Members of Team HISD preparing parent packets for the start of the 2017-2018 school year can download all compliance documents at www.houstonisd.org/parentforms. The website contains the most up-to-date forms HISD Is required by law to distribute to parents and guardians along with useful one-pagers on topics such as proper online behavior, the importance of attendance, and the district's electronic services acceptable use policy.

The documents should be downloaded and printed by each campus, expect for items listed below. All schools should have already received a campus supply of the following items:

• 2017-2018 Code of Conduct acknowledgement form/request for hard copy

• 2017-2018 student media release form• 2017-2018 school year calendar• SchoolMessenger "How do you want to be communicated with?"

If your school has not yet received the campus supply of these documents, please call the HISD Warehouse at 713-676-9292.

Code of Conduct

All schools will receive a 10 percent supply of the 2017-2018 Code of Conduct in late August. The supply should be distributed ONLY to those students/families who request a hard copy. The 2017-2018 code is currently available online to all staff, students, and parents in four languages at www.houstonisd.org/codeofconduct . It also will be posted to all school websites.

Free and Reduced Lunch form

All schools will receive a campus supply of the National School Lunch Program application/income survey form in late August. In addition to the printed form, the meal application/income survey is also available online in English, Spanish, Vietnamese, and Arabic. Nutrition Services encourages all principals to direct parents

Chancery is now on‐line for the 2017‐2018 School Year Important Reminders 

   

Registration and Enrollment:  When Enrolling students or entering a ‘Left’ record the date always defaults to the current date and should be edited if necessary. 

When enrolling a new student, use 08/01/2017 or the next available date.  The last date schools should use to enroll students before the first day of 

school is 08/11/2017.  Do not use 08/21, 22, 23, or 24/2017 to enroll students prior to the first day 

of school.  Using dates after 08/25/2017 will interfere with No‐Show processing.

Both the Federal Ethnicity and Federal Race fields are required for enrolling 

students and PEIMS reporting.

Enter a “Left” record using any date between 08/01/2017 and 08/11/2017 to release students who were actively enrolled in your campus at the end of the 2016‐2017 school year.  Using prior dates will cause PID errors. 

No‐Shows:  

Do not use the No‐Show status at this time. 

No‐Show status should be used only after school starts.  The official No‐Show date for all schools is 08/25/2017 and should be entered 

only for students who are enrolled but not in attendance on the first day of 

school.

Release Of Students In Chancery SMS For Enrollment In Other SchoolsDue Date Audience Contact

Students who have moved from one address to another or who have obtained a transfer during the summer may need to be withdrawn from one HISD school before they can enroll in another HISD school. Federal and State Compliance staff will work with all schools to help expedite this process. A request to withdraw a student must only be made if the parent or adult student has come in person to enroll at the school with proof of residence or has an approved transfer from the Office of School Choice (Student Transfer Department). Please see the attachment for required action related to specific enrollment scenarios and communicating with Federal & State Compliance regarding the release of students. The FSC mailbox ([email protected]) will be available for releasing students from August 14-25, 2017. After August 25, 2017, all requests must be made to the campus in which the student appears as enrolled.

Parents should not be directed to the HISD Administration Building for routine matters related to transfers. Transfer requests, transfer approvals, and release of students can all be completed via e-mail. Please see attachment for details.

Attendance Clerk The attendance personnel generating absence summaries and transcribing the absences or coding information into the accounting system are responsible for adhering to all laws and regulations pertaining to student attendance accounting. Each person entering data into the attendance accounting system must sign an affidavit attesting that the data he or she has entered are true and correct to the best of his or her knowledge—or, in the case of a paperless attendance accounting system, indicate electronically that the person attests that the data he or she has entered are true and correct to the best of his or her knowledge.

Tasks Prior to Opening of School

1. Post rosters throughout campus for parents to review. 2. Review the updated Membership Reporting procedures and the school district

calendars. 3. Distribute Class Rosters to teachers with written instructions on how to document

“No Show” students. 4. Verify that ADA time is posted in your school.

First Day Procedures

1. Distribute first day rosters to teachers to verify attendance (must be signed in ink by teachers).

2. If a student is not present, the teacher should mark a single line through student’s name and mark NS to the right.

3. Make sure that teachers send attendance to the office no later than 30 minutes after official ADA time.

4. Log onto the HISD Portal to Report Manual Membership Figures by 10:30 am. KEEP originals of first day rosters (ALL SCHOOLS).

(electronically).

Reminder: If a student who was marked on the 1st day as a "No Show" reports on the 2nd day or thereafter, the "No Show" should be changed to a "Left” record and a new enrollment status must be created, effective the first day the student is present at ADA time.

Note: ADA must be taken by the teacher at the ADA time, for example, if ADA in your building is at 9:30 a.m. and a student is not present in the teacher’s classroom, the teacher should mark the student absent. Teachers have 20 minutes after the ADA time to submit the attendance in GradeSpeed, they do not have 20 minutes after the ADA time to take attendance.

The role of the teacher is to:

• take attendance at the official time designated and for each class period;

• maintain accurate attendance records in GradeSpeed and verify accuracy of

Chancery records;

• send excuse notes from parents to the office in a timely manner;

• notify parents when a student returns from an absence without an excuse or has been absent for three or more days;

• insure that students make up work for excused and unexcused absences;

• complete an attendance referral form for appropriate students, in accordance

with district guidelines

First Day Procedures for Teachers

1. Use the Classroom Roster provided by the clerk, on the roster mark one single line through the name of any student on the list who is not in the classroom at official attendance time and mark NS to the right of the student’s name.

2. Add the names of any students present in your classroom at official attendance time whose names are not on the roster, but whom you accepted into class with documentation from the office.

3. Record the total number of students present and sign and date the roster in blue or black ink (NO PENCIL).

4. Do not “trade” students with another teacher without the knowledge of the principal or principal’s designee, who in turn, gives the information to the attendance clerk.

