House of Charity’s Housing Programs provide various lengths of stay and levels of service to single adults who are working to attain suitable long-term housing. With staff support, clients tackle the barriers that keep them from maintaining housing stability.

O N - S I T E P E R M A N E N T H O U S I N GHousing Supports: County-funded Housing Supports rooms for men and women who have experienced homelessness. Individuals must be referred by a case manager, outreach worker, or other professional. Priority is given to individuals living on the streets or in shelters with mental, chemical and/or physical health concerns, difficulty meeting basic needs, or are vulnerable to exploitation.

Veterans Administration (V.A.)Beds: For homeless veterans who are referred through the V.A. Clients are generally on waiting lists for permanent Veterans housing and need a place to live until a unit becomes available. The V.A. refers the client and both agencies provide support services until the client can move into their own housing.

HousingFirst: Clients who have experienced homelessness are placed in apartments throughout Hennepin County. Open to single adults who are Hennepin County residents or transferring from a Housimg Supports facility in another county. Individuals must meet the State’s Long-term Homeless Definition to qualify for funding. Landlords/property managers each set their own screening criteria for tenants, so a variety of housing histories and criminal backgrounds can be considered.

client stability and provide mediation as needed with landlords, neighbors, etc.

• Assistance with budgeting and shopping• Referrals to local community, health and social

resources

The Coordinated Entry System is the county’s approach to organizing and providing housing services for people experiencing homelessness in Hennepin County.Because housing resources are limited, this process is designed to ensure that individuals and families with the highest vulnerability, service needs, and length of homelessness receive top priority in housing placement.
