Institution Nomination Form

1.Student’s Contribution to Co-op/WIL – Provide specific examples on how the student

represents her/his academic institution internally and/or externally such as creating

marketing/promotional materials, participating in networking/recruitment events, serving as an

ambassador for the institution and department, or acting as a mentor to other Co-op/WIL

students. (600 words max.)

2.Extracurricular Activities and Achievements – Provide specific examples of how the student

demonstrates campus and community involvement such as regular involvement in clubs,

student government, community organizations, religious groups, volunteering, sports and

leadership. (350 words max.)

3. Academic Achievements – Provide student’s cumulative GPA score to date and list any

academic achievements such as scholarships and awards. (100 words max.)

4. Student Biography - Provide a short biography that will be used by EWO (press releaseand/or website announcement of the winners) once the awards are granted. (150 words max.)
