Worksite Agreement Form
Students enrolled in the Automotive Technology Baccalaureate Degree program are given the opportunity to earn college
credit while on a paid internship or cooperative education work experience with an approved business, corporation,
organization, or government agency in the automotive or related field. This work experience period serves as an experiential
learning activity that is designed to help students apply academically learned concepts, theories, principles, and skills in a
workplace setting. Students will have the opportunity to gain additional knowledge, expertise, and work experience while
allowing the employer the opportunity to develop potential future employees and complete timely projects.
To participate in the Southern Illinois University (SIU), Automotive Technology Internship (AUT 320) course, the following
must be fully understood:
1. The student has read and fully understands this packet, course syllabus and all expectations.
2. The “Work Site Agreement” form must be completed, returned to the Department Internship Coordinator for approval
of the worksite and duties. The worksite must be approved prior to enrolling in the class. Check with the Coordinator
regarding any questions.
3. Students serving in an internship/cooperative work experience must have emergency health insurance coverage.
Questions regarding the SIUC Student Medical Benefit (Student Insurance) can be addressed to Student Health
Programs at 618-453-3311.
4. Grades will be determined by the following:
Logbook: A brief log of student activities and/or duties for each day of the work experience period is required. The
Logbook must be submitted via the “Desire 2 Learn” (D2L) course instructional enhancement website. A blank
word document form is available at this site to assist in formatting. The document is to be completed and
uploaded in the “Logbook” dropbox. It is the student’s responsibility to update the log each week and have it
uploaded. Failure to maintain timely logbook entries can result in final grade reductions. A copy of the Logbook
form is included in this packet for reference.
Worksite Supervisor Evaluations: A separate evaluation is required at the end of each calendar month of work
experience. The evaluation form is to be completed by the student’s assigned supervisor (as indicated on the
Work Site Agreement form) and returned to the Coordinator once completed. An email submit button is included
on the form for the supervisor. It is the student’s responsibility to ensure that their supervisor receives the
evaluation form for completion. The blank Student Evaluation form is available on the D2L site. It is
recommended that the student communicate with their supervisor as the end of each calendar month approaches
and present the evaluation form to them. Again, it is the student’s responsibility to give the evaluation form to
their supervisor for completion. A blank copy of the evaluation form is included in this packet. Please
communicate with the Internship Coordinator if the employer has a different evaluation form or if there are
policies regarding their evaluation process.
Final Report: A 3-4 page single-spaced report covering the nature of the student’s assignment and how the experience
helped in obtaining their career objectives is required toward the conclusion of the enrolled academic period. This
report should concentrate on 1 or 2 major points of knowledge gained during the work assignment to develop the
level of detail desired. The Final Report is to be uploaded to the “Final Report” dropbox prior to the beginning of
the last week of the academic period. It will be graded using the Final Report Grading Rubric.
The updated logbook and final report must be in the appropriate dropboxes on the D2L site or received by the
Internship Coordinator prior to the beginning of the last week of the academic period in order to be included in any
grade calculations.
5. The Internship Coordinator will contact the student and employer during the work experience period to assess the
student’s progress. This requires the student to keep the Coordinator informed of any changes as the work experience
progresses. (i.e.: changes in supervisors, job duties, phone number, . . .)
Student Responsibilities:
Complete Work Site Agreement form.
Ensure health insurance coverage during work period.
Keep Coordinator informed of progress and/or changes.
Present evaluation form to supervisor for completion at the end of each month.
Keep updated logbook on D2L site each week.
Upload Final Report to the D2L site by beginning of the final week of the semester.

Student Logbook
Dear Employer:
Thank you for participating in the Southern Illinois University (SIU), Automotive Technology
Internship/Cooperative Education Program. Students enrolled in the Automotive Technology Baccalaureate Degree
program are given the opportunity to earn college credit while on a paid cooperative or internship work experience
with an approved business, corporation, organization, or government agency in the automotive or related field. This
allows the student an opportunity to gain additional knowledge, expertise, and work experience while allowing the
employer the opportunity to develop potential future employees and complete timely projects.
For the student to receive credit, the following requirements must be satisfied:
The amount of credit that a student may earn is 1 credit hour per 100 hours of supervised paid work experience.
Thus, the student may desire to complete a specified number of hours during the work experience.
The student, the employer, and the SIU Automotive Internship Coordinator must complete the Work Site
Agreement form. The completed agreement must be returned to the Coordinator’s office no later than the first
day of the work experience period.
The SIU Automotive Technology program will determine the suitability of the cooperative work location and
will monitor the cooperative experience.
The employer must agree to supervise and evaluate the student during the internship/cooperative work
experience. The student will give their work site supervisor an evaluation form to be completed and returned at
the end of each calendar month during the work period for which the student is receiving credit. Please
communicate with the Internship Coordinator if the employer has a different evaluation form or if there are
policies regarding the evaluation process we need to be aware of.
The student will be required to complete the following written assignments:
Logbook: A brief log of their activities and/or duties for each day of the work experience period.
Final Report: A 3- 4 page single spaced report covering the nature of their assignment and how the
experience helped them in meeting their career objectives, concentrating on 1 or 2 specific points of
knowledge gained.
If you should have any questions, or concerns please feel free to contact me. Once again, thank you for employing
an SIU Automotive Technology student during their work experience period of training.
Sincerely,
