Employee…use to request an Ergo Eval for yourself

Click “Request My Evaluation” and the form will automatically populate

with your employee information.Just select “Reason for Evaluation” from the dropdown menu and click

“Request Evaluation” button

Supervisor…use to request an Ergo Eval for employee

Click “Request Evaluation” and enter the employee’s name.

Just select “Reason for Evaluation” from the dropdown menu and click

“Request Evaluation” button

Supervisor… use to check on Evals of My Direct Reports

Enter Supervisor name, click Submit and view status of your direct reports

Supervisor…use to complete Actions Pending items

3. When new equipment or modifications are in place,

the Supervisor should change the status from

Pending to Completed to close out the Eval

1.Supervisor should discuss Ergo Issues and Recommendations with Employee and agree on

the course of action

4. Always SAVE FORM when you’ve completed your entries

2. Supervisor should order/procure all products

and services listed in Actions Pending sections

All Users…use to communicate within the Ergo Database

Use the communications log to update the Employee’s record and communicate with the Evaluator and other parties to resolve

issues such as…- Changes in the course of action you and the employee decide on - Problems you might have implementing recommendations.

It’s also very helpful to record Employee’s progress or satisfaction with solutions.

Click Add to make a new entryClick Refresh to bring up the entry in logClick the Edit Icon to modify the entry

Click up/down Arrows to reverse date sort

Supervisor… use to print and review Employee’s Eval Report

Actions Pending items are clear and concise to convey

required actions that must be taken by Supervisors.

These pending items will also appear in Supervisor’s email

reminders

The employee’s ergonomics issues and recommendations are

at front of report for quick review

All communications and updates are captured in the Case Management Log and

these updates will be flagged in

My Direct Reports and Eval Status Screens

Click View Report in Eval Form
