Andy MacWilliam Chairman of the Board

Dear Friends,

Something looks different. We’re glad you noticed! Allow us to reintroduce ourselves . . .

For the past two years, Easterseals has been on an exciting “brand renaissance” journey nationwide. The goal is to reintroduce our organization to the world with clarity and renewed relevance.

The new look is in the form of a brighter logo, a new spin on our established name, and a refined brand voice. You’ll see elements of our fresh brand change in the next year.

Our corporate structure is changing as well. After 20 years of developing and managing services in all the New England states and New York, we continue our voluntary withdrawal, finding new organizations to run New York, Connecticut, and Rhode Island, while growing our New Hampshire, Maine, Vermont and Farnum Center services at unprecedented rates.

Our Purpose is strong and will serve as a guidepost for the years to come: To change the way the world defines and views disabilities by making profound, positive differ-ences in peoples’ lives every day. To this end, we are addressing today’s disabilities, both visible and invisible, from birth to later in life—because we believe every person, regardless of ability, can live a life of their choosing.

In 2016 we celebrate the 80th Anniversary of Easterseals NH and the impact we have made across the Granite State, the Northeast, and the United States of America. Every day, we break down barriers, dispel stereotypes, overcome obstacles, and challenge attitudes and perceptions about people with disabilities and their potential. But we can’t do it alone. It takes a team. On behalf of the thousands of children, adults, and seniors with disabilities whose lives your support touches at Easterseals NH, thank you for being part of our team and for your wonderful generosity.

Sincerely,

A New Look, Same Life-Changing Services

Larry J. Gammon President & CEO

The monies invested in Easterseals New Hampshire are managed efficiently and impactfully.

We are proud to report

Celebrating Our 80th Year . . .

Treatment & Recovery

Common Man Owner and Founder Alex Ray issued a Challenge for Recovery to help fund the expansion at Farnum North. More than 320 individuals and businesses as well as the NH Charitable Foundation and the Easterseals Boards of Directors raised more than $282,000 for renovations at The Ray House. This renovation was part of a huge expansion that resulted in an additional 62 beds to respond to the state’s addiction epidemic.

Due to the dedicated leadership of the Farnum Center Board of Directors, working in

concert with staff, the NH Charitable Foundation recognized Farnum with the 2016 Excellence in Non-Profit Management Award. Farnum also achieved the highest level of international accreditation for rehabilitation facilities (CARF) with a constant focus on client access, quality improvement, and development, achieving a three-year accreditation. In April, Manchester and Franklin received a three-year renewal, which speaks to the program’s high standards.

Thanks to a $1.6 million award from NH DHHS, we look forward to creating a

substance use disorder treatment expansion program by opening intensive outpatient programs in Manchester and Franklin and a 15-bed transitional living program in Manchester.

Military & Veterans We established the Renee & Dan Plummer Endowment Fund to provide financial

support for service members & veterans of all eras to receive substance abuse treatment & recovery services through Easterseals NH programs. Aggressive fundraising totaled $500,000! Farnum and Military & Veterans Services partnered to provide comprehensive care for veterans needing substance abuse treatment. This innovative program, supported by the NH Charitable Foundation, ensures housing stability, employment, and access to long-term support.

We secured a Federal Homeless Veteran Reintegration Program grant that provides

for employment services for homeless and at-risk veterans as well as placing veterans in stable good-paying jobs at ESNH and with other employers. Additionally, our Care Coordinators established a vital role in NH’s implementation of treatment-based court programs for justice-involved veterans. They are providing treatment planning and case management for those who are not eligible for VA services.

We wrapped up the statewide, yearlong Ask the Question Initiative, awarded by NH DHHS, which focused on improving care for our military population by promoting informed engagement between them and civilian service providers, and getting them connected to the military and veteran-specific services and benefits they have earned. By asking one question, “Have you or a family member ever served in the military?,” more than 2,000 providers across NH, working in twelve different service fields, have improved the quality of their care for our State’s military families.

Transportation The Manchester School District awarded our Special Transit Service (STS) with the contract to provide transportation to its special-education population. The contract involves providing daily transportation for over 900 students in NH and MA. Additionally, our transportation services continue to provide mobility for seniors and people with disabilities, providing important access to services that allow them to remain active and independent in the communities in which they live. Our community transit program integrates and coordinates specialized services, efficiently utilizing multiple funding sources to provide community transportation services in Southern NH. In total, STS provided service to over 2,000 unduplicated passengers, providing over 300 thousand trips while travelling 2.7 million miles! We are getting people where they need to go when no other option is available.

Seniors

The Currier Museum in Manchester gave our Adult Day clients the unique

opportunity to create artwork for their Alzheimer's Art & Cafe program. Their exhibit entitled: Experience This Moment was on public display over the summer. This program provided a creative social experience for people with Alzheimer's and dementia and their loved ones or caregivers.

