    Three part client interview:

    Identify and get permission from a client to perform this three-part interview. You may select more than one client for thisassignment if the interview becomes to tedious for one client.

    1) Interview:Stress and Coping: Potter and Perry (2009), Chapter 31Identify a client to interview and assess regarding stressors. The intent is to provide opportunity to explore

    how the person perceives and is affected by stress, how the client manages stress, and nursing interventions

    to assist in management of stress.

    1. Review textbook

    2. Explain intent of assignment to client.

    3. Elicit from the client examples of stressors (a minimum ofthree) and how the client

    manages the resulting stress. Be sure to review Box 31-6, page 492, of Potter and Perry for nursing

    assessment questions for your selected client. Record the responses in the designated columns.

    4. Submit completed work to instructor for grading on WebCT

    Client Interview: Influence of Stress on Health

    Client Perception of Stress Client Management of Stress:

    adherence to healthy practices.

    Client expressed some stress

    concerning his movement/falling

    alarm on the back of his wheelchair

    and bed.

    Client regrets that he cannot drive

    anymore. He mentioned that he no

    longer has the freedom or

    authorization to drive.

    Client mentioned that he was

    originally stressed about lack of

    opportunity to practice his

    photography hobby.

    Client pointed out that he had very

    few close friends at the facility. He

    luckily still has his wife (who

    happens to also live at the facility).

    After many unintended falls, the

    client has understood the primary

    reason behind the alarm is his own

    personal safety and protection.

    Client understands that in his current

    physical condition, he cannot

    operate an autmobile saflely any

    longer.

    Client had all of his personal hobby

    photography tools of the trade

    brought to him at the residence. He

    can now practice his hobby anytime.

    Client has be-friended his roommate

    as well as some other people he has

    recently met while eating in the

    dining room at the facility.

    Nurse could assign a CAN or friend,

    etc. to spend a certain amount of

    time each day, 1-2 hours with client.

    This way, he could enjoy not being

    tethered to this alarmdevice.

    Client has learned to avail himself of

    the facilities transportation services

    as well as family, friends and staff.

    Staff could help to encourage the

    enjoyment of his hobby by enlisting

    him to be the facilities resident

    photographer.

    Staff could foster social get

    togethers or mixers so the client as

    well as others have more opportunity

    to meet new faces and get to know

    them better.

    2) Health and Wellness: Potter and Perry (2009), Chapter 6Interview and assess a client regarding his/her perception of current status of wellness-illness. Apply

    knowledge and skills gained in NURS 3032 Therapeutic Communication to interact with the client. The

    intent is to provide opportunity to explore how the person feels in each of the following five dimensions

    at this given point in time: Physically, psychologically/emotionally, intellectually/cognitively, socially,

    and spiritually.

    1. Prior to the interview prepare question(s) for each of the five categories stated above.

    2. Explain the intent of your assignment to the client.

    3. Conduct interview/assessment

    4. Using open-ended questions, ask the client about his/her state of being (how s/he is feeling) in each

    of the categories.

    5. Write brief quotes from the client in the appropriate column.

    6. Over-all, how did your client perceive him/herself: Healthy? Well? Was this different from how you

    perceived the client? Describe how? Elaborate on their input.

    7. What other internal and external variables did you discover? (See Potter & Perry, page 73-74).

    8. Include self-evaluation of strengths and weaknesses of the interview.

    9. Submit to instructor for grading on the WebCT assignment site.

    Client Interview: States of Wellness-Illness

    Interview the client on his/her current state of health in the five dimensions.

    Physical Dimension Question(s)

    How do you currently view your personal health?

    Responses

    Even with a plethora physical maladies, the client

    still seems himself in general good health. He

    considers himself lucky to be in such a nice facility

    with so many people to take care of him.

    Psychological/Emotional Dimension Question(s)

    How happy have you been able to stay here at this

    facility?

    Client has very positive outlook on his overall

    happiness. He recently celebrated his 96th

    birthday

    and his 70th

    wedding anniversary. He keeps busy

    with his favorite hobby, photography.

    Intellectual/Cognitive Dimension Question(s)

    How have you been able to stay mentally active?

    Client was a Professor/Doctor of Animology &

    Botany at the University of Arkansas. He still keeps

    his mind sharp by reading, surfing the internet,

    listening, observing. He recently turned over some

    of his professional animal photography to the UA.

    Social Dimension Question(s)

    How often you have friends and family over to

    visit? What do you do to keep your spirits up?

    Client responded that, his daughters are not able to

    come since they both live far away, but they do

    come when they can. He does his best to keep

    busy and tries to socialize with others at the

    facility.

    Spiritual Dimension Question(s)

    I know that you adhere to the Baptist faith. How

    often do you get to go to church services?

    Client responded that, I dont get to go often butthey do sometimes have services here at the

    facility.

    3) Client Interview: Culture and Ethnicity: Potter and Perry (2009), Chapter 9The intent of this assignment is to explore how culture influences the individual especially in the area of

    health practices to prevent illness and to maintain and/or restore health. If you have the opportunity, within

    your agency, to interview someone from another country, thats great. Realistically, this is unlikely. Focus

    your interview on exploring differences within our so-called dominant culture.

    1. Prepare the client for the interview2. Use the following format for questions.3. Column one: Elicit a minimum of two preventive health care practices and two maintenance or treatment

    practices of illnesses which the client learned while growing up. Example: Takes lemon juice in hot water

    to promote bowel function every morning; takes diabetic medication every day; visits chiropractor for back

    pain.4. Column two: Describe how/where/when the client developed the practice. Examples: Taught by mother as

    a child; prescribed by physician; recommended by a friend.

    regular and healthy. He also mentioned

    that his Grandmother used to rub her aching

    joints with hot vinegar before going to bed.

    Client mentioned that he has remained

    physically active most of his 96 years by

    walking everywhere he went. He also

    presribes to a happy marriage as helping him

    remain healthy. He obviously is doing

    something right since hes already 96!

    Strengths and weaknesses of the interview process:

    Some strengths during the interview were using probing questions helping to unearth more information on each

    topic. Unfortunately, the client was very sleep due to eating and being of 14 different types of meds. It was very

    challenging trying to work around this and keep him awake long enough to finish.

    Some weaknesses were I should have taken my time more and spaced out the interview over a couple of days

    instead. Also, I could have asked staff when his eating and med admin. were normally given so as to work around

    the tiredness.
