At the heart of every decision we make at Southwestern Vermont Health Care is our mission:

to provide exceptional care and comfort to the people we serve.

Our focus on caring was most notably evidenced by our nursing staff earning its fourth

Magnet® recognition, the highest honor an organization can receive from the American Nurses

Credentialing Center.

Our connection to the community was strengthened through new initiatives. Most significantly,

we established and expanded our presence in many of the communities we serve, ensuring that the

type of care required most is available when and where it is needed.

In addition to strengthening our partnership with Dartmouth-Hitchcock, we forged new relationships

with like-minded organizations in the area and are collaborating on efforts that aim to build

healthier and more vibrant communities.

We are extremely proud of all that we accomplished in 2016 and pleased to share stories of how our

caring, connecting, and collaborating approach has enhanced the care we deliver and benefitted

all we serve in our region.

At Southwestern Vermont Health Care (SVHC), clinical excellence and providing exceptional care and comfort go hand

in hand. These key measures are the tenets on which our health system was founded. Excellence cannot be attained

without exemplary employees, and we are proud to say we have many.

In 2016, Southwestern Vermont Health Care received several organizational awards and accolades, including an

“A” Rating for Patient Safety from the Leapfrog Group, and Consumer Reports’ highest score for hospital quality

and safety performance in Vermont and New Hampshire, which placed us third for all hospitals in New England.

These accolades follow our fourth Magnet® Designation—awarded every four years—which is the highest honor an

organization can receive for excellence in nursing and the highest standards of quality and safety in patient care. We

are particularly pleased with being recognized by Vermont Business Magazine as one of the Best Places to Work

in Vermont for the third year in a row and named to Becker’s Hospital Review list of “150 Top Places to Work in

Healthcare” for the second year in a row.

Central to all that we achieved last year is our commitment to caring, connecting, and collaborating with the

individuals, organizations, and communities we serve. This commitment was the impetus behind expanding and

enhancing care through the opening of our Pownal Campus and the renovation of our Northshire Campus; recruiting

providers for several of our established practices; having staff complete advanced clinical training; acquiring the

latest technology for our Cancer Center and Gastroenterology departments; and the ongoing partnership with

Dartmouth-Hitchcock. All of these initiatives will increase access to care, lead to earlier interventions, and improve

outcomes for our patients in the community, while allowing us to continue to provide a level of care on par with

significantly larger health care organizations.

On behalf of the Southwestern Vermont Health Care’s Board of Trustees, we want to recognize and thank our

physicians, associate providers, nurses, and staff for achieving the results highlighted in this Annual Report. Without

their talent, dedication, and teamwork, we would not have realized these accomplishments. And we want to thank

Providing precise cancer careSouthwestern Vermont Health Care is committed to providing exceptional

care close to home. Toward that end, the Southwestern Vermont Regional

Cancer Center is now one of the first in the area to be equipped with the

latest in linear accelerator (Linac) technology.

Delivering high doses of radiation to cancer cells in a much more precise and

faster manner than allowed by previous technologies, the machine improves

patient experience through reduced treatment times, fewer side effects, and

less tissue damage.

As explained by Radiation Oncologist Matt Vernon, MD, “One of the keys to

successfully irradiating cancer cells is being able to reproduce the precise

positioning of the patient and targeting the same cells at each treatment.

Through a system of laser and camera arrays that visually monitor the

position of the patient, VisionRT allows us to make very reproducible

Increasing access, enhancing careThe first day of the New Year saw SVMC

Orthopedics open its doors with an expanded

roster of physicians and a new approach to

providing care. Drs. Jonathan Cluett,

Suk Namkoong, and James R. Whittum,

(all formerly in practice at Orthopedic

Associates of Northern Berkshire) joined Dr.

Matthew Nofziger in our newly renovated

facility—significantly increasing access and

availability for patients.

Using a patient-centered approach to care,

the board-certified team is able to manage

problems including sports injuries, arthritis

and joint replacement, fracture management,

and orthopedic emergencies. They use the

latest in both surgical and non-surgical

techniques, including arthroscopic surgery,

along with a customized rehabilitation

program, to help patients get back to living an

active, pain-free life.