A $50,000 grant from The Pearl Manor Fund allowed our Adult Day Services

to develop and facilitate a Community Care Coordination program with Catholic Medical Center and Dartmouth Hitchcock physicians to better serve our elderly clients. We also benefitted from a $10,000 grant from the Brookdale Foundation to offer the “Your Brain Matters” program that provides support and guidance for people with early onset Alzheimer’s disease.

We are proud to offer AlzBetter, a specialized Alzheimer's training and service

program which places ongoing knowledge and expertise into the hands of those who need it most: our caregivers. Our partnership with the YMCA gave our Adult Day clients the opportunity to exercise weekly, which helps them to improve balance, coordination, increase strength and range of movement, and keep joints healthy.

In our Caring Companions program, we served 336 people; 40% of whom

were over 80 years old. The support provided included transportation to doctor appointments, light housekeeping, meal preparation, personal care, medication reminders, help with laundry, and companionship.

With Partnerships Throughout the State

Children The Madeline A. Galeucia Child Development Center received accreditation

from the National Association for the Education of Young Children (NAEYC), the nation’s leading organization of early childhood professionals.

Due to the generosity of Galeucia Family and the Cogswell Benevolent Trust,

our child development center students started the year off with a new playground that included resurfacing, a bike path, soft bark mulch, climbing structures, a large sand box, and a sensory table.

Through the Neighborhood Health Improvement Strategy, we partnered

with the Manchester Health Department to offer a variety of supports at Beech Street and Gossler Park elementary schools. We launched our HiSET® preparation program and began offering parent education and parent support groups. This partnership allows us to ensure all children and families have the early developmental support they need for a healthy start.

We began offering Positive Solutions for Families, a parent education

program designed to promote positive and effective parenting which helps parents respond successfully to children’s challenging behavior, as well as improve children’s social, emotional, and mental health.

Family Centered Early Supports and Services (FCESS) has seen a vast

increase in the number of DCYF referrals for babies who are exposed prenatally to opioids and often are born addicted and having to endure withdrawal. The developmental effect on the children is unpredictable, with some babies adjusting well and others significantly delayed. As a result, our outpatient program continues to expand, fulfilling the short and long term therapy needs of children.

Changes at the state level necessitated that FCESS integrate services for the

visually and hearing impaired children in our program. From our staff, we recruited vision and hearing specialists and are now compliant with this state guideline.

FCESS had the annual on-site State Monitoring Visit by the DHHS staff.

Our program achieved 100% compliance on all aspects of our operations. As this is the third year consecutive year we have achieved 100% compliance, future visits will be done virtually through the state database.

Impacting Our Communities

6 6

Employment We worked closely with NH Vocational Rehabilitation community mental

health centers for adults needing help with employment supports, job placement services, and job exploration services

Our Community Based Services (CBS) staff developed a high risk and high

need consultation team to address the clinical needs of the growing and complex Mental Illness (MI) and Intellectual Developmental Disability (IDD) population. We now have the ability to obtain funding for this team to consult outside of ESNH and two vendors have been secured. Also, in partnership with the Bureau of Developmental Services, we developed and implemented a Clinical Foundations Training for front line staff working with this high risk and high need population.

We secured a contract with Great Bay Community College to deliver a “Social Strategies for Adult Learners Group” Curriculum that we developed for college-aged students.

Youth Transitional Services worked with 30 NH school districts to provide

individualized community-based programming to 103 students, ages 12 to 21. Additionally, we supported 66% of students in volunteer positions at non-profit organizations, 51% of students in work-based learning experi-ences at for profit businesses, and 23% of students in paid employment.

Easterseals Independence Cleaning provided a variety of specialty floor-care and general cleaning services at five local business and seven ESNH facilities, thereby creating employment opportunities for veterans and individuals of all abilities.

We established a partnership with local recycler Apparel Impact to explore

ways to reduce textile waste, create employment opportunities for our clients and provide low cost clothing to families in need.

And more program highlights. . . Timberland selected Easterseals and the “green” renovation efforts at the

Robert B. Jolicoeur School to be the recipient of funds from their annual employee sample sale. The final donation was $41,560!

We are excited to expand our services at the Robert B. Jolicoeur School to students in grades K-2 so that this younger population will receive the specialized education it requires.

Our Dental Center, sponsored by Northeast Delta Dental, instituted an income-sensitive sliding scale. As one of the few dental centers in the state that accepts Medicaid, this enables more patients to access care.

Funded by the Citizens Bank Foundation, 17 students, ages 18 to 40,

completed our HiSET® program which provides group and one-on-one opportunities to study for and take this high school equivalency test.

Where We Live, Learn, Work and Play

Staff & Agency On June 29, 2016, we celebrated the 80th Anniversary of Easterseals NH.

What started as a small store front in Nashua has thrived and allowed our organization to expand capacity and programs, currently serving 26,912 individuals through 90 unique programs.

