Plus, the providers verifyeligibility and obtain the necessaryinformation, so members don’t haveto hassle with paperwork.

• use the provider directoryon the anthem.com home page,where they can also find mapsand driving directions.

• contact Anthem Blue Visioncustomer service by calling thenumber listed on their ID cardsor (800) 828-3677 in Indianaand (800) 367-5897 in Kentuckyand Ohio.

Depending on their plan, memberscan also use non-network providers.Services and materials are paidfor up front. Members maysubmit itemized receipts forreimbursement. Coverage may belimited and out-of-pocket expensesmay be higher.
