Project on Nutrition Improvement, Income Generation, and Environmental Conservation

Municipality of Manicore in the State of Amazonas is one of the poorest regions in Brazil, and its primary schools face
serious health problems, such as malaria, dengue, intestinal parasites, diarrhea, malnutrition, underdevelopment,
unwanted pregnancy by teenagers.
Manicore, which is composed of hundreds of communities along the tributaries of the Amazon River, has no hospital besides
the Municipality Hospital. Hence, as a means to protect themselves, the Manicore Education Office and the community
people requested "health" to be integrated into primary education as a formal subject. However, due to problems such as
lack of funds and human resources, the request never materialized.
From October 2003 to March 2006, HANDS, supported by JICA, implemented “Project for Strengthening Community
Health Services in Amazon” and aimed for the improvement of health status of the population in Manicore through
strengthening the capacity of Community Health Workers (CHWs). Utilizing this experience and asset, HANDS aims to
improve the health standard of primary and secondary school children in rural communities of Manicore.
HANDS also has started new challenges approaching nutrition improvement and income generation by introducing
agroforestry system and empowering community residents.

【Project Purpose】
Purpose To improve the nutrition status of newborns, infants and children in Kericho District, by increasing
the number of mothers who continue breastfeeding
Outputs 1. Postnatal and newborn care services are upgraded at health facilities, particularly the
knowledge and skill to promote breastfeeding.
2. Awareness and knowledge of the postnatal and newborn care are improved in the community,
especially topics related to breastfeeding.
3. Monitoring system for maternal and child health care services related to breastfeeding are
improved at Kericho district health office.
Main
Activities
・ Conduct technical training for the health facility staff on postnatal and newborn care, especially
focusing on breast feeding, such as promoting awareness of breastfeeding, improving the
methods to handle breastfeeding trouble and providing appropriate information of
breastfeeding for HIV positive women.
・ Support the health facility staff to regularly conduct and monitor postnatal and newborn care
services.
・ Prepare and conduct “Partners Workshop,” in which the health facility staff and community
representatives both participate, for the purpose of effective collaboration of health facilities and
community in order to promote breastfeeding.
・ Encourage community groups to participate in "Baby Friendly Campaign" in Kenya to promote
breastfeeding at the community.
・ Support Kericho District Health Office to supervise the postnatal and newborn care services at
the health facilities and develop a plan of promotion on breastfeeding for each community based
on the experiences through the project.

【Project Purpose】
Overall Goal Mothers and children are harmonized into the strengthened health systems and the mortality among
mothers and children in a Pilot State are reduced.
Purpose Village midwives(VMWs) are empowered and organized in the PHC context to perform ideal
continuum of care for maternal and child health (MCH) in Pilot State.
Outputs 1. Organizational capacity of FMOH and SMOH, and rules and regulations of VMWs are
strengthened to perform appropriate MCH services.
2. MCH services are provided through strengthened and organized VMWs in Pilot State.
3. Horizontal communication network among northern states and relevant stakeholders is
strengthened to address the issue on MCH.

Purpose The number of adolescents who receive integral reproductive health care increases in the 7 cities of
Olancho Department
Outputs 1. Healthcare personnel provides adolescent reproductive health (ARH) services in consideration
for adolescents on the basis of the regulations of National Program of Integral Care for
Adolescents (PAIA).
2. A health system to provide ARH services is established.
3. Adolescents take a positive attitude about receiving ARH services by activities of health
promotion.
4. A system of administration and management for provision of ARH services, including IEC, is
established.

【Project Purpose】
Purpose Maternal and infant mortality rate in the Eastern Visayas region is over the national average. The
DOH gives priority to maternal and child health program and has issued the Maternal, Neonatal and
Child Health and Nutrition (MNCHN) policy, which focuses on improving the quality of services and
delivery care. Through this project, we expect the number of pregnant women and newborns who
receive safe pregnancy, safe delivery and postpartum care is increased.
Outputs 1. Facilities which provide the Basic Emergency Obstetric and Newborn Care (BEmONC) and
MNCHN services are increased.
2. Technical skills of government service providers in BEmONC/ MNCHN are enhanced.
3. The function of DOH-CHD EV and PHO/CHO in achieving and maintaining quality of
BEmONC/ MNCHN services is enhanced.
4. Woman’s Health Teams are institutionalized and operate in areas covered by the proposed
BEmONC facilities.
5. Political support for pregnant women and their families in accessing BEmONC/ MNCHN
services is enhanced in areas covered by the proposed BEmONC facilities.

【Project Purpose】
Purpose Due to the lack of doctors, CHNs have come to play a core role in health service delivery. In order to
deal with diversified health problems, CHNs have an opportunity to take the national training
courses to improve their capabilities. However, the courses do not necessarily meet the training needs
among CHNs, as offering a greater number of the top-down courses. Our project aims that the
mechanism of the CHNs’ need-based in-service training (NB-IST) is strengthened to raise the quality
of community health services.
Outputs 1. The NB-IST policy for community health nurses takes effect.
2. A nationally standardized monitoring and evaluation system for the NB-IST is operated.
3. A mid-level nursing management training course (incl. community health context) is conducted.
4. All types of NB-IST for CHNs are adequately coordinated at divisional level.
5. The progress and results of the project are shared among Fiji, Tonga, and Vanuatu.

【Project Purpose】
Purpose To help to restore a safe and nurturing environment for mothers and children in the disaster stricken
areas.

・ Conduct small-scale survey targeting mothers with small children in order to identify their needs
on health and welfare.
・ Provide assistance to childcare support groups / facilities / network for reconstruction of its
services.
・ Implement advocacy activities in order to appeal the importance of long-term relief assistance for
vulnerable populations such as mothers and small children or prospective mothers.
<In Kesennuma, Miyagi / Joint with PWJ >
・ Monitor and evaluate the PWJ’s program “Art and Sports Caravan,” which aims to help children
