We have new residents joining us all the time. Remember what it was like when you were new? When you see someone new trying to find a seat in the dining room help them out. Why not try inviting them to come with you to an afternoon activity. Transitioning into a new living situation can be dif-ficult. Let’s all try to make it as easy as possible for our new residents.

Lynden Manor Library

Our library is taken care of by Sharon Worthen, a resident volunteer. She has recently been joined by Dorothy Noteboom, also a resident. The two ladies have been working very hard the past few weeks to get our library organized. They have been getting rid of duplicate books and books that are in poor condition. They have reshelved the books in alphabetical order by author. We ask that you don’t try to reshelve books yourself. Put them in the basket and they will put them away.

Thanks Sharon and Dorothy - we appreciate all you do!

Conversations can make all the difference! Making your end-of-life wishes known in advance of a crisis is one of the most pro-

found gifts you can give yourself, your family and your friends.

What is advance planning? Advance care planning involves making decisions about care you want to receive if you were unable to speak for yourself because of illness or accident. The decisions are based on your personal values, preferences and discussions with your loved ones and healthcare providers. What is an advance directive? An advance directive is a legal form that describes the medical care you want to receive if something happens to you, and you can’t speak for yourself. Putting your wishes in writing helps to minimize confusion or conflict in times of crisis or uncertain-ty.

There are two main types of advance directives: • A living will, or healthcare directive, tells your

family and doctor what kinds of treatment you want to receive if you are unable to speak for yourself.

• A durable power of attorney for healthcare lets you name a person - a healthcare agent - to make treatment decisions for you when you can’t speak for yourself.

• How do you write an advance directive? The simplest advance directive form in Washington

is the Washington State Medical Association form. It is available at wsma.org or through your healthcare provider. Before completing the docu-ment, discuss your values, concerns and preferences with your family, friends and healthcare team. Visit wsma.org for more information. The forms are completed, now what? Make several copies of your completed forms, Give one copy to your healthcare agent, one copy to your doctor and send a copy to PeaceHealth Medical Rec-ords at the address below (even if you do not have a PeaceHealth primary care provider).

PeaceHealth Attention: Health Information Management 1115 SE 164th Avenue, Dept. 336 Vancouver, WA 98683 Or fax to 360-729-3378

Please include a brief note requesting that the docu-ment be scanned into your medical record. If you do not have a current patient file, please include your birth date, phone number and address with the form so that a record can be created exclusively for your advance directive. For more information about ad-vance care planning or to sign up for a workshop to complete an advance directive, visit our website: peacehealth.org or contact your healthcare provider.

Advance Care Planning

Resident Rights Corner:

Chapter 70.129RC protects your rights at Lynden Manor. It is the intent of the legislature that individu-als who live in boarding homes and facilities receive appropriate services, be treated with courtesy and continue to enjoy their basic and legal rights, as well as having the opportunity to exercise reasonable con-trol over life decisions. So in an effort to assist our residents in understanding their rights, we are gointo feature a Resident Right each month in our newsletter going forwards.

This month’s “Resident Right”:

Each resident and legal representative has the right to: Participate in planning care and treatment or changes in care and treatment (unless adjudicated incompe-tent, then legal representative participates).
