Comparing POCT testing for troponin, D-dimer, CRP and β-hCG to

central laboratory analyzers

Diagnosis in an acute care setting requires the rapid and accurate measurement

of biomarkers for appropriate triaging of patients. For diagnosing acute myocardial

infarction (MI), the Canadian Cardiovascular Society states that either cardiac tro-

ponin I or T are the preferred biomarkers. Thrombosis Canada guidelines state that

using D-dimer as the initial test reduces the need for diagnostic imaging for patients

at low risk for venous thromboembolism. C reactive protein is used to detect or

monitor significant inflammation in acute conditions. And finally, β-human chorionic

gonadotropin is applied to the determination of pregnancy. Although provision of

such testing on automated central analyzers in tertiary hospital centres is common,

implementation in a rural, urgent care setting requires use of either near-patient or

point of care testing.

In evaluation of the Radiometer AQT90 Flex immunoassay instrument, good

concordance was achieved between it and Abbott Architect or HCL TOP central

labor atory analyzers. Display of concordance studies for each analyte and discus

sion of incorporating such POCT into rural, urgent care setting using current guide

lines demonstrates the benefits of POCT to patient care in the clinical presentation

of MI, thromboembolism, sepsis and pregnancy.

Antibiotic Susceptibility testing performed directly from positive Blood

Culture

RGH Microbiology department conducted a pilot study to determine the accuracy

and effectiveness of antibiotic susceptibility performed directly from a positive

blood culture. The purpose of the study was to ensure appropriate antibiotic

therapy was initiated in a timely manner to improve patient care.

Dr. Malejczyk and Ms. Grund will provide an overview on their pilot project and the

outcomes.

Early Sepsis Biomarker and Technical Solutions

Patients with sepsis most often present to the emergency department, and delayed

detection is harmful. WBC count is often used to detect sepsis, but changes in WBC

count size also correspond to sepsis. This presentation will discuss the incorporation

of MDW with WBC count using Beckman coulter new technology to improve

detection of sepsis compared with WBC count alone.

Basic approach to common causes of anemia

This will be an interactive session with case studies covering the most common

causes of anemia in our patient population. We will discuss the components of the

CBC, the differential diagnoses of common red cell indices changes and assess

red cell morphology features in order to reach a final diagnosis and discuss some

treatment implications. It’s going to be a blast!