Exceeding expectations in careOver the past year, SVMC’s Gastroenterology department

was dramatically transformed and updated in almost every

imaginable way.

Dr. Adnan Sohail and Stephanie Sauer, NP, joined Dr. Ashley

Maranino, significantly increasing patient capacity and ensuring

short wait times for appointments.

A half-million dollar construction project enhanced workflow

and allows for more efficient reprocessing of scopes. An all-

new Automated Endoscopic Reprocessor elevates the level of

disinfection and allows the department to exceed all standards

related to infection control. Plus, the acquisition of the latest

generation scopes for two advanced procedures—EUS

(endoscopic ultrasound) and ERCP (endoscopic retrograde

cholangiopancreatography)—has brought the hospital’s

gastroenterology care to a level on par with many significantly

larger facilities. All of these investments have resulted in a

27 percent increase in cases handled by the department.

New Pownal Campus designed for care and comfort

The SVMC Pownal Campus—strategically developed less than one mile north of the Vermont-Massachusetts border—opened its doors to patients on October 19, 2015. The 5,000-square-foot family medical practice was keenly designed for our patients’ comfort and convenience, including a covered drop-off entrance, a comfortable waiting area, ten spacious exam rooms, a procedure room, and an onsite lab and X-ray facilities. The SVMC Pownal Campus team of physicians and associate providers offer care for children and adults, plus obstetric care, diabetes education, and nutrition counseling services. Since its opening, the practice has seen more than 12,000 patients.

Delivering care and compassionStaffed with four board-certified physicians and

a four-time Magnet-recognized nursing team,

SVMC’s OB/GYN department sets an example in

implementing quality improvement initiatives and

elevates the level of care provided to our patients.

Themarge Small, MD, FACOG, outgoing Chair,

Department of Obstetrics and vice president, Medical

Staff, notes, “One thing we do exceptionally well is

‘care’ for our patients. Not only do our providers

confer on patients to ensure all perspectives are

considered; we also work very closely with our

partners at Dartmouth-Hitchcock to care for high-

risk patients. We maintain open communications with

referring physicians so that the continuity of care

with their patient is maintained.”

In addition to Small, the physician team includes

Drs. Annika Chadee (right), Malcolm Paine, and

Kimberley Sampson. In partnership with the award-

winning nurses and other departments, SVMC is

able to provide OB/GYN patients with safe, quality

care delivered with respect for personal choices.

tsReconnecting patients to the lives they loveOf all people, dairy farmers

understand the value of routine and

hard work. That understanding paid

off for lifelong dairy farmer Lenny

Austin of White Creek, NY, when his

world was turned upside down by a

catastrophic farming accident.

The accident left Lenny in a coma

for three weeks followed by two

months of stabilizing. When he arrived

at SVHC’s Centers for Living and

Rehabilitation (CLR), he could barely

talk or move.

But over a period of eight months

involving intense daily physical,

occupational, and speech therapy,

Lenny slowly regained his mobility,

strength, and joy for living.

This past spring Lenny returned home

to his family and farm. While he never

wants to relive the experience, he

does say, “I miss the therapists at CLR,

and I’m grateful for what they were

able to do for me.”

“ I love my farm, and I enjoy my work,” Len says. “I care about my animals so much, I stay awake thinking about them.”

Michael and Barbara Powers of Manchester, VT, have a

deep and long-standing history with SVMC. In fact, Barbara

refers to going to the hospital as a “family tradition.”

“Michael’s a fifth-generation Vermonter who was born at

the hospital, as were many of his ancestors, our own two

sons, and even our granddaughters,” says Barbara. “I was

so impressed by the care I received when the boys were

born that I joined the Board of Directors.”

In the years that followed, she has held many other roles

at the hospital, including as a trustee, a member of the

Regional Advisory Board, and, a little less eagerly, as

a patient.

“Michael and I have both faced cancer,” she explains,

receiving their diagnoses at the hospital and undergoing

surgery at Dartmouth-Hitchcock. When it came time

for follow-up care, including chemo and radiation, the

doctors at Dartmouth encouraged the Powers to get

treatment closer to home at the Cancer Center.

