days by 31stAugust,10. The LTI injury accidentwas on 27.01.2010

Incident Details

26.6.2010- PE bagging- Sweep drum tumbled and fell down hittingperson near eye during trolley pushing job.First aid injury.

14.07.2010- ETP-Sulfuric acid spillage due to failure of tanker from

welding during pressure transfer of the acid.

01.09.2010 Truck lane-Fall& crush by trailer during attempt for free

ride-fatal road accident.

04.09.2010- Injury on feet due to struck by falling radiant tube- LTI

Tanker/Trailer condition to be checked with checklist

before it is released from main gate . Leaky sulfuric acid

tanker came in ETP plant (11/09/10)

Many heavy trailers run adjacent to PE plant, newloading bay work to be expedited. ( Job will get

completed in this month. )

Audit by B.S.C .is scheduled on. 6th of December to10th December

Road traffic Control--- Security to accept ownership.

PPE compliance needs further strengthening.

Risk assessment of all jobs to be done in plants/ facilities

by Mechanical ,Civil, Electrical , instrument and non routine

process job.
