Any part of this program not used within a year of purchase is fortfeited. New programs begin on date renewal payment is received, Canceled plans due to unforeseen events will default to regular treatment fees before refunds are given. This program is for individuals who do not have Periodontal Disease. Patients with Periodontal Disease have the option of our Periodontal Savings Plan. Periodontal condition / diagnosis must be completed by your dentist for the program qualification. In the event patient is diagnosed with Periodontal Disease while enrolled in the Dental Family Savings Plan and enters into the Periodontal Savings Plan, any difference in fees must be paid and a new annual date will begin from that date forward. All Services included in this plan must be completed in our office. Any services provided outside of our office are your responsibility and are not subject to any of our saving plan inclusions. This program cannot be transferred to another person. Benefits are only available to the enrolled family member. This program cannot be used or combined with any service provided prior to the date of initial sign-up. Membership expires one year from date payment is received.