“Their endorsement gave us complete confidence that

the Cancer Center was the best choice, and they were

right,” says Barbara. “The whole transition went very

smoothly … and we had physicians at both Dartmouth

and the hospital paying attention to our cases. We were

very grateful for that.”

But beyond the treatment, Barbara is thankful for the

care and compassion she received at SVMC. “When

you’re ill, sometimes you just want a hug. Through it all,

the nurses at SVMC were nothing but compassionate …

I never once felt alone.”

While preparing to watch her son play baseball, 47 year-old Penny Grace was stung several

times by bees. As loyal moms do, she took some antihistamine and carried on. However,

the next morning she found herself out of breath, hot, and dizzy. “Something just wasn’t

right,” she says. “I thought it was an allergic reaction to the stings.”

She headed to her primary care office, SVMC Internal Medicine, where Dr. David Sischy did

an electrocardiograph and delivered the news: she had had a heart attack.

“I was shocked,” Penny said. “… a heart attack, and I didn’t even know.”

After meeting SVMC Cardiology’s Dr. Scott Rogge in the Emergency Department, Penny

was transferred to Albany Medical Center for a stent and a three-day stay.

She spent the next 12 weeks attending SVMC’s Cardiac Rehab’s phase II and phase III

sessions and credits the Rehab staff, along with her primary care doctor, Kim Fodor, for

helping her get physically and mentally fit.

“You think you are going to rehab to get physically fit, but they are also helping you get

mentally fit to get back to your life.”

“ I was shocked,” Penny said. “… a heart attack, and I didn’t even know.”

Tackling community hunger Without collaboration, and a good

bit of compassion, one of SVMC’s

Hospitality Services’ biggest

undertakings would have never

gotten off the ground.

Together with Grateful Hearts, a

non-profit organization that converts

surplus produce from local farms

and other sources into ready-to-eat

meals, SVMC has contributed more

than 16,000 individual servings of

food to community members

in need.

According to Tiffany Tobin, director of Hospitality Services at SVMC, “Hunger

is a big and very real issue in our community. Our partnerships with Grateful

Hearts, the local food bank, and the Kitchen Cupboard, enable us to use the

resources and talent we already have in place to help combat the issue.”

Using the industrial-sized cooking equipment and their experienced team

of chefs and prep cooks, the department is able to transform the gleaned

products Grateful Hearts delivers into healthful and nutritious meals served

at the Kitchen Cupboard and other community food providers.

Elevating care through collaborationBarbara Richardson, MSN, RN-BC, CCRN, is living proof

that one person can make a difference.

A champion for transitional care nursing and identifying at-

risk patients, Richardson created a support network with

various care providers and organizations in the community.

The community care team’s efforts resulted in a 69 percent

decrease in hospitalizations and a 26 percent decrease in

ED visits for patients enrolled in the program.

Richardson’s work was recognized by the American Nurses

Credentialing Center, which named her a National Magnet

Nurse of the Year®, an honor bestowed to only five nurses

across the country in 2016.

Accountable Communities

SVHC’s Accountable Communities for Health (ACH) initiative aims to improve the health of the entire community by taking care beyond the hospital walls and linking resources throughout the community to improve outcomes and reduce costs. By redeploying clinical nurse specialists as transitional care nurses, SVHC’s ACH team is actively—and successfully—applying this approach to high-risk, chronically ill patients; behavioral health patients; patients dealing with substance abuse; and most recently, high-risk expectant mothers and newborns.

Without question, 2016 was a year of significant achievement and growth for Southwestern Vermont Health

Care, including our Foundation. It was filled with countless moments of caring, connecting, and collaborating,

but mostly it was a testament to the generous spirit of our donors.

Our sincere thanks go out to each and every donor, event attendee and sponsor, health care provider and staff

member, and grant-funding organization—including the State of Vermont. Their generosity helped provide

the resources necessary to advance the level of care and access throughout all the communities we serve. We

are both thankful and encouraged by these achievements, which we see as a trend of growing interest and

support of our mission and the work we do.

