Remaining item from previous 1.1 TET requested ABB to submit LOTO procedure and asked

about IRPC procedure. LOTO

ABB was already submitted LOTO procedure to TET. TET

waiting IRPC procedure to comply with.

1.2 K.Pongsapak requested PTEC to provide project information

board at Lay Down Area.

1.3 K.Pongsapak reminded about HSE booklet/handbook.

- All to provide ( ABB=150, MACS=125, TCC=125, PTEC=300

& Dialog=300). Confirm price 25.50 bath/1 book. Delivery date

before shut down (10June2013)

1.4 TET requested all to prepare scaffolding log book.

1.5 TET will set schedule for training safety personnel and fire

watch warden soon. TET set meeting all safety and fire watch

personnel every Monday. TET invited IRPC safety team to join

for discussing about safety issue at shut down period.

1.6 TET waiting detail of lesson learn - IRPC about electrical

shock on scaffolding so all need to be careful about avoid

hanging electrical cable on scaffolding or else use S hook with

the effective insulator.

1.7 TET informed about TET home office audit #02 results is all

to improve about pressure cylinder storage. Chain was needed

for cart.

machine hit to fire hydrant hand wheel. 3. On June 19, 2012 Property damage case (TBC) tremie pipe

stuck in drilling hole. 4. On January 19,2013 Near-miss case (AS First) smoke from

combustible material cover oil sand.

5. On March 5,2013 Property damage (Dialog) 2 boxes fell

down from forklift

6. On March 6,2013 Property damage (PTEC) found insulation

jacket damage by PTEC workers

General / Information 3.1 IRPC invited all safety personnel to join the meeting about

EBSM plant turn around period on May 23, 2013 at 8.30 -12.00

am.

3.2 TET informed that PTEC need to move 4 tents between

Dialog and ASF within this Friday (May 24, 2013).

3.3 TET reminded to all about OT request need to submit before

16.00 pm

3.4 About smoking area at lay down area need the good

housekeeping and schedule to all contractors to take care by

ABB - Monday, Dialog - Tuesday & Wednesday, MACS

Thursday & Friday, TCC Saturday & Sunday.

3.5 TET reminded about no hand over anything between EBSM

fence and outside.

3.6 IRPC reminded about hanging all bags along fence.

3.7 MACS informed about flushing and test work is ongoing and

the pressure is not over 10 bar.

3.8 TET requested PTEC to provide 10 workers for

housekeeping at height starting May 23, 2013 afternoon.

3.9 TET informed that about parking vehicle in front of lay down

area is permitted by memo of K.nares to K.Rachin as attached.

Comment 4.1 Observed 2 empty drums obstruct the access to IRPC waste

bin.

4.3 Observed TCC worker try to fix the hook of land yard by

4.6 Observed PTEC workers were sitting under scaffolding

storage. Line of fire.

4.7 Observed Ensure hard barricade to protect the fire hydrant

obstruct the access to fire hydrant in case of emergency.

4.8 Observed the waste is not segregated in bag and the

unused material not remove.

4.9 Observed the under reactor unused material scatter all area.

Ensure to remove ASAP.

4.10 Observed the argon pressure cylinder on cart need to have

more chain to lock.

4.11 Observed the access to the plant fire extinguisher was

blocked by scaffolding at area 05. PTEC need to move fire

extinguisher to outside and noted that the fire extinguisher

shelter was damage before relocate it as picture attached.

4.12 Observed PTEC 05 working area need to do housekeeping

ASAP.

4.13 Observed TCC fire watch warden wearing safety shoe not

meet standard.

4.14 Observed PTEC painting work need a container for all

paint can.