We’re off and running with our Big Ideas for Building Independence Capital

Campaign for the renovations at Auburn Street. The project was inspired by a generous gift from PROCON and the Stebbins Family in tribute to former Easterseals NH Board Member and PROCON Vice Chairman John Samenfeld. Upon learning of the Stebbins Family Gift, Tom Raffio and the Board of Northeast Delta Dental quickly voted funds to allow the expansion of our Dental Center, giving the Easterseals Board to extra impetus needed to approve the campaign.

When the renovation is completed next year, Easterseals NH will offer the largest array of services under one roof for children, adults, and seniors with disabilities and special needs.

Congratulations to President and CEO Larry Gammon, who celebrated his 45th

anniversary at Easterseals NH. What a tremendous gift he is to this organiza-tion and the people we serve. Larry was also recognized with an honorary doctorate degree from Granite State College.

Our employee wellness program is thriving. 465 employees (63% of those enrolled in our benefits) completed their biometrics on-site or with their physician. Of this number, 22% maintained or improved their overall health GPA from last year. 36% of our staff participated in some form of wellness program, an 8% increase over last year.

We know our strength comes from within, and have developed the leader-ship academy with Granite State College so staff can advance their skills and become more engaged in our organization.

Events The NH Snowmobile Association celebrated more than four decades of

fundraising for Camp Sno-Mo, raising more than $101,000 for this year’s Ride-In, bringing the 44-year total to $3.05 million.

We knocked it out of Veterans Park for the 17th Annual Eversource Walk With Me & 5K Run for Easterseals. Our new partners at Eversource helped us take this beloved event to a whole new level, by adding a 5k component managed by Millennium Running, garnering strong media coverage by Binnie Media and WBIN, attracting 2,500 participants, volunteers, and spectators, and raising $250,000.

We had huge day at our Annual HK Powersports Land & Lake Poker Run for Easterseals at the Naswa Resort. This was our most successful year in the history of the event, raising over $95,000 and bringing the 16-year total to more than $1.026 million.

Ron Ribeiro joined the Army at the age of 19. Growing up in a rough neighborhood, Ron didn’t always make the best choices and felt enlisting would make him part of a productive team. Ron was deployed to Iraq in November 2005 when he was 21-years-old. He served as a rear gunner on a Humvee. In his first few days, his convoy was involved in a brutal ambush—an event that set the tone for the rest of his 12-month deployment. In fact, this was the first of Ron’s two deployments that would result in financial and legal issues, a drug addiction, and a complete downward spiral. Despite the fact that in 2009 Ron was granted an honorable discharge from the Army as an E5 Sergeant, due to his years of service and accomplishments, he had reached an all-time low. He was also suffering from PTSD and had little hope for himself. “The only way out was death,” he said. “I went from a war hero to a degenerate.” In 2012, Ron was living with his girlfriend and searching online for fuel assistance when he came across Easterseals NH’s Veterans Count. He learned the program provided a hand up to veterans and decided it was time to ask for help. “I was tired of living with the demons and having nightmares and flashbacks,” recalls Ron of this very dark time. Help came in the form of Easterseals Care Coordinator Peter Collins. The day they were to meet, Peter arrived at Ron’s house. Although he was at home, Ron told Peter he wasn’t. Peter waited outside for hours to no avail. “He was trying to help me and I was waiting for him to leave,” said Ron. A week later Ron and his girlfriend broke up and he was homeless. He only cared about was drugs and “everything else was a distant second.” When he learned Ron was homeless, Peter reached out and found him a hotel room, where they also for the first time. Peter made sure Ron had food and Ron began to trust Peter and the help from Veterans Count. With Peter’s help, Ron checked himself into a drug detox program just a couple of days later. It was the start of a new life.

Peter and Ron have formed a strong, trusting bond. They’ve worked together to rebuild Ron’s life and deal with his demons. “Peter saved my life,” said Ron. “Without him I wouldn't have had the opportunities that I have now. I was incapable of enjoying anything that brought me happiness.”

Today, Ron is employed and putting his life back on track. He is grateful for all he and Peter have accomplished. He says, “I'm happy living in my own skin. I've got my head screwed on straight and I’m proud of myself again.” Ron has been sober for nearly three years and he credits Easterseals. “You have given me everything that I thought I couldn't get, everything that I thought would take a lifetime to fix,” he said. You’ve inspired me to help myself.”

“I had a mind I couldn't trust and a disease that wanted me dead.” —Ron Ribeiro, Military & Veterans Services Client

Veterans Count, the philanthropic arm of our Military & Veterans Services and founded by Michael Salter, continues to thrive with volunteer chapters in Manchester, Nashua, the Lakes Region and the Seacoast. This year we celebrated our 10th Anniversary and the milestone of serving 10,000 service members! Our volunteers are outstanding! In the past year, more than 150 volunteers held 35 events and raised a total of $2,006,600 to help our military members and their families. Veterans Count provided more than $475K in emergency and critical financial assistance, which ensures a hand-up to self-sufficiency.
