2019  Summer  Enrichment  One-­‐on-­‐One  Workshop  Registration  You  must  complete  a  separate  form  for  each  workshop.  

Additional  forms  only  need  to  include  the  student’s  name  and  workshop  information.  

 Using  the  back  of  this  form,  please  check  the  dates  you  would  like  to  schedule  your  sessions.  You  

may  select  either  16  one-­‐hour  sessions,  or  8  two-­‐hour  sessions  in  one  term  per  workshop.  Our  summer  tutoring  hours  are  9:00AM-­‐6:00PM,  Monday-­‐Thursday,  and  9:00AM-­‐5:00PM  on  Friday.

    Summer  Reading  Support:  Please  list  all  books  we  will  cover  by  title  and  author.  We  will  purchase  a  copy  of  

each  book  for  the  tutor.  Students  must  bring  a  personal  copy  for  highlighting  and  writing  in  the  margins.    

   

    Math  Support:  Does  your  child’s  school  use  a  specific  math  program,  such  as  Eureka  Math,  Everyday  Math,  or  

IB  Math?    

 Cost:  $760  for  each  workshop,  ($280  off  our  regular  tutoring  rate).  A  deposit  of  $65  is  due  with  each  form.  Deposits  are  non-­‐refundable,  but  may  be  applied  to  future  tutoring  sessions  if  a  cancellation  is  needed.  Full  payment  is  due  before  each  workshop  begins.  You  may  pay  by  cash,  check,  or  charge.  Once  a  workshop  begins,  no  refunds  may  be  issued.  Individual  sessions  may  be  rescheduled  with  a  24-­‐hour  notice.  Make-­‐up  sessions  must  be  scheduled  to  take  place  before  August  16th.  After  this  date  any  missed  session  will  be  forfeited.    Complete  and  return  this  form  to  The  Bowie  Center  or  call  to  register  over  the  phone  at  901-­‐474-­‐1081.

2019 Summer Workshop Calendar

The  Bowie  Center  offers  a  flexible  schedule  to  accommodate  your  individual  needs.  Use  these  calendars  to  determine  dates  that  work  best  with  your  summer  schedule.  

 Students  may  sign  up  for  multiple  workshops,  but  one  workshop  may  not  be  split  over  two  months.  

Families  may  choose  to  split  one  workshop  between  two  children.