Beyond financial donations, we are grateful for the contributions of time and effort, along with the many

connections, provided by the volunteers who make up our Foundation Board, Board of Trustees, and

Regional Advisory Boards. These groups have demonstrated exceptional leadership, forged new and valuable

connections, and ensured that our message and mission are conveyed accurately and fully both internally and

externally. Their behind-the-scenes work is invaluable.

Thanks are also due to our physicians, nurses, clinical leaders, and staff who fulfill our mission daily and

maximize the impact of every donation. They have tirelessly collaborated with the Foundation, lending

countless hours serving on committees and as ambassadors at numerous events throughout the year. In

addition, their financial support has been unprecedented. We are proud to serve alongside them.

In 2018 we will commemorate our Centennial by celebrating our past 100 years of caring for our community

while focusing on our future. Our plans through 2020 are ambitious but essential to creating our vision of

becoming a preeminent, rural health care system. The support you lend has the power to improve the health

WHY WE GIVE:

Bill, Charlie, and Tony Hoyt When the $2.5 million Varian Linear Accelerator (Linac) was installed at SVMC in April of 2016, a number of people made their way to the hospital to see it for themselves. But for three visitors, the machine represented more than the state-of-the-art technology.

For brothers Bill, Charlie, and Tony Hoyt, the machine embodied the giving spirit of their aunt, Lynn Hoyt, whose generous estate gift made the purchase of the machine possible.

“Both my Aunt Lynn and Uncle Win loved southern Vermont and were very supportive of the organizations that contribute to the health and vibrancy of the community,” says Tony.

For Bill, whose wife is currently battling cancer, the Linac touches even deeper. “What they can accomplish with this machine is very impressive. To see my aunt’s wishes honored with a technology that can improve the lives of people with cancer is really very meaningful.”

WHY I GIVE:

Paul FosterEighty-five years ago Paul Foster was born at SVMC. In the years that followed, he returned three times for the birth of his own children. And more recently, he’s returned for treatment at The Cancer Center.

“If it weren’t for the care I’ve received at The Cancer Center,” he says, “I wouldn’t be here. I really believe that.”

Diagnosed with stage 3 prostate cancer three years ago, Paul and his son Michael, who often accompanies him for treatment, have made many visits to the Center.

“I’ve had family and friends travel a long way to get the same care we receive right here,” says Michael. “At a time when a lot of local hospitals are closing, it’s a real blessing to be able to stay close to home and to know that all the providers Dad sees are communicating with each other. When we walk in the door, we’re always greeted by name and always treated with respect and compassion.”

Paul’s ongoing experience at The Cancer Center inspired him to make a donation. “I owe them my life, and I’d like to make sure that they’re here for a long time to help others facing what I’m facing.”

WHY I GIVE:

Ruth Metcalfe SVMC’s director of perioperative services, Ruth Metcalfe, has a long history with the hospital. But when it comes to her reasons for contributing to the Annual Fund, it’s all about the future.

A 50-year employee, Metcalfe says, “Whatever I give, I give for the future. The hospital is such an important resource to the community, I think it’s important to do what I can, not only to ensure that it’s here for others in the future, but that it’s able to achieve the vision that’s been put forth.

“Frankly, it’s a triumph for a community hospital to simply still be operating in this day and age,” she adds. “But SVMC is doing more than that. We’re growing and evolving to serve the needs of the community today and the generations that will follow. I’m proud to have that creation be a part of my legacy.”

Cancer CrusadersOver the past four years, a quiet grassroots effort has been making a big difference in the lives of cancer patients in our community.

Organized and run strictly by volunteers, the Cancer Center Community Crusaders (4Cs) have raised over $150,000 for the Southwestern Vermont Regional Cancer Center (SVRCC) Patient Fund with the specific intent of easing the financial burden experienced by individuals and their families coping with cancer in our community.

The Fund places no restrictions on where recipients receive care, meaning that patients in Bennington and Windham Counties in Vermont, eastern Rensselaer and Washington Counties in New York, and northern Berkshire County in Massachusetts are eligible for assistance. In 2016 recipients used funds to cover expenses related to heating bills, car repairs, groceries, and dental care. In addition to the fundraising efforts of volunteers, local businesses have also generously contributed to the 4Cs. Wendy Petitt, Practice Manager at the SVRCC, notes that when she reaches out to local businesses and explains why they’re purchasing a gift card or requesting an oil delivery, more often than not, the business will offer a discount or provide a service for free. “Everyone’s been touched by cancer in some way and, in some ways, the caring is contagious,” says Petitt.