5. Color-Coded Cards (If applicable) • Must be completed in ink (NOT PENCIL) • No erasures or white out • Must be signed by the teacher with whom the student is present with at the

Second Day Procedures for Teachers On the second day of school and any day thereafter, a student entering a teacher’s room must have documentation from the office (ex. Admit slip or class schedule). Verify that the student’s name is in GradeSpeed the day the student enters class and, if not, notify the attendance clerk by e-mail.

• Take attendance at the official ADA time. • Submit attendance in GradeSpeed each day no later than 20 minutes

after designated time of attendance. Attendance needs to be submitted, even if a class has 100 percent attendance. Notify the attendance clerk in writing by using the Attendance Correction Form in GradeSpeed or by e-mail of any absence corrections needed. All manual official attendance documents (Absentee Slips) must meet these conditions: (This also applies when a teacher cannot log into GradeSpeed or there is a substitute in the teacher’s classroom.)

• always use blue or black ink (never record manual entries in pencil, use liquid correction fluid, and never use a signature stamp);

• always use the legal names of the students (no nicknames or shortened version of the name);

• always use the correct student ID number; • Teacher or Substitute Teacher’s Signature in ink; • If errors are made on any official attendance document, strike one line

through the error, enter corrections nearby, and initial in ink.

GradeSpeed Attendance Process Description - GradeSpeed’s attendance interface can exchange attendance data with Chancery SMS the Interface will:

• Post attendance recorded in GradeSpeed to Chancery SMS

• Pull attendance corrections made in Chancery SMS into GradeSpeed

The GradeSpeed process with Chancery SMS Interface includes the following Calendars:

• Calendars and configurations are pulled from Chancery into GradeSpeed

nightly starting at 9.00 p m Schedules:

• Schedule changes for secondary schools are pulled from Chancery into

GradeSpeed every 15 minutes. • Schedule changes for elementary schools are pulled from Chancery into

log back in, after the schedule change is downloaded into GradeSpeed

• The Grades roster page is updated when the teacher logs out and back in

after the schedule change is downloaded into GradeSpeed

Attendance:

• Attendance records are uploaded from GradeSpeed into Chancery every 15 minutes.

• Attendance corrections are pulled from Chancery into GradeSpeed every

15 minutes. Grades:

• Grades are exported from GradeSpeed into Chancery when the

campus grade clerk/administrator manually performs the exports

Membership Reporting on the HISD Portal

All students who are physically present in school on the first day are to be counted in their respective grade level, even though they may be in a self-contained special education class. Membership figures should be shown by grade level (EE through 12), reflecting a physical count of the student present at official ADA time. Students present in the auditorium, library, counselor's office, nurse's office, etc. at official ADA time must be documented a manual roster with a teacher, counselor or administrator’s signature.

An application is available for reporting membership figures in the schools by grade level. Authorized staff will log in to the HISD Employee Portal by 10·30 a.m. and click on Membership Reporting on the HISD Applications Links list to access the system. After the first day of school, membership includes all students who have enrolled and attended at least one day during the current school year and who have not withdrawn. Students who enrolled, but have not attended at least one day should not be Included in the membership count. Please refer to the Memo to Principals about Membership Figures for School Year 2017-2018 for all dates on membership reporting on the HISD portal.

Withdrawals

A student can be withdrawn from school by either biological parent if no court action is involved, regardless of who enrolled the student in school, or by the person who signed the enrollment card and/or has legal custody of the student.

• The SIS Student Withdrawal Record/Transfer Form must be completed with

all information requested after the first week of school, if the student has been in attendance in the school for five days or more, academic grades must be recorded on the check-out sheet. All absences (excused, unexcused, and extracurricular) should be transferred with the student, so they may be recorded at the new school. The student's ID number should be double-checked by utilizing the PET Search PID on Edit+ for accuracy. One copy of the check-out sheet is given to the parent/guardian; one copy is retained by the school, and another copy is sent to the receiving school, if known, together with a copy of the report card. The parent/guardian/student should be given the original report card, and the school should retain a copy in the student's permanent folder. On the top right-hand side of the withdrawal form, write the PEIMS state ID number and highlight.

• In order to track dropouts, a parent signature and the name of the new school

in which the student the student plans to enroll is needed. This is imperative to avoid underreported students.

• The official date of withdrawal depends on the time of day the student

withdraws.

If a student withdraws from school before the time that ADA is checked at the student's school, the effective date of withdrawal is that same day If a student completes the withdrawal process after ADA is checked, the effective date of withdrawal that must be posted to the student's computer file is the next school day. Parents and legal guardians should be encouraged to give the school a 24- hour notice of withdrawal to allow time for the school staff to process the check out sheet without disrupting classes.

• A student is not in membership on the withdrawal date. • Note the effective date of withdrawal and the Withdrawal code (see HISD

Withdrawal Codes) in the office use sect ion o f the student's color coded card. The color-coded card should then be filed alphabetically by grade level.

• Write the applicable information on the Membership/ Transaction Log using the effective date of withdrawal. (THIS APPLIES ONLY TO DESIGNATED PK CENTERS)

• It is critical that withdrawals be posted to the computer files on a daily basis to avoid conflicts with other schools or within the statewide PID file.

Withdrawal Procedures for Non-Attending Students The principal only may withdraw students for non-attendance after specific actions are taken and documented. These actions must include attempting to contact the student to suggest possible Interventions or alternative placement to prevent the student's leaving school.

• For general education students between six and nineteen years of age,

compulsory attendance laws apply. A student should not be withdrawn for non attendance unless school personnel can document that appropriate procedures have been followed. For students in violation of compulsory attendance the proper procedures must be followed and withdraw using Withdrawal Code L98- Other/Unknown. The parent should be notified in writing of this action and informed to re-enroll the student once they regain control.

W I T H D R A W A L SHOULD NOT BE USED WHEN WITHDRAWING STUDENTS FOR NON ATTENDANCE

• On or after the eighteenth birthday, general education students can be withdrawn

for nonattendance. If the student has more than five unexcused absences in one semester, a school district may revoke the student's enrollment for the remainder of the year. Attempts to notify the parent should be documented, a letter informing the parent and adult student of the withdrawal should be sent. The student will be counted as a dropout.

