New Members of the Executive Committee -2011

We are pleased to introduce the two new members of the Hazards Forum’s ExecutiveCommittee mentioned in the previous Newsletter (No. 71), seen below with Forum chairmanPaul Thomas after their first Executive Committee meeting on 14 th June 2011.

Dave Fargie Luise Vassie Chairman, Paul Thomas

As reported in Newsletter 71, both members were welcomed to the Committee at the AGMon 22 nd March. They are already known to many members of the Forum.

Dave Fargie BEng CEng FIChemE

Dave Fargie is a Chemical Engineer with 23 years experience in the petrochemicals, refiningand oil & gas sectors.

He graduated from Exeter University in 1987 with a BEng in Chemical Engineering. Sincethen he has held a variety of process and process safety engineering roles within BP andMobil.

Dave initially worked in BP Chemicals with periods at the Hull Salt End manufacturing siteand the central engineering team in London. He then moved to Mobil Oil spending severalyears based at Coryton Refinery performing operations and project support activities. Davethen spent several years as a senior chemical engineer in the BP Chemicals PolyolefinsLicensing group. For the last 9 years he has specialised in process safety engineering in the

BP upstream area and currently holds the position of Process Safety Discipline Lead for theBP Exploration and Production upstream engineering team.

Dave is a Chartered Engineer and Fellow of the Institution of Chemical Engineers, he sits onthe EEMUA Pressure Relief Committee and is a committee member of the IChemE Safetyand Loss Prevention Subject Group.

Dr Luise Vassie BSc PhD MInstP CFIOSH

Luise is Executive Director, Policy, at IOSH. She joined IOSH in 2005 as Research andTechnical Services Manager, before becoming Head of Research and Technical Services in2008. She has been a member for more than 10 years. Luise leads the Policy division,including public affairs, research, technical and communications activities. She isresponsible for leading the strategic development and communication of IOSH policy,research, technical advice and guidance. This includes lobbying to raise IOSH’s profile,influence and position as a health and safety thought leader.

Prior to joining IOSH, Luise spent 14 years working in academia carrying out research andconsultancy and delivering postgraduate teaching and training in health and safetymanagement. She has a PhD in laser physics and is a Chartered Fellow of the Institution.

Keeping the Country Running: Government’s

Approach to Infrastructure ResilienceJames Kearns

On Tuesday 14th June 2011 the HazardsForum hosted an evening event. Theevent was sponsored by the Institution of Civil Engineers and was held at their premises in Westminster, London.

This event was concerned with thevulnerability of national infrastructure tohazards and threats, and how buildingresilience into our infrastructure canreduce this vulnerability. This can beachieved by, for example: improvingprotection; encouraging ability inorganisations and their infrastructurenetworks and systems to absorb shocksand recover; and enabling an effectivelocal and national response toemergencies.

The intention of this evening event was toprovide an insight into how Government istackling these issues, from theidentification of the risks, through to thepolicy initiatives to reduce those risks andalso to look at a new document (near to

publication) giving recommendations.

The event began with a few brief wordsfrom Hazards Forum Chairman Rear Admiral (retd) Paul Thomas CB , whowelcomed the audience and thanked theInstitution of Civil Engineers for sponsoring the event. He then welcomedchair for the evening Mike Granatt CB.Mr. Granatt, who set up the CivilContingencies Secretariat at the CabinetOffice, thanked the Hazards Forum for holding this timely event, and reaffirmedthe importance of reducing the

vulnerability of critical infrastructure, asany such failure could have very seriousconsequences.

There were three speakers at this event.The first speaker was John Tesh, DeputyDirector of the Civil ContingenciesSecretariat at the Cabinet Office , whogave a talk titled “ National Risk Assessment ”. In this talk, Mr. Teshexplained how Government assesses andmitigates the most significant risks to theUK. He also explained the background tothis important work and demonstrated theway in which it has been developed, andalso where the work is heading. Followingthis was a presentation by Dr. Mat

Barber, also of the Civil ContingenciesSecretariat at the Cabinet Office , whosetalk was titled: “ Keeping the Country Running: National Hazards and Infrastructure ”. This talk presented thelatest policy thinking on approaches tobuilding resilience in the UK’s nationalinfrastructure, particularly in respect of natural hazards. Dr. Barber alsodiscussed the guide on infrastructureresilience, including the approach to riskassessment of natural hazards, resilience

standards and information sharing, whichhad undergone public consultation earlier in the year (in the Spring) and was due tobe published later in the year. The finaltalk of the evening was given by a senior representative of the Centre for theProtection of National Infrastructure .This talk explained how an integratedprotective security approach aims toreduce the vulnerability of the UK’s criticalnational infrastructure to national securitythreats. Also discussed was howGovernment, infrastructureowners/operators and the CPNI worktogether to achieve common objectives.

