Approach any member of the Library Team (VP for Acads,
Deputies to the VP, PIC of the library services)
Ask an upperclassmen to lend you books na lang… sorry po.
Tell them the books you wish to borrow.
Fill-out the information needed in the Library Logbook.
Claim the books you borrowed.
Return borrowed books after one month.
OK na… you can renew if no one applied for a “hold”
Fine of P5.00/day overdue
One can borrow a book anytime within
the semester.

One can file a “hold” for a book if the book he is interested in is currently on
loan. Upon the deadline for borrowing of that book, the one who applied for “hold” shall be prioritized as opposed to someone who applies
for book lending renewal.

Secure Tutorial Request Form (ACheS Bulletin Board or online: academaches.wordpress.com)
Submit accomplished form to VP Academics
Wait for confirmation via text or email. If approved,
TUTORIAL SESSION
Answer tutorial evaluation form
Submit a softcopy (e-mail to the VP for Academics) or
hardcopy (to the VP for Academics’ pigeonhole in
the tambayan of the accomplished request slip at least 3 days (for groups
less than 7 people) or 5 days (for 7 or more people)
before the target tutorial date.