3.2.1.6 Code 5 Ineligible Half-Day

Code 5 indicates that a student is scheduled for and provided instruction for at least 2 hours but fewer than 4 hours each school day but is ineligible to generate ADA (for any of the reasons listed in the preceding subsection on code 4). The students that code 5 applies to include any student who is provided half-day instruction but does not meet the eligibility criteria for the service he or she receives. Examples include the following:

students enrolled only in PK who do not meet the PK eligibility requirements in Section 7

students enrolled in locally funded PK programs offered by your district

students provided instruction by a JJAEP on the basis of an expulsion under the TEC, §37.007(a), (d), or (e), unless specifically authorized in writing by the TEA

students who are required, as a condition of obtaining or holding the appropriate US student visa, to pay tuition to your district to cover the cost of their education21

3.2.1.7 Code 6 Eligible Transfer Student Half-Day

Code 6 indicates that a student is a transfer student who is eligible to generate half-day attendance. Code 6 applies to a student who is a nonresident, is legally transferred into your district, and is scheduled for and provided instruction for at least 2 hours but fewer than 4 hours each school day. The student must meet all eligibility criteria other than residency. The information on transferring students included in the previous subsection on code 3 applies for code 6 as well. A charter school should not use ADA eligibility code 6.

3.2.1.8 Code 7 Eligible—Alternative Attendance Program Participation

Code 7 applies to a student who is eligible to participate in and is enrolled and provided instruction in an alternative attendance program, such as the Optional Flexible School Day Program (OFSDP) or the High School Equivalency Program (HSEP).

3.2.1.9 Code 8 Ineligible—Alternative Attendance Program Participation

Code 8 applies to a student who is enrolled and provided instruction in an alternative attendance program, such as the OFSDP or HSEP, but is ineligible to participate in the program.

3.2.2 Funding Eligibility To be eligible to generate FSP funding for attendance, a student must either:

1) be scheduled for and provided instruction at least 2 hours (half-day attendance) or at least 4 hours (full-day attendance) each day (referred to as the “2-through-4-hour rule”22) or

2) be eligible for, enrolled in, and scheduled for and provided instruction in an alternative attendance accounting program (such as the OFSDP).

The following table explains more fully the requirements a student must meet to be eligible to generate ADA and thus FSP funding. Note: The 2-through-4-hour rule includes recess and in-class breakfast.

Students who are funding eligible and attending on a half-day basis may earn only one-half day of attendance each school day. Your district determines these students’ attendance by taking attendance during a period in which the students are scheduled to be present.23 If a student who is funding eligible and attending on a full-day basis is not scheduled to attend school during the second or fifth instructional hour, your district determines the student’s attendance by taking attendance during a period in which he or she is scheduled to be present. See 3.6.2.2 Alternative Attendance-Taking Time for Certain Student Populations. Attendance for any full-day student (ADA eligibility code 1 or 3) cannot exceed the number of days of instruction for the same reporting period for the same instructional track. 3.7 General Education Homebound (GEH) Program Any student who is served through the GEH program must meet the following three criteria:

The student is expected to be confined at home or hospital bedside for a minimum of 4 weeks. The weeks need not be consecutive.

The student is confined at home or hospital bedside for medical reasons only.

The student’s medical condition is documented by a physician licensed100 to practice in the United States.

A student served through the GEH program at home or hospital bedside must be served by a certified general education teacher. Over the period of his or her confinement, the student must be provided instruction in all core academic subject area courses in which the student is enrolled. In addition, over the period of confinement, the student should be provided instruction in all other courses the student is enrolled in, if possible. Note: For guidance in determining GEH instruction for pregnant students who are receiving pregnancy-related services, see Section 9. A student who is served through the GEH program retains the same ADA eligibility code he or she had before receiving GEH services, regardless of how many hours the student will be served through the GEH program. For information on remote homebound instruction, see 12.3.3 Remote Homebound Instruction—Regular Education Students.

3.7.1 GEH Policy Requirements To qualify for GEH funding, your school district must have policy and procedures for implementation of GEH instruction that have been approved by the local school board.

3.7.2 GEH Committee A designated campus committee must make decisions regarding GEH placement. Members of the committee should include but are not limited to the following:

a campus administrator,

a teacher of the student, and

a parent or guardian of the student. The role of the GEH committee is to review and consider the necessity of providing instruction to a general education student at home or hospital bedside. If instruction is to be provided at home or hospital bedside, the GEH committee determines the type(s) and amount of instruction to be provided. Over the period of his or her confinement, however, the student must be provided instruction in all core academic subject area courses in which the student is enrolled and should, if possible, be provided instruction in all other courses in which the student is enrolled.

In making these decisions, the GEH committee must consider information from the student’s licensed physician. However, documentation from the licensed physician is not the sole determining factor in the committee’s decision-making process.

3.7.2.1 GEH Committee Documentation Responsibilities

In qualifying a student for and providing the student GEH services, the following documentation is required:

a district-developed form that documents GEH committee decisions regarding whether a student is to be served through GEH;

documentation on the form of the GEH committee’s decision regarding the type(s) and amount of instruction to be provided to the student, including the designated amount of time per week that instruction will be provided;

a note from a licensed physician stating that the student has a medical condition that requires the student to be confined at home or hospital bedside for a minimum of 4 weeks;

documentation of the day(s) homebound instruction started and stopped; and

the teacher’s homebound instruction log.

The minimum documentation required in homebound logs (the attendance record maintained by a homebound teacher) is the following:

the name of the homebound teacher,

the student’s name and Texas Unique Student ID,

the date that the homebound teacher visited the homebound student, and

the specific time period that the student was served (for example, 10:00 a.m. until 12:00 p.m.).

Additional documentation may be maintained as part of this record at your district’s discretion. This documentation may include, but is not limited to, mileage records for the homebound teacher and information on subjects that were taught as part of the homebound instruction.