Mr. John Tesh began the first of theevening’s talks by explaining theGovernment’s approach to assessing UKsecurity risks. Ensuring resilience to suchthreats is now an objective of the CivilContingencies Secretariat, who aims toachieve this by protecting the UK’speople, economy, infrastructure, territoryand way of life from all direct major risks.

The approach to security risk assessmenttaken by the Civil ContingenciesSecretariat is to prioritise such risksaccording to their impact and likelihood.The highest priorities are classed as “Tier 1” risks. These include hostile attacksupon UK cyber space and large scalecyber crime, international terrorismaffecting the UK, including chemical,biological, radiological or nuclear attack byterrorists, an international military crisisdrawing in the UK, and a major accidentor natural hazard requiring a nationalresponse, such as flooding or influenzapandemic.

Mr. Tesh then discussed some of thefuture challenges to the UK’s domesticresilience strategy. The UK’s risk profilewas not considered likely to change much.The profile is diverse, with no single riskdominating, as well as being complex andunpredictable, with links randomly andsuddenly emerging between events. Therisk profile, however, was affected byfactors such as climate change, globalinstability, societal change, and

unforeseeable risks of cascading failures.Management of this risk profile requiresgeneric emergency response capabilities,

specific plans for catastrophicemergencies, improved socio-economicresilience, and also a need for a constantawareness of the risks and how they maybe changing.

Some more detail regarding how thelikelihoods and impacts of the specificrisks were assessed. Risks are classifiedas either “threats” or “hazards”. For hazards, likelihood is assessed fromhistorical evidence, predictable events,such as weather forecasts, and expertadvice. For threats, plausibility, rather thanlikelihood, is assessed. Plausibility isdetermined from intelligence aboutcapability, intent and vulnerability. Impacts

are calculated from a ranking of thenumber of fatalities, casualties, economicdamage, psychological damage, andsocial disruption.

Mr. Tesh then discussed some of thereasons for a National Risk Assessment.Such risk assessments providepolicymakers with information to steer mitigation investments toward their greatest economic and societal benefit.Without a tool such as the National Risk

Assessment, it may be difficult for toplevel policymakers to make informeddecisions on the relative benefits reducingrisks to public health, safety or security.The efforts of the UK have also beeninternationally recognised, along with theNetherlands, as standing out as the bestpractices in this regard, for producing toolsto help high level policy makers comparemultiple risks.

Dr. Mat Barber continued the theme of infrastructure resilience by discussing howto keep the country running in the event of a natural hazard occurring.

The need to improve infrastructureresilience was highlighted in the PittReview of the 2007 floods 1. Whendeveloping policy to address the resilienceof infrastructure, three key themes wereidentified:

What are the hazards? How to improve resilience? How to improve understanding?

The Cabinet Office had recently publisheda document 2 addressing these issues, aswell as offering guidance on other featuresof infrastructure resilience, which wastitled “Natural Hazards and Infrastructure”.

The first of these three issues – “what arethe hazards?” is addressed in thedocument by presenting the reasonableworst case scenarios for natural hazards,as determined in the National Risk Assessment. The document also looks at

the linkages between the initialconsequences of a hazard and the knock-on consequences. For example, the initialconsequence of a prolonged period of hotweather is heat, while the knock-onconsequences are thunderstorms,drought, dust/smog or haze, landinstability and wildfire.

The second issue – “how to improveresilience?” is dealt with by analysing four components of resilience – resistance,redundancy, reliability and response &recovery – and understanding thatinfrastructure resilience is achieved byapplying different approachesproportionally to networks and systems.For example, the most critical assetsmight be physically defended from a risk(resistance) whereas less critical assetsmay rely on a simple business continuitystrategy (response and recovery).

The third issue – “how to improveunderstanding” – is tackled byconsiderations of governance andorganisational resilience, which aims to

enable infrastructure owners securecontinuous improvement in their resilienceto natural hazards by embeddingorganisational resilience in day to dayoperations and strategic decision making.

