Clinical Rotations -Medical AcademyParent Meeting- May 15, 2019
Start Dates○ All clinical rotations will start on or around
September 30th.○ All clinical paperwork must be turned in by
August 5, 2019 or your child will not be able to attend clinical rotations
○ You can either drop paperwork off at the front office of school or mail it to:

Clinical Requirements○ 75 hours (includes classroom time &
actual clinical site time)○ EMR, Med Lab 3 & 4, and Nursing students
can do clinical hours over the summer at local hospitals, private offices, and non-profit medical groups. This is strongly encouraged!!
Summer Clinical Hours There is a hand-out in your folder that goes over the requirements for doing clinical hours over the summerReminders:○ Contact site ahead of
time to confirm arrangements
○ Wear Park Vista Uniform○ Remember HIPAA○ No Cell Phones
Summer Clinical Hours○ Take notes on your
experience and have at least 3 questions answered
○ Have the site supervisor sign your log sheet and email your medical teacher verifying that you were there and for how long
○ Write a one page summary of your clinical experience
Clinical Summary
Uniform Dress CodeUniform○ Scrubs should be clean, pressed, and in good
repair (blue Park Vista scrubs or navy blue BDU pants and blue polo shirt for First Responders)
○ Shoes shall be clean and neat (black and all leather)
○ Appropriate undergarments should be worn and should not be noticeable (t-shirts worn under scrubs must be white, black, or royal blue)
○ Student Clinical ID must be worn at all times!
Uniform Dress CodeHair○ Should be neat, clean, combed, and
above the collar so as not to interfere with work or be a safety hazard.
○ Hair should not be of extreme color or style- no designs or verbiage cut into hair
○ Facial hair should be short, trimmed, and well groomed
Uniform Dress CodeFingernails○ Should be clean, neatly groomed, and
trimmed to a conservative length○ Nail polish should be conservative- clear or
French tip○ Acrylic nails are NOT permitted
as per OSHA standards
Uniform Dress CodeJewelry○ Earrings shall be limited to one stud
earring only per ear. Male students may not wear earrings
○ No other visible body piercing is permitted
Uniform Dress Code○ Students who choose to wear cosmetics
are required to create a professional and business-type appearance.
○ No perfumes, aftershave lotions, or perfumed body lotions may be worn
○ Tattoos must not be visible

Student Expectations○ Arrive on time! (Park Vista Tardy Policy will be enforced)
○ Check in and out of the clinical location with their teacher
○ Be cooperative and respectful to all school and clinical staff
○ Exemplify a positive attitude and inspire positive behavior
○ Notify teacher of any change in the daily schedule (absences, running late, etc.)
Student Expectations○ Obey the established rules of travel by
reporting directly to the clinical site and returning promptly to school during assigned clinical hours
○ Dress in required clinical uniform and wear student identification at all times
○ Park in designated areas at school and clinical site
Student Expectations○ Hold in confidence all information or facts
pertaining to patients that they may encounter
○ Abide by school rules and policies listed in the student handbook at the clinical site as well as on the school campus
○ NO ELECTRONIC DEVICES are allowed while on duty!
Student Expectations○ Students should bring a small,
pocket-sized spiral or composition notebook and pen every day to take notes on their experiences- In addition, Nursing students need a pair of blunt scissors. EMR students will be issued trauma shears & a stethoscope .
○ No purses, backpacks, or jackets should be brought to the clinical site- There is no place to store them
TransportationStudents can either:○ Drive alone○ Drive and carry other students○ Ride with a fellow student in their class○ Have a parent drive them○ Other approved by parent (Uber, Lyft,
bus, etc.)TRANSPORTATION WILL NOT BE PROVIDED BY MEDICAL ACADEMY TEACHERS! PLEASE MAKE ARRANGEMENTS FOR YOUR CHILD.
Flu Shot RequirementRequirement
Students who will be at any hospital clinical facility will be required to have a flu shot. It must be the current season’s flu shot.
Due Date 10/31/19-You will be responsible for getting your own flu shot! ***This may change depending on clinical facility requests.***
Clinical Forms○ You have two packets in your folder to be
completed this evening and one that will need to be completed and turned in by August 5th.
○ Please fill out all forms in blue or black ink.○ Please wait to fill out each form until I go
over it. Some forms will need to be notarized. We have several notaries present tonight to take care of these forms.

Student Driving-Riding Permission Form
• Check the modes of transportation that you will allow your child to use on the top left of form.
• If your child will be driving, you must provide us with their driver’s license and proof of car insurance.
• Wait to sign bottom of form until you are in the presence of the notary. Please only sign sections that you will actually approve for your child.

Medical Sciences Academies Clinical Handbook2018-2019● Fill in student name.
PPD and other Required Forms• A PPD (Tuberculosis test) must be
done on each student• Date placed, date read, and
result must be documented on Medical Academy Screen Form-Physician Signature is also required
• If test is positive, a chest X-ray must be done and results included with paperwork
• Complete both Medical Academy Health Screen Forms
The PPD test requires two doctors visits 2
to 3 days apart.
○ A ten panel drug screen must be completed for each student between August 1st-30th. (This is the only exception to the paperwork deadline)
○ Drug screens must be completed at MD NOW or AAA Medical Compliance Testing. Results will be automatically emailed to the school.
○ DO NOT USE PRIMARY CARE PHYSICIAN FOR DRUG SCREENING.

Physical Requirements• A complete physical must be
completed on each student• You can submit the State of Florida
Yellow Form as proof of physical• Park Vista Health Exam Form needs to
be filled out by parent and Park Vista Medical Evaluation and Medical Screen Forms needs to be filled out and signed by the physician
• Physicals must be completed after June 1, 2019
Immunization Requirements
• Immunization Records can be submitted on State of Florida Blue Form
• A copy of your immunization records can also be obtained in Guidance
• Medical Academy Health Screen Form must also be completed by physician’s office.