3.7.3 GEH Funding Chart For GEH services to generate ADA, the services must be provided as follows: A general education student served at home earns eligible days present based on the number of hours the student is served at home by a certified teacher each week. 9.1 Responsibility

9.2 Eligibility and Eligible Days Present Eligibility for Services: Any school-age student who is in the prenatal or postpartum period of pregnancy is eligible for services under the PRS program. This includes students who are pregnant and students who deliver a live, aborted, or stillborn baby; suffer a miscarriage or death of a newborn; or place a baby up for adoption. Eligibility for Funding: Students who are eligible for average daily attendance (ADA) and who are eligible for PRS program services are eligible for funding under the PRS program beginning on the date services begin. End of Eligibility: Eligibility for PRS ends when the student returns from postpartum confinement to resume her regular classes at a school campus or the first day of the seventh week, whichever comes first. For a student requiring extended confinement, eligibility for PRS ends when the student returns from postpartum confinement to resume her regular classes or the first day of the 11th week, whichever comes first (see 9.9.3 CEHI during Break-in-Service Confinement for a limited exception).

9.2.1 Absences During the prenatal period, a student is absent if she does not come to school or, if the student is receiving CEHI, if CEHI is not provided. During the postpartum period, a student is absent if CEHI is not provided. A student may be recorded absent on the effective date of a program change (date of enrollment in or withdrawal from the PRS program). However, as with all other students, PRS students cannot earn eligible days present on days they are absent.

9.2.2 PRS Eligibility and Participation in Other State-Funded Programs PRS students may simultaneously participate in other state-funded programs (special education, career and technical education, bilingual or ESL education), provided all eligibility requirements of the other programs are met (see Sections 4, 5, and 6). During CEHI, a student may remain coded and continue to generate special program ADA as long as the student continues to receive services at home as designated by the individual program requirements. (See 9.10 Confinement and Earning Eligible Days Present and 9.17.4 Example 4.)

CEHI requirements and eligible days present are determined each week. The week for CEHI purposes is from Sunday through Saturday. CEHI service hours must not be accumulated and carried forward from one week to the next, and service hours must not be applied to a previous week. A student who is provided CEHI services retains the same ADA eligibility code she had before receiving CEHI services, regardless of how many hours she will receive CEHI. Over the period of her confinement, a student receiving CEHI services must be provided instruction in all core academic subject area courses in which she is enrolled. In addition, over the period of confinement, the student should be provided instruction in all other courses in which she is enrolled, if possible.

9.11 Returning to Campus for Support Services or Testing A student confined to the home may be allowed to return to campus and remain coded PRS to receive temporary, limited support services (see the introduction to Section 9 for a list of examples of support services) or take required state assessments. The time spent on campus receiving temporary, limited support services or taking required state assessments must not count as any part of the number of hours served as CEHI for eligible days present. A student receiving CEHI who returns to campus to receive temporary, limited support services or take required state assessments must have a medical release from a licensed204 medical practitioner to do so.

9.12 PRS and Special Education Services (SPED) Regular education students must not be referred for special education services just because they become pregnant. 4.7.2.2 Homebound Services and Pregnancy-Related Services (PRS)

4.7.2.3 Homebound Services for Infants and Toddlers with Auditory or Visual Impairments or Both

Infants and toddlers (children from birth through 2 years of age) with auditory impairments or visual impairments or both may receive homebound instruction as determined by the IFSP team. A court appearance is considered to be required if the law (federal or state) or the court mandates an appearance by the student in a criminal, civil, or traffic matter. Examples of required court appearances would be appearances in response to a jury summons in the name of the student, a subpoena in the name of the student, or a traffic ticket marked “You Must Appear” or “Court Appearance Required.” Additional examples would be a student's appearance in court as a plaintiff or defendant or as the subject of a court proceeding, such as an adoption or custody proceeding. Acceptable forms of documentation may be a copy of a pleading or other document filed with the court, a notice from the court clerk regarding a hearing or trial date, a jury summons, a subpoena, etc. Important: Absences to meet with probation officers and other absences related to court- ordered activities outside the courtroom do not qualify as required court appearances.

A student may be considered in attendance for travel days related to an absence to serve as an election clerk or student election clerk but not for travel days to serve as a student early voting clerk. Travel days for which the student is considered in attendance are limited to not more than 1 day for travel to and 1 day for travel from the site where the student is serving as an election clerk. Your school district may elect to excuse additional travel days; however, the student would be considered absent for the additional travel days for attendance accounting purposes. Travel days do not count toward the 2-days-per-school-year maximum.

To serve as a student election clerk or student early voting clerk, a student must: be ineligible to serve as an election clerk under the Texas Election Code, §32.051(c); be at least 16 years of age; have the consent of the principal of the school the student attends; be a US citizen; and have completed any training course required by the entity holding the election.

A documented appointment with a health care professional includes an appointment of a student diagnosed with autism spectrum disorder with a health care practitioner to receive a generally recognized service for persons with that disorder. To be considered temporarily absent, the student must begin classes or return to school on the same day of the appointment. The appointment should be supported by a document, such as a note from the health care professional.

The appointment must be a face-to-face consultation with a health care professional. A consultation over the phone or via video (telemedicine) is not considered an appointment with a health care professional.

• is participating in a mentorship approved by district personnel to serve as one or more of the advanced measures needed to complete the Distinguished Achievement Program outlined in 19 TAC Chapter 74.

The agency does not maintain a list of days that qualify as religious holy days for purposes of being considered in attendance for FSP purposes. However, we do provide the following guidance: To be considered a religious holy day, the day should be one generally recognized by the student’s religious denomination as a holy day that is required to be observed by all members of that denomination. Church retreats, camps, and mission trips Your school district is responsible for determining what constitutes a religious holy day for purposes of excusing absences. The agency does not maintain a list of days that qualify as religious holy days for purposes of being considered in attendance for FSP purposes. A BILL TO BE ENTITLED

AN ACT

relating to excused absences from public school for the purpose of

pursuing enlistment in a branch of the armed services of the United

States or the Texas National Guard.

