With reference to Office Memorandum F.No. S. 11045/36|2OL2(CGHS/HEC dated' 10th

December, 2014, the undersigned is directed to state that continuous empanelment scherne has beeninitiated in CGHS, Jaipur for empanelment of Hospitals, Diagnostic / tmagining Centers, Exclusive Eye

Centres & Dental Clinics.Accordingly, in continuation to this Office Memorandum of even number dated L4.LL.2OL4, in

' addition to the list of hospitals, Diagnostic / lmaiing Centers, Exclusive Eye & Dental Clinics alreadyempanelled, the list of HCOs enclosed herewith are also empanelled w.e.f. date of this OM. The HCOs

should be treated as included in the existing list already notified with same terms and conditions as

indicated in the Office Memorandum dated L4.11.20t4. The empanelment of the HCOs mentioned inthe'enclosed list as per Annexure CEll, shall be for a period of two years or as applicable to otherHCO's empanelled earlier in 2Ot4 through e-tende whichever is earlier.
