldwell�Hospiceor�www.nhdd.org.

Specifically,�on�April�4,�CaldwellHospice�invites�the�community�tolearn�more�at�Start the Conversation,�a�program�that�explores�why�thesedecisions�are�so�important�andprovides�practical�solutions.�Theprogram�will�be�presented�threetimes, 10�a.m.,�12�noon,�and�2�p.m.,at�McCreary�Family�ProfessionalCenter�in�Hudson,�NC.�There�is�nocost,�but�registration�is�requestedby�April�3.

As�a�result�of�National�HealthcareDecisions�Day,�many�more�people�in�our�community�can�be�expectedto�have�thoughtful�conversationsabout�their�healthcare�decisions�andcomplete�reliable�advance�directivesto�make�their�wishes�known.�Fewerfamilies�and�healthcare�providerswill�have�to�struggle�with�makingdifficult�healthcare�decisions�in�theabsence�of�guidance�from�thepatient,�and�healthcare�providers�andfacilities�will�be�better�equipped�toaddress�advance�healthcare�planningissues�before�a�crisis�and�be�betterable�to�honor�patient�wishes�whenthe�time�comes�to�do�so.

Volunteer Reba Barlow Receives AwardReba�Barlow�of�Lenoir,a�long-time�volunteer�with�Caldwell�Hospice�and�Palliative�Care,�has�received�the 2016 Judith Lund Person Hospice Volunteer Award through�The�Carolinas�Center�for�Hospice�&End-of-Life�Care.�

The�award�is�presented�annually�toone�hospice�volunteer�from�Northand�South�Carolina�whose�serviceexemplifies�exceptional�service�andcommitment�to�hospice�and�thepatients�it�serves.�

With�an�attitude�of�service�andcompassion,�Barlow�has�spentnearly�700�hours�at�Hospice�in�just�the�last�five�years�providingcompanionship�and�caregiver�relief,offering�11th�Hour�support,�helpingwith�gardening�projects,�lending�a�hand�at�special�events�and�health�fairs,�facilitating�the�Virtual�Dementia�Tour® within�thecommunity,�and�assisting�withcountless�other�projects.