BE IT ENACTED BY THE LEGISLATURE OF THE STATE OF TEXAS:

SECTION 1. Section 25.087, Education Code, is amended by

adding Subsections (b-5) and (b-6) and amending Subsection (d) to

read as follows:

(b-5) A school district shall excuse a student who is 17

years of age or older from attending school to pursue enlistment in

a branch of the armed services of the United States or the Texas

National Guard, provided that:

(1) the district may not excuse for this purpose more

than four days of school during the period the student is enrolled

in high school; and

(2) the district verifies the student's activities

related to pursuing enlistment in a branch of the armed services or

the Texas National Guard.

(b-6) Each school district shall adopt procedures to verify a

student's activities as described by Subsection (b-5).

(d) A student whose absence is excused under Subsection (b),

(b-1), (b-2), (b-4), (b-5), or (c) may not be penalized for that

absence and shall be counted as if the student attended school for

purposes of calculating the average daily attendance of students in

the school district. A student whose absence is excused under

Subsection (b), (b-1), (b-2), (b-4), (b-5), or (c) shall be allowed

a reasonable time to make up school work missed on those days. If

the student satisfactorily completes the school work, the day of

absence shall be counted as a day of compulsory attendance.

SECTION 2. This Act applies beginning with the 2017-2018

school year.

SECTION 3. Not later than October 1, 2017, each school

district shall adopt a policy for verification of student

activities relating to pursuing military enlistment as required by

Section 25.087(b-6), Education Code, as added by this Act.

SECTION 4. This Act takes effect immediately if it receives a

vote of two-thirds of all the members elected to each house, as

provided by Section 39, Article III, Texas Constitution. If this

Act does not receive the vote necessary for immediate effect, this

Act takes effect September 1, 2017.

ATTENDANCE CLERK

Things you should be doing on a Daily Basis:

1. Enroll and withdraw students if applicable. Research students in PID before enrolling them. 2. Make sure that attendance is taken on every student enrolled in your school by the teacher of record or substitute

teachers (If students are with other school personnel, then you must get the correct documentation along with the appropriate signature and enter the change in Chancery).

3. Enter attendance from manual rosters submitted by substitutes or teachers who cannot access GradeSpeed. 4. Run the Display Teacher Progress from GradeSpeed. Send report to the principal or administrator in charge of

attendance. Report teachers who do not consistently take attendance. 5. Update color-coded cards with new entries, withdrawals, returns, grade level changes and ADA code changes. 6. Designated PK Centers and JJAEP - Update the Membership Transaction Log. You are not to wait until the end

of the cycle and copy your Membership Transaction Log from the TX Daily Attendance Report, this will get you in trouble with the Texas Education Agency. This your daily manual record that you compare to the TX Daily Attendance Report.

7. Enter excuse notes from students. 8. Enter attendance changes in Chancery as soon as you receive the correct documentation.

Effective school year 2016-2017, attendance clerks may only accept two forms of documentation from teachers regarding attendance changes:

GradeSpeed Attendance Correction Form An email from the teacher with the student’s name, ID number and the reason for the change.

Teachers are not allowed to complete the Attendance Correction Forms generated from Chancery. If a substitute teacher makes an error and the error is caught on the same day, allow the substitute

to correct the error on the Substitute Roster. The substitute needs to initial the correction. If the error is caught the next day by the classroom teacher, the classroom teacher will correct the

error by using the GradeSpeed Correction Form or email. 9. Enter ADA code changes. The documentation and the change form should be signed by the principal. 10. Check and correct the Certify reports daily.

What you should do at the end of the cycle prior to your Federal & State Compliance review:

1. Run the Entry Date and Withdrawn Students Reason reports and compare the data from the reports with your color-coded cards. These reports are located under the Registration and Enrollment panel.

2. Designated PK Centers and JJAEP - Compare your Membership Transaction Log line by line with your TX Daily Attendance Report for the entire cycle.

3. Run the TX Campus Summary for the previous cycle(s) and the current cycle. – This report should be signed by you and the principal. - The report is located under the Summary Report panel.

4. Run and check the Daily Absence Membership Eligibility report – include non-ADA from the start of school until the end of the current cycle. The report is located under the Attendance panel.

5. Compare ADA code changes to Daily Attendance Summary report in GradeSpeed.

File by Cycle for End of Year Documentation. Only the Attendance Office should use the Attendance Change Form from Chancery. Teachers must use the GradeSpeed Attendance Change Form to submit attendance corrections.

Helpful Opening of School Data Tips 2017-2018

1. Do a PID search prior to enrolling students in the district/school to help avoid duplicate student

ID/records. If you do NOT have access to YOUR campus PEIMS Edit Plus, complete the TEASE EDIT + Authorization Form located on the Portal under the Federal & State Compliance Department (see Forms & Documents). While you are waiting for access email you FSC contact person to assist you in completing the PID search(s).

• NOTE: You can search for a student in the PID with:

o Social Security Number o State Alternate ID o First Name – enter part or all of the first name o Last Name – enter part or all of the last name

ALWAYS use an asterisk (*) after the names. (This will give you more options; perhaps the student had the last name changed…)

2. When enrolling students be sure to complete the required field codes. (Ex: ADA, ethnicity, gender, SS#/Alternate ID, Title I, etc. remember you do not have to add a new line for ADA or Title I each year if the code is the same as the previous year) in order to avoid receiving any Certify errors. On a weekly basis review the following reports to check for missing data and errors and make corrections.

When enrolling NEVER use a Saturday, a Sunday or a holiday as the enrollment date. DO NOT use a future date because these students will not appear on any class or student roster until that future date.

