    before shut down (10June2013)

    1.4 TET requested all to prepare scaffolding log book.

    1.5 TET will set schedule for training safety personnel and fire

    watch warden soon. TET set meeting all safety and fire watch

    personnel every Monday. TET invited IRPC safety team to join

    for discussing about safety issue at shut down period.

    1.6 TET waiting detail of lesson learn - IRPC about electrical

    shock on scaffolding so all need to be careful about avoid

    hanging electrical cable on scaffolding or else use S hook with

    the effective insulator.

    1.7 TET informed about TET home office audit #02 results is all

    to improve about pressure cylinder storage. Chain was needed

    for cart.

    1.8 TET requested all to submit the Safety certificate of

    General / Information 3.1 IRPC invited all safety personnel to join the meeting about

    EBSM plant turn around period on May 23, 2013 at 8.30 -12.00

    am.

    3.2 TET informed that PTEC need to move 4 tents between

    Dialog and ASF within this Friday (May 24, 2013).

    3.3 TET reminded to all about OT request need to submit before

    16.00 pm

    3.4 About smoking area at lay down area need the good

    housekeeping and schedule to all contractors to take care by

    ABB - Monday, Dialog - Tuesday & Wednesday, MACS

    Thursday & Friday, TCC Saturday & Sunday.

    3.5 TET reminded about no hand over anything between EBSM

    fence and outside.

    3.6 IRPC reminded about hanging all bags along fence.

    3.7 MACS informed about flushing and test work is ongoing and

    the pressure is not over 10 bar.

    3.8 TET requested PTEC to provide 10 workers for

    housekeeping at height starting May 23, 2013 afternoon.

    3.9 TET informed that about parking vehicle in front of lay down

    area is permitted by memo of K.nares to K.Rachin as attached.

    Comment 4.1 Observed 2 empty drums obstruct the access to IRPC waste
