Another reason to use Ioban 2 antimicrobial incise drapes . . .

In order to analyse antimicrobial activity, the 3M Infection Prevention Discovery Centrehas created genetically engineered staphylococci that glow whilst they are alive.2.

Conversely, when they are killed by an antiseptic agent like iodine, the glow ceases. Thisallows a visual demonstration of the release of the antimicrobial agent under laboratoryconditions. The continuous antimicrobial activity of an iodophor impregnated incise drape(Ioban incise drape) is shown through these organisms in a new in vitro study.

What we found out:

Non-antimicrobial incise drape .

Antimicrobial incise drape (Ioban)

It is clear that most of the bioluminescent bacteria remained aliveafter two hours under a non-antimicrobial incise drape. TheIoban 2 antimicrobial incise drape, however, was successful inkilling most of the bioluminescent bacteria.

Restricted to bacteria in growth phase. (Skin bacteria typically grow at a slower rate.)Potential false negatives (low/no light emission) can occur if bacteria are temporarilypushed into dormancy. Luminescent bacteria are modified from their original geneticstructure. Previous research on other genetically altered organisms reported in the literature suggests no difference in phenotype. 3M tested the staphylococci of this studyand verified that they did not deviate from their unmodified counterparts.

These laboratory experimental results cannot be extrapolated directly to on-patient performance. However, results from this bioluminescence method are consistent withother studies available on Ioban 2 antimicrobial incise drapes.

What’s on your patient’s skin when you get ready to operate ?

The following is the summary of a study2. generated by the 3M Infection PreventionDiscovery Centre.

The more we learn about the largest of our human organs, the skin, the more we understand the microbial challenge it presents. The skin is a living organ that provides an effective barrier to the outside microbial world.

In this study, eight subjects were prepped with common antiseptic solutions.

What we found out:*Efficacy of the preps varied with test

subject*It was related in part to initial bacterial

density on the skin*Although all preps reduced bacterial

counts by at least one log, individual responses varied:

-70% ETOH reduced more bacteria than PI on one subject while PI worked better than 70% ETOH on another subject.

As long as we are not able to identify “high counters” we shoulduse incise drapes to create a sterile field around the incision

New evidence presented in this study:* A percentage of the patient population naturally host larger

numbers of skin bacteria than others. These are referred to as “high counters”

* After surgical site disinfection, these “high counters” continue to harbour significant levels of skin organisms

* The rate of “high counters” in the general population is about one in every three to four people.

* Efficacy of the preps varied with test subject.

Disinfect the skin with your usual skin-preparation. Allow the skin-preparation agent to dry completely.

Using two people, remove the lineron the 3M incise drape until thexx sign appears.

Hold the drape over the intended incision site with adequate tension but without overstretching.

Smooth the drape first along theintended incision line with a steriletowel.

Working away from the incisionline, smooth the remainder of thedrape into place.

A p p l i c a t i o n

Create a crease in the drape bypinching the film near the incision’s edge.

Pull up on crease, separating thedrape from the skin at the incision’s edge, exposing up tothree centimetres of skin surface.Proceed with closure.

After skin closure

Before removing the drape, firstcover the incision with the dressing.

Remove the fabric drape with thefilm attached by folding thedrape/towels 180° back on itself.Pull gently with even tension. Do not pull up.

To remove incise drapes fromreusable linen drapes or towels,wet the material and then peel theincise drape off the material.