3. PK student must be eligible to attend school as described in the 2017-2018 TEA Student Attendance Accounting Handbook, Section 7. PK Military and PK Foster Indicators Remember to place a √ in the appropriate box(es) on the Overview screen for a student whose parent/guardian is in the military or a student who is in CPS/Foster Care. See screen shot below. Please read and adhere to the required guidelines for PK enrollment eligibility for military and CPS/foster care families. Reference Prekindergarten: A child is eligible for enrollment in the prekindergarten class if the child lives anywhere within district boundaries; is four years of age on or before September 1; (if you do not have any more 4 yr olds on a waiting list and still have space available you can enroll up to a total of five 3 yr olds per campus) and meets one of the following criteria:

1. Unable to speak and comprehend the English language,

2. Be educationally disadvantaged (eligible to participate in the National School Lunch Program [NSLP]; see 7.2.3 PK Eligibility Based on Being Educationally Disadvantaged (Eligible for the NSLP); or

3. Homeless, as defined by 42 (USC) Section 11302, or 4. The child of an active duty member of the armed forces of the United States,

including the state military forces or a reserve component of the armed forces, who is ordered to active duty by proper authority; or

5. The child of a member of the armed forces of the United States, including the state military forces or a reserve component of the armed forces, who was injured or killed while serving on active duty. TEC§29.153

The Texas Education Agency (TEA) requires HISD to submit leaver records, referred to as “203 records”, for students served by the district in grades 7-12 during the prior school year and who do not enroll in the district in the school-start window of the current year (from the first day of school through the last Friday in September). A record is not required for grades 7-12 students who were enrolled during the prior year and are enrolled in the school-start window of the current year. HISD is not required to report leavers and movers who were in grades EE-6 during the prior school year, unless a student was reported for even 1 day in grades 7-12 then later reclassified to 6thgrade.

6. Discipline: (All Grade Levels) DO NOT wait to input discipline for the end of the year or the second semester. Input discipline immediately as it happens including any discipline that occurs in the first day of school.

Important Enrollment Steps

1. Do a thorough PID search, one by entering the student’s social security number (SSN) and another by first and

last name. Place an asterisk (*) after both the student’s first and last name to see all naming possibilities.

2. Complete a thorough HISD search in Chancery. On the homepage, go to Actions and then select Register/Enroll

Student. Search by partial first and last name to see all naming possibilities, which helps in avoiding a duplicate

enrollment. DO NOT use the asterisk (*) in Chancery.

If the student is already registered in HISD, click on the student’s name. Open student record and verify student

demographic information to ensure you have the correct student. Write down the student’s ID number on the

enrollment card. If the student is not found, then click New and register the student in district file.

3. Click on Enroll Student in a School. Then click Add, select your campus name for Home School, enter Date, Status

Code (Enrolled), Grade, and Select Address (add address information or update it), click Ok. Now the student is

in enrolled your school

4. Update the student’s information. Update/enter missing information on the following tabs: Overview, Personal

Info, Address/Phone, and Federal Info. (Pay special attention to the Federal Info Screen to check Federal Ethnic

category, Federal race categories, Migrant & Immigrant Information, and Home Language.

*Note: Must complete immigrant information for students born outside of the United States

On the Overview screen, enter the SSN; if the student does not have a SSN, or is not on the PID with a state

alternate ID or SSN, click on PEIMS Coding and choose Assign State ID. Then check the box Request State Alt ID#

and click OK. During the overnight process, Chancery will assign the student a state alternate id in the social

security field.

5. Select PEIMS Coding and click on ADA Eligibility. Add/update ADA Eligibility coding. If the student already has a

code, DO NOT change it. Only add a line if the student’s ADA eligibility code need to change from one code to

another. Example: When a prekindergarten student moves to kindergarten, the ADA eligibility code changes

from ADA 2 (eligible half day) to ADA 1 (eligible full day).

6. Select Spc Populations and click on Title I. Add/update Title I Coding; if campus is Title I Schoolwide, student

MUST have 6 Schoolwide Participation for Title I Part A code.

7. Every student enrolled must be assigned a schedule, even if enrolled for only one day. IMPORTANT: Switch the

effective date to the 1st Day in School. Ensure bilingual and ESL students are scheduled appropriately.

STUDENT DATA PROCESS FOR ELEMENTARY CAPPING DECISIONS

In order to be prepared for the capping process on Monday August 28, 2017. Elementary

Schedulers must complete the following steps in Chancery before Monday August 28th.

REQUIRED STEPS THAT MUST BE DONE PRIOR TO AUGUST 28th:

1. All students enrolled in Chancery SMS must be assigned a homeroom. No student should

be without a homeroom during this process.

2. All physical classroom type rooms must be entered into Chancery SMS. Example: if there is

a room in your building designated for pullout G/T students enter the physical room number

into Chancery SMS applying the correct dropdown selection on “Room Type” even if you

don’t schedule that room in Chancery.

3. There have been additions made to the dropdown list for “Room Type” in order to assist

with the capping process. This field must be populated with the appropriate “Room Type”

for each classroom in which students are instructed. See following instructions for adding

room type.

Hot Topics for Elementary and Middle School Records Clerks

• Records Inventory Form - complete inventory form and submit to the HISD student records office. Form and deadline will be provided at a later time.

• Cumulative Folder Clean Up - Clean out cumulative folders before sending to the middle schools or high schools (See Handout)

• Permanent Record Cards – make certain they are up-to-date with previous year’s grading labels and test labels. When preparing to send the cumulative folder to another HISD school, place perm card(s) in the front of cumulative folder so it is visible to the receiving school registrar.

• Enrolling English Language Learners (ELL) for which no records can be obtained - A student entering HISD from a foreign country will be placed in the appropriate grade level according to the incoming school records. Recent arrival immigrant students for whom no records can be obtained must be placed according to their chronological age. However, recent arrival immigrant students who enter in the second semester and then are determined to be preliterate (as outlined in the Bilingual/ESL Program Guidelines) and/or has had interrupted schooling, may be placed one grade level below their chronological age.

• ELL students 15 years of age or older – ELL students who enter HISD from another country and for whom no records can be obtained or whose schooling was interrupted should be placed in the ninth grade. See 2017-2018 HISD School Guidelines - Section V for complete information.

• Credit-By-Exams – CBE’s will be ordered four times a school year. Contact Student Assessments.

• Middle School Students with High School Credit - if appeals have been granted for student credit, email information to Allease Shepard, District Registrar [email protected].

