Dear participant,

As a part of my Research work, I have taken up a research work on the important

topic of Performance of Supply Chain Management. I have received the research fund

from Mumbai University to carry this work.

Supply chain is a continuous process from raw material to finished goods via each

traditional distinct function such as forecasting, purchasing, manufacturing, distribution,

and sales and marketing. So improvement of this business integration enables the

management to focus upon managing the core business. So supply chain consists of

different levels and it is a network of companies which influence each other. The

complexity and the large network affects one another’s performance. So how manager

choose the most optimum supply chain best suited for its particular industry.

This questionnaire is precisely designed to reveal the importance each criteria

plays in the SCM selection problem so that the selection problem in each level is tackled

effectively. You, being a person in the industry will be in a right position to give the

necessary information needed for this research. I request you to respond to the questions

frankly and honestly and be the expert resources for this study. Your responses will be kept

strictly confidential and the information provided by you will be used only in connection

with the above mentioned research. Also attempt will be made to share the aggregate

results of this study with the respondents. Thank you very much for your time and

cooperation. You and your organization’s help in carrying out this endeavor is greatly

acknowledged.

Thanking you

Sincerely Yours,

Avinash D Sarode

This questionnaire is precisely designed to reveal the importance each criteria plays

in the SCM selection problem so that the selection problem in each level is tackled

effectively.

This questionnaire is precisely designed to reveal the importance each criteria plays

in the SCM selection problem so that the selection problem in each level is tackled

effectively.

Sincerely Yours

Avinash D Sarode
