A “minimum list” of dangerous abbreviations, acronyms, and symbols has been approved by JCAHO. Beginning January 1, 2004, items 1 through 9 must be included on each accredited organization’s “Do not use” list. These additions were approved by the Health Information Services Committee and reviewed by the Patient Safety Committee.