Records Clerks

Cumulative Folder Clean Up - Maintain the cumulative folder before forwarding to the next HISD school. Remember to place the permanent record card in the front of the cumulative folder so the next registrar can easily locate the perm card.

High School Credits on Middle School Campus - The District Registrar will add any distance learning courses, grades and credit to student's Historical in Chancery.

Attendance Appeals for High School Credit Courses - Approved attendance appeals are awarded in Chancery by the District Registrar. Documentation from the campus is required prior to the student receiving credit. This documentation may be scanned, emailed or faxed. Include the following: middle school campus contact, student name, ID#, grade level, course, grade, semester course was taken and principal signature approval.

Transferring records for fall semester, 2017

The deadline to transfer cumulative folders to other HISD schools is Friday, September 29. 2017.

Requesting records HISD to HISD only - Requesting records via TREx is district policy and mandated by the State. Registrars may print the Last Year Campus Report (LYC) from Chancery then send it via email, fax or HISD mail to other HISD schools until Friday, September 29, 2017. Beginning Monday, October 2, 2017, all Registrars and Record Clerks must request records via TREx.

2017 Transfer of Records

Since most records will be forwarded from elementary schools to middle schools within the same administrative district, the principal and/or department administrator should appoint a staff member to assist schools in the transfer of records. Each principal and /or department will determine the manner in which records will be transferred in order to ensure that all records are accounted for. Records for all students who enroll in the new school should be forwarded as quickly as possible after the opening of school, including permanent record folders, LEP records, and special education records. The chart below lists most student academic records and the required retention period according to the HISD Record Management Plan for Schools. This document is available on the Federal & State Compliance web site.

Transferring records for Fall Semester, 2017

The Membership / Transaction log adds and subtracts from the first day counts on a student-by-student transaction

basis. Totals for campus counts and grade level counts are kept on the right-most columns for easier use in the

reconciliation process.

NOTE: The CODE column (E, R, W) determines whether to add (+) or subtract (-) from the GRADE LEVEL count

column. “E” and/or “R” will add to the appropriate grade level column, and “W” will subtract from the appropriate

grade level column.

FIRST DAY OF SCHOOL COUNTS

Enter the first day counts by grade level onto the first line of the spreadsheet. The color-coded cards should

match this count exactly by grade level.

First day cards should have upper right corner cut at an angle to indicate that they were “first day” cards

Enter each student that enrolls and withdraws after the first day of school. This includes grade level changes.

ENROLLMENT PROCESS

ELIGIBILITY CRITERIA

APPLYING

Providing Houston's youngest learners with the best education requires beginning the learning process as early as possible. That’s why HISD began offering full-day free prekindergarten programs to all eligible children in 2005.HISD offers prekindergarten on a tuition basis to students who do not meet the eligibility requirements to attend prekindergarten for free. Learn more about tuition-based prekindergarten here.

To be eligible for enrollment in a district free prekindergarten program, a child must:

• Be four years of age on or before September 1 of a given school year; Parents can complete and submit the 2017-2018 official prekindergarten application document to the school of their choice.

• Live within the boundaries of the Houston Independent School District

AND meet at least one of the following criteria:

• Be homeless• Be unable to speak or understand English• Be economically disadvantaged• Be the child of an active-duty member of the U.S. military or one who has

been killed, injured, or missing in action while on active duty• Child is or ever has been in the conservatorship of the Department of

Family and Protective Services following an adversary hearing held as provided by Section 262.201, Family code

• All children who meet any eligibility criteria for Head Start, not only those who meet the low-income eligibility criteria for Head Start.

HISD POLICY ON THREE-YEAR-OLDS

Intent to Apply: Intent to Apply documents will be used for pre-registration purposes prior to June 21, 2017. The Intent to Apply document must be replaced with the official prekindergarten application.

Applications: Official prekindergarten applications for the 2017-2018 school year will be available no more than 60 days before the first day of school. The applications below are the “official” prekindergarten applications that must be filled out to register for prekindergarten for the 2017-2018 academic year. Parents with children who will be 4 years of age on or before September 1, and meet other eligibility requirements can complete the 2017-2018 prekindergarten applications.

HISD policy gives priority for enrollment in the prekindergarten program to eligible students who are at least four years of age on September 1, 2017. If additional space is available and there is no waiting list for eligible four-year-old students and non-eligible tuition-paying four-year-old students who reside in HISD, schools have the option of enrolling students who will be three years of age on September 1, 2017 provided they meet the eligibility requirements. All three-year-old students are to be placed on a waitlist until August 14, 2017, unless enrolling at a school participating in a collaborative partnership with Head Start. Schools should not enroll more than 5 three-year-old students school wide, unless the school is participating in a collaborative partnership with Head Start.

SUBJECT: Free Prekindergarten: Star of Texas Award Eligibility Criterion During the 85th legislative session, an additional prekindergarten eligibility criteria was created to provide free public prekindergarten to children of peace officers, firefighters and first responders who have demonstrated remarkable courage and heroism in serving and protecting their communities throughout the state.

Free Public Three- and Four-Year-Old Prekindergarten In 2017, the Texas Legislature amended via House Bill 357 Section 29.153 of the Texas Education Code (TEC) to include among those who are eligible for free public prekindergarten, a child of a person eligible for the Star of Texas Awards. House Bill 357 was signed by Governor Abbott on June 15, 2017 and is effective immediately. Summary of Prekindergarten Eligibility for Certain Children: A child is eligible for free prekindergarten if they are four years old on or before September 1 of the current school year and meets one of the seven criteria listed below. A district may offer (but is not required to offer) a three-year-old program to eligible children, and the same criteria would apply. Age is always calculated as of September 1, of the current school year (for purposes of establishing eligibility). Therefore, a child is eligible for enrollment in prekindergarten if at least three years of age and:

1. is unable to speak and comprehend the English language; 2. is educationally disadvantaged (which means a student who is eligible to participate in

the national free or reduced-price lunch program); 3. is homeless; 4. is the child of an active duty member of the armed forces of the United States; 5. is the child of a member of the armed forces who was injured or killed while on active

duty; 6. is or ever has been in the conservatorship of the Department of Family and Protective

Services (foster care) following an adversary hearing held as provided by Section 262.201, Family Code; or

7. is the child of a person eligible for the Star of Texas Award as: a. a peace officer under Section 3106.002, Government Code; b. a firefighter under Section 3106.003, Government Code; or c. an emergency medical first responder under Section 3106.004, Government

Code.

