Employee Benefit Guide 2015
WHO IS ELIGIBLE FOR BENEFITS? You are eligible to enroll in benefits if you are a full time, benefits eligible employee working an average of 20 hours or more a week and are also actively at work at time of enrollment.
Open Enrollment Details Open Enrollment will begin Monday, August 3rd. There will be Open Enrollment meetings held on August 3rd at the RESA location and August 6th at Rutland Academy. During these meetings, you will be able to schedule an appointment to sit with an enroller. Enrollers will be available all day on August 4th at the RESA and August 14th at Rutland Academy. Please make sure to attend the Open Enrollment meeting to sign up with an enroller.
Detailed benefit information can be found at MyNEGARESABenefits.com. You may also call the Northeast Georgia RESA Benefits Call Center at 1-877-422-3312 with any benefit questions. This call center is open year round!
WHAT IF I DON’T NEED TO MAKE A CHANGE? Due to plan changes, every full time employee will be required to go through the enrollment process, even if you choose to decline all benefits.
We all face risks in our lives. Some are large, and some are small. When deciding on your benefits, look at covering the largest risks that most people face. We call these the Core Four.
1. Health Insurance 2. Disability Insurance
3. Life Insurance
4. Savings
These benefits provide protection for possible unexpected injuries and illnesses in life. They also protect you against losing your income if you are unable to work for a period of time. As you review your benefits and make decisions to protect you and your family, start with these benefits first.
Once you have these four benefits in place, consider the other options that are offered.

HOW TO VIEW PLANS ONLINE
1. Visit ‘MyNEGARESABenefits.com’ and click the green button “Start Here for Information.”
2. In the Employee Benefit Center, you will find dropdowns for all benefit information under “My Health,” “My Life” and “My Savings.”
3. Click on each benefit for more detailed information.
Every full time benefit eligible employee is required to go through the enrollment process this year.
Dependent children may be covered up to age 26, regardless of student status
Duplication of life, dental or vision coverage will not be allowed for spouses or dependents who both work in Northeast Georgia RESA.
Be sure to review plan details on MyNEGARESABenefits.com before enrolling.
Pre-existing condition limitations may apply on some of the plans. Please review the plan documents and policy details.
1. Visit ‘MyNEGARESABenefits.com’ and click the blue button “Enroll Here.”
2. Enter user name and initial password using login instructions at the top of the page and create a new password when prompted.
3. Follow prompts to update personal and dependent information. To cover a spouse or child(ren), enter them as dependents.
4. Continue through ‘Benefits Plan Enrollment’ to elect benefits and then add beneficiaries.
5. After completion, the ‘Consolidated Enrollment Form’ will show current and future benefits. Print this page for your records.

Short Term Disability Insured by Mutual of Omaha
You may cover 40%, 50% or 60% of your weekly salary The benefit will start after a 14-day waiting period should you
become disabled due to an accident or illness (including maternity)
The plan offsets with sick leave and other sources of income The benefit will pay up to 11 weeks There are no health questions to enroll this year
Long Term Disability Insured by Mutual of Omaha
Provides income protection up to 60% of your gross monthly income
The benefit will start after a 90-day waiting period should you become disabled due to an accident or illness
The plan offsets with sick leave and other sources of income The benefit will pay up to Social Security Normal Retirement
Age (SSNRA) There are no health questions to enroll this year
Voluntary Term Life Insured by Mutual of Omaha
5x salary up to $500,000, Guaranteed Issue (GI) $200,000 Spouse coverage is available up to 100% of the employee
election to a maximum benefit of $500,000, GI $50,000 Child(ren) Term Life Insurance is available up to $10,000 Employee must purchase Term Life and AD&D coverage for
spouse and child(ren) to be eligible If your employment ends, the plan is portable or convertible
Permanent Life Insured by Unum
Guaranteed Issue (no health questions) up to $50,000 for employees
Rates are locked and guaranteed for life Whole Life policy that builds cash value Coverage is available for spouse and children up to $15,000
