I n case you are wondering what the title “BYT – Another Dental Magazine”

is all about, there is a simple answer. First off, we are not planning to enter the Dental Media Landscape and make this a permanent publication. This is a one-shot project, even though it may look and feel like it’s not.

So, why is it needed then? Because we want to offer a deeper insight beyond all things Dental. Yes, it features top-notch case reports from AG users all over the world and tons of valuable information about our world leading product portfolio which you can’t find anywhere else. But that’s not it.

There is a second content feature which makes all the difference. It’s about the AG community, the people & their pas-sion, presented both in and outside the labs. In order to understand why people do things the way they do them, we need to understand what inspires them in the first place. Inspiration often appears in situations where we feel comfortable and free, unleashed from our daily routines.

At best, reading this magazine can distract you a little bit from all the madness, even if just for a minute. Like a short “mind-walk” in the park. That’s why we have also featured a lot of what our homeland has to offer, country- and culture-wise.

You will also get some curious insights into the life of some of our key users and cooperation partners from all over the world. It should help sharpen the picture of the AG community a bit. Because it is truly an outstanding community – some-thing we’re very proud of, something worth telling stories about.

If you have ever taken a ride on a ski lift or cable car, anywhere in the world, you will probably have come across a product made in Vorarlberg. Firstly, Vorarlberg is a pioneer of alpine skiing,

Girrbach, shares scientifically proven insights about the connection between fractures in all-ceramic restorations and the crown marginal area.

Previously in dental technology the crown margin was regarded as the area on dental technical restorations subjected to least load-ing. Different scientific studies into the frac-ture behavior of dental restorations, however, arrive at the opposite result.

The main conclusion of these studies is that the origin of fractures in all-ceramic restora-tions can be found in the marginal area in the majority of cases. The reason for this is that the occlusal and incisal masticatory forces

After about eight years working in the dental technology sector (Dental Laboratory Glaser; Boblitz/Brandenburg, Germany), during which he mainly specialized in the fixed/removable and implant prosthetics, Falko Noack decided to study at the University of Applied Sciences Osnabrück. After four years at the university he gained the title Dipl.-Ing in Dental Technology. During his study time he worked at the uni-versity on various projects in the field of metallography and material testing of dental materials. The topic of his diploma thesis was the development of a process chain for a zirconia pre-sintered blank manufacture. He then applied his practical and technological knowledge in re search and development at Amann Girrbach, especially in the field of zirconia production and application technology. Falko Noack is now head of the Research and Development Department at Amann Girrbach.

[see green arrows in pictures 1 & 2] are trans-mitted from the surface of the restoration to the marginal area and result in splitting of the crown exposed to loading [see orange arrows in pictures 1 & 2], particularly with thin-walled geometries. Tensile stresses criti-cal for ceramic materials are thus created in the marginal area of the restoration [see red arrows in pictures 1 & 2], which could be the cause of fractures. The risk of fracture is also increased by rough margins, inhomogeneous surfaces and larger gaps for the luting material.

Particular attention should therefore be given to the following points when designing the crown margin to ensure permanently reliable all-ceramic restorations:

1. Adequate material thickness, including in the marginal area (tangential preparations are therefore contraindicated for all-ceram-ic restorations)

2. Smooth and even margins

3. Perfect surfaces (polish)

4. Adequate, uniformly thin cement gap thickness

A CASE REPORT.Anterior Esthetic Restorative Treatment of Idiopathic Dental Pathology.

Taking teeth out is the only thing that we do in dentistry that is permanent.

For this reason, every effort is made during the care of our patients to provide minimally invasive treatment options and to preserve the dentition. For some of these patients, however, extraction of one or more teeth is an unfortunate necessity. Specific situations that involve the restoration of the patient’s anterior dentition and/or soft-tissue defects require a carefully coordinated effort between the clinician and the ceramist.

In March of 2017, a 32 year old male patient presented for a consultation with chief cos-metic concerns regarding the ongoing and unexplained deterioration of hard and soft tissue structures around the maxillary right lateral and central incisors (Fig. 1). The patient reported little to no discomfort at the affect-ed site. A full medical and dental history was reviewed for the patient and found to be unremarkable. At the time of the initial visit, the intraoral evaluation revealed pink com-posite resin had been applied by another provider to temporarily aid as a splint and to mask the loss of interdental papilla that were evident when the patient smiled. Radio-graphic evaluation utilizing cone beam volu-metric tomography (Fig. 2) revealed signifi-cant horizontal and vertical loss of osseous scaffolding around the maxillary right lateral and central incisors. Prior consultations with multiple specialists, including oral surgeons

and periodontists, were unsuccessful at determining a cause for the current pathological findings.

Diagnostic alginate impressions and shade photos of the dentition and gingiva (Fig. 3) were sent to the ceramist prior to beginning treatment. Treatment planning for this case involved the extraction of the maxillary right central and lateral incisor, simultaneous bone graft/membrane placement and restoration of the edentulous space with a layered Zolid HT+ white zirconia 4-unit fixed dental pros-thesis.

Following the removal of the failing dentition, provisionalization with a temporary prosthe-sis and an appropriate healing phase of approximately 4 months, the tooth prepara-tions (stumps) were analyzed for shade (Fig. 4). Even though a zirconia framework was being utilized for the definitive restora-tion, it was critical to assess the foundational color of each stump- due to the conservative hard tissue reduction of each tooth and the highly translucent nature of the zirconia ceramic being used for this particular case. Upon completion of the final VPS impres-sions, the zirconia framework was milled, stained, and sintered at the dental laboratory (Fig. 5). To verify accuracy of fit and seamless margins of the restoration, the framework was evaluated both in-situ and radiographi-cally during a try-in visit (Fig. 6) ...