The Office of the Governor, Criminal Justice Division honors recipients annually in September. The resolution (certificate) awarded to an individual serves as proof of eligibility to enroll these children in free prekindergarten if they are age-eligible. A list of past honorees may be viewed on the Criminal Justice Division—Past Honorees webpage. If an individual has a prekindergarten-aged child and has been nominated, but not notified as an honoree prior to the current school year, an individual may make a request to the Office of Early Childhood Education to make an eligibility determination based on the nomination submitted for review to the Criminal Justice Division. General Instructions

Use this authorization form to request, modify, or revoke access to TEASE for PEIMS EDIT+, the Person

Identification Database (PID), and PID Enrollment Tracking (PET).

To complete the request for access:

1. Complete and sign the request form.

2. Submit the completed and signed form to your supervisor for approval.

3. Please allow three weeks for processing. If you have not received access after three weeks, [email protected] to check the status of your application.

4. IMPORTANT: When new users receive the system generated e-mails with login information, they should log in and set up a new password. New TEASE accounts that are not accessed within 90 days are systematically deleted.

confidential data. • I understand that procedures must be in place for monitoring and protecting confidential PEIMS information.

PID & PET • I agree that access to PID will be used for the limited purpose of validating demographics of:

1. students currently enrolled in the applicant's school district or region. 2. students previously enrolled during the current or prior school year in the applicant's school district or region. 3. staff currently employed by the applicant's school district or region.

• I agree to limit the data I view to that necessary to validate student and staff demographics for consistency and accuracy in reporting to the TEA.

• I agree to notify TEA Computer Access if my job responsibilities change such that I no longer need access to PID or when I terminate employment with my current educational entity for any reason.

• I understand that procedures must be in place for monitoring and protecting confidential information. • I understand that any unauthorized disclosure of confidential student information is illegal as provided in the Family

Educational Rights and Privacy Act of 1974 (FERPA), and in the implementing of federal regulations found in 34 CFR, Part 99. FERPA is specifically incorporated into the Texas Public Information Act as an exception to records that are subject to disclosures to the public (Government Code, Section 552.026).

• In addition, I understand that any datasets or output reports that I, or my authorized representative, may generate using confidential data are to be protected. I will not distribute to any unauthorized person any datasets or reports I have access to or may generate using confidential data. I understand that I am responsible for any computer transactions performed as a result of access authorized by use of my TEASE username and password.

• I also understand that releasing confidential information is prohibited by the Texas Government code, Section 552.352, and that such an offense constitutes a Class A misdemeanor.

SAF • SAF provides Superintendents with the capability to approve their district's PEIMS submission by electronically signing the

General Instructions

Use this authorization form to request, modify, or revoke access to TEASE for PEIMS EDIT+, the Person Identification Database (PID), PID Enrollment Tracking (PET), and the Superintendent Approval Form (SAF).

To complete the request for access:

1. Complete and sign the form.

2. Submit the completed and signed form to your supervisor for approval.

3. Please allow three weeks for processing. If you have not received access after three weeks, email [email protected] to check the status of your application.

4. It is important when new users receive the system generated e-mails with their login information that they log into their TEASE account and set up a new password. New TEASE accounts that are not accessed within 90 days are systematically deleted.

5. Certification of Responsibility

• I understand that my TEASE username and password are CONFIDENTIAL and may not be shared with another person or entity under any circumstances whatsoever.

• I will neither divulge my password nor use a username and password assigned to someone else. • I understand that I am responsible for all transactions made with my username and password. • If I suspect that my password has been compromised, or that someone else has used my account, I understand that it is

my responsibility to change my password immediately. • I agree to notify TEA Computer Access when my job responsibilities no longer require access to the requested

information, or I terminate employment with my current organization. • I understand that I must reset my password every 90 days. • I understand that if I do not use my TEASE account for 15 months, the account will be deactivated and I will need to

reapply to activate the account.

PEIMS Edit+ • I will not knowingly or intentionally enter any unauthorized data or change any data without authorization. • I understand that any reports or table downloads that I may generate using confidential data are to be protected. • I will not distribute to any unauthorized person any reports or table downloads that I have access to or may generate using

confidential data. • I understand that procedures must be in place for monitoring and protecting confidential PEIMS information.

PID & PET • I agree that access to PID will be used for the limited purpose of validating demographics of:

1. students currently enrolled in the applicant's school district or region. 2. students previously enrolled during the current or prior school year in the applicant's school district or region. 3. staff currently employed by the applicant's school district or region.

• I agree to limit the data I view to that necessary to validate student and staff demographics for consistency and accuracy in reporting to the TEA.

• I agree to notify TEA Computer Access if my job responsibilities change such that I no longer need access to PID or when I terminate employment with my current educational entity for any reason.

• I understand that procedures must be in place for monitoring and protecting confidential information. • I understand that any unauthorized disclosure of confidential student information is illegal as provided in the Family

Educational Rights and Privacy Act of 1974 (FERPA), and in the implementing of federal regulations found in 34 CFR, Part 99. FERPA is specifically incorporated into the Texas Public Information Act as an exception to records that are subject to disclosures to the public (Government Code, Section 552.026).

• In addition, I understand that any datasets or output reports that I, or my authorized representative, may generate using confidential data are to be protected. I will not distribute to any unauthorized person any datasets or reports I have access to or may generate using confidential data. I understand that I am responsible for any computer transactions performed as a result of access authorized by use of my TEASE username and password.

• I also understand that releasing confidential information is prohibited by the Texas Government code, Section 552.352, and that such an offense constitutes a Class A misdemeanor.

SAF • SAF provides Superintendents with the capability to approve their district's PEIMS submission by electronically signing the