The final talk of the evening was given bya senior representative of the Centre for the Protection of NationalInfrastructure (CPNI), who discussed

further the Government’s approach toinfrastructure protection. CPNI is the UKgovernment authority that providesprotective security advice to the nationalinfrastructure. It helps to minimise risk tothe national infrastructure by deliveringauthoritative advice to reduce thevulnerability of such infrastructure toterrorism and other threats. CPNI has anadvisory role, and is not a regulatory body.It takes a holistic approach that is impactdriven, vulnerability focused and threatinformed. It works in a tripartiterelationship with owners and operatorsand sponsoring Government departments,as well as conducting a wide-rangingresearch and technology programme.

CPNI provides integrated advice regardingphysical security, electronic security andpersonnel security & behaviouralassessment in order to reduce

vulnerability in the national infrastructure.National infrastructure is divided into ninesectors, such as communications,

emergency services and energy. Eachsector is different, although not allinfrastructure is critical.

CPNI has introduced a system for categorising infrastructure according to itsvalue, or “criticality”, determined on thebasis of the impact of its loss. Thecategorisation is done using theGovernment “Criticality Scale”, whichassigns categories for different degrees of severity of impact. Category 5 indicatesinfrastructure the loss of which would havethe most severe impact, whilst category 0indicates infrastructure whose loss wouldbe minimal when considered in thenational context.

Some of the more recent developments incritical infrastructure risks which manyindustries are seeking advice on are cyber security risks. It was explained that animportant facet of this risk is the “insider threat”, such as employees using their authorised access for unauthorised

purposes. These risks are difficult toprotect against and can lead toinfrastructure becoming severely

compromised. One way in which theserisks can be mitigated is by identifyingsuspicious behaviour.

Mike Granatt then thanked the speakersfor their presentations and remarked thatthe issues posed by critical infrastructurewere very complex and that a lot of goodwork was being done in ensuringresilience. He then opened the floor for questions.

Opening the discussion, Professor JimNorton , currently president of the BritishComputer Society gave a short talkabout his experience in helping to puttogether a disaster recovery plan following

a major accident affecting criticalinfrastructure. He then asked about whatcould be done to manageinterdependencies and stop cascadingfailures in an emergency, and also how toget the right balance between electronicand physical threats, as companies haverecently been spending too much on cyber threats and neglecting physical issues. Inresponse to the first question, John Teshmentioned that such interdependenciesand cascading failures are often

unforeseeable and cannot be prevented.He also mentioned that there will be moreinterconnectivity in the future. Mat Barber mentioned that the USA and Australia hadtried to model such interdependencies, buthad been unsuccessful.

Another question was how to ensure thatit was possible to get enough people onthe ground to help during an emergencyand whether there are enough skillsavailable, to which it was replied that therewas a problem with information sharing,and a need to try and get morecommunications in such a situation. Other questions included whether the CabinetOffice accounted for societal risks, suchas by including a dread factor, andwhether there is too much communicationfollowing big events, where, for example,media interest may hinder the responseand recovery process.

John Tesh then summed up withconcluding remarks in a short talk. Hecommented that risks are always

changing, and that there is a need to takea long term view. The Civil ContingenciesSecretariat and the CPNI are workingtogether to ensure resilience of trulycritical infrastructure. This work needs joined up thinking between Governmentdepartments and also needs tounderstand and communicate the risksbetter. In addition, there is also a need for an effective tripartite relationship betweenthe Government, regulators and industry.

What is SIESO? Originally establishedin 1953 as the Society of Industrial CivilDefence Officers (SICDO), it became theSociety of Industrial Emergency Service

Officers in 1972. Now known simply asSIESO, it is the professional associationfor individuals whose interests lie in theprevention of industrial or commercialaccidents, and in the planning for andresponse to them. Its aim is SharingInformation and Experience for Safer Operations which it achieves through:

Identifying good practice throughliaison with industry, commerce,the emergency services, centraland local government and theregulatory bodies.

Providing opportunities for discussion on the avoidance andmitigation of the effects of major incidents amongst those involvedin the planning and managementof a response to an incident.

SIESO is represented on national bodiessuch as The National Steering Group onWarning and Informing the Public, TheChemicals and Pipelines EmergencyPlanning Liaison Group and is alongstanding member of the HazardsForum. Throughout the United Kingdom itenjoys a sound working relationship withthe regulatory bodies associated withregulations and guidance and worksclosely with professional associations andinstitutions in sponsoring events of mutualinterest. These include a jointSIESO/Hazards Forum conference onCommunicating Risk that was held at theHome Office Emergency PlanningCollege. It also continually seeks tobroaden its links with industry, commerceand appropriate trade associations. TheNational Council comprising its officersand other elected members managesSIESO. Notably, it is a not for profit

organisation and ensures itsindependence by never seekingsponsorship for its events.

Publications. In 1956 SICDO produced“The Industrial Defence Planning Manual”with a revised second edition appearing in1961 when membership stood at around1000. With the demise of the CivilDefence organisation in 1968, no further editions were produced but in 1985, theNational Council established a working

group to prepare guidelines for industrialists and others to achieve a highstandard of preparedness for theprotection of persons and their environment in the event of a major emergency. The resulting “Guide toEmergency Planning” was enthusiasticallyendorsed by the HSE which itself set up aworking group, 50% of which were SIESOmembers, to produce its own formalguidance to responding to a major incident. To complement “The Guide toEmergency Planning”, in 1993 SIESOpublished the “Guide to Help in a Major

Emergency” which outlined the roles of the emergency services and other organisations that could be involved in theresponse to a major emergency.Throughout its existence SIESO hasprovided its members with a variety of magazines, newsletters, pamphlets andtoday electronic media are used to keep intouch.

Membership. Acting as the honestbroker between industry, commerce andthe regulatory authorities, SIESOwelcomes as members all those whoseduties or interests include safety and theprevention of, or response to, industrialand commercial emergencies. Our members embrace all aspects of businesscontinuity, emergency planning,communications and EU legislation andcome from many areas: industry,commerce, the emergency services, theregulatory bodies, central and localgovernment, the health authorities,voluntary agencies and the media,amongst others. While most memberswork within the United Kingdom, anincreasing number are based overseas.Members are invited to attend workshops,seminars, study periods and visits, all of which are noted for their professionalismand the competence and authority of thespeakers. Free membership is availableto undergraduate students registered onany full-time degree course inengineering, management or theenvironment. Other courses may behonoured if recognised by the SIESONational Council.

SEVESO and CIMAH/COMAHRegulations . Since the early 1980s ,SIESO has been proactive in bringing

together industry and the regulators indeveloping the both the CIMAH andCOMAH Regulations aimed at ensuringUK’s compliance with the EU SEVESO 1and 2 Directives. SIESO has hosted anumber of workshops providingopportunities for discussing thepracticalities of the proposed draftCOMAH Regulations before they wereimplemented in 1999. In ongoingworkshops, topics have included thevexed question of the content of SafetyReports, testing and exercisingemergency plans, competency, land useplanning, site security, corporateresponsibility, plume dispersion and riskassessment among many other key

issues. As SEVESO 3 approaches, on 16May 2011 SIESO hosted its 17 th COMAHWorkshop at the Manchester Conference

Centre. Over 100 attendees were able toquestion the competent authority’s latestoperational strategy, the EuropeanUnion’s proposals for SEVESO 3 and howcentral government is implementingprocedures for warning and informing thepublic about civil protection.

The Future. Employing the combinedknowledge and expertise of itsmembership and continuing its closerelationships with like-minded professionalinstitutions and associations, SIESO willcontinue its efforts to improve the safety of operations particularly within the highhazard industries.

From the Secretary …The evening event on 20 th September will take an innovative form for the Forum, withmore speakers than usual - all within the same time slot, however. The discussion periodwill be with an expanded panel where comments and ideas, as well as questions, will beactively encouraged from those present to further stimulate the discussion period. Thisevent can be regarded as a follow-on from the Forum’s evening event in June and takes thetopic of infrastructure resilience further. More information is available on the Forum’swebsite. (The write-up of the June 2011 event can be seen at page 3 of this Newsletter.)

Members will be delighted to know that the Forum’s community is growing - from newIndividual members to Corporate Associate members now joining.

HSE eNews++ Site Licence Application for Hinkley Point C Received ++The Office for Nuclear Regulation (ONR) has received an application from NNB GenerationCompany for a nuclear site licence, relating to its proposed development of a new nuclear power station in Hinkley Point, Somerset.

++ RR878 – Levels of Respirable Dust and Respirable Crystalline Silica atConstruction Sites ++The purpose of this pilot study was to assess the potential for inadvertent exposure of thepublic to respirable crystalline silica (RCS) from construction activities.

The study assessed the respirable dust (RD) from, demolition, block cutting, road building,general construction activities and city centre air from 13 visits to 7 sites. In total, 48 samplesfrom the construction activities and 11 city centre air samples, for comparison, werecollected.

The results obtained for RD and RCS were generally very low. Only 10 % of results (fromtwo sites) for RCS were above 0.01 mg.m-3, which is 10 % of the current Workplace