Barry ReardonRetired Warner Brothers studio executive Barry Reardon has a flair for creating memorable experiences. Last year he lent his creative talents to SVHC and launched what is planned to be a recurring benefit for our most generous donors.

2016 Kunzelmann FellowsFor over fifty years Helen Dixon Kunzelmann called Bennington home. Throughout her years in the Southshire, she was committed to building community and enhancing the quality of life for friends, neighbors, and family. Even today, years after her passing, her hopes and legacy live on through The Kunzelmann Fellowship and The Helen Dixon Kunzelmann Professional Nursing Fund. The Fellowship provides financial assistance to physicians at SVMC while the Fund provides scholarships to be applied to continuing education.

In 2016 the fellowship was awarded to Karena Webber, PA, and Keri Noel, NP, was the beneficiary of the Professional Nursing Fund.

Henry W. Putnam CircleMembership in the Henry W. Putnam Circle is awarded to those individuals who provide for Southwestern Vermont Health Care or one of its non-profit subsidiaries in their wills or estate plans. The Circle is named in honor of our first benefactor, Henry W. Putnam, Sr., who deeded the Bennington Water Works to the Town of Bennington in 1912 for the establishment of a public hospital. The Circle also pays tribute to his son, Henry W. Putnam, Jr., who supported the hospital with many lifetime gifts, followed by his $3 million bequest that forms the nucleus of the health system’s endowment funds.

WHY WE GIVE:

Mark and Kristen WallaceFor nearly 30 years, Mark and Kristen Wallace have called Southern Vermont home. They raised their two kids here, ran two successful businesses, as well as faced one of life’s biggest challenges: cancer.

“Both Kristen and I are cancer survivors,” explains Mark. “And like most folks, when we got our individual diagnoses, we went for second opinions. In both cases we went to Dana Farber and shared the plan the providers at the Southwestern Vermont Regional Cancer Center suggested. And both times, they said, ‘We can’t offer you anything better than what you’ll receive at the Cancer Center. Go there.’ We followed that advice and are here today because of it.”

That experience, combined with other encounters at the hospital, inspired the Wallaces to include the Cancer Center in their estate planning.

“We think it’s important to pay it forward,” says Kristen. “Including the Cancer Center in our will is an easy way to ensure that quality care is available to families and individuals like us in the future when they need it.”

Seventy-five SVHC staff members and their families celebrated our fourth Magnet designation by participating in the Bennington Battle Day Parade, the region’s largest.

SVHC staff members and their families lent a hand to renovate the extensive walking trails on SVMC’s Bennington Campus during the SVMC Trail Day.

As the kick-off to the Cancer Center Community Crusaders’ Day of Celebration, runners and walkers enjoyed a 5K route dotted with Revolutionary War landmarks.

The 13th Annual “Mom’s Day Off” at Bromley offered moms $20 lift tickets and all proceeds to the Southwestern Vermont Regional Cancer Center.

The community came out to the Cancer Center Community Crusaders Day of Celebration on June 4 to show their support of local cancer patients.

Staff participated in the annual National Wear Red Day to raise awareness of heart disease among women.

Committed to Community HealthWith an understanding that home ownership and quality living situations go hand-in-hand with good health, SVHC was proud to become a founding partner of Healthy Homes for Bennington, L3C, this past year.

Created in partnership with the Bank of Bennington, Healthy Homes revitalizes single-family homes in the downtown area. Once the renovation is complete, homes are independently appraised and made available to SVMC employees at a reduced price.

The program’s first house became a home to Russell Carrier and Leah Hart, who have 12 combined years as SVMC employees. The couple moved into the newly renovated home—their first—on Park Street with their baby boy. “This has been so exciting for us,” said Russell on move-in day. Hart added, “We are so grateful to SVHC and Healthy Homes for making home ownership possible for us at this stage in our lives. As a young couple with a nine-month-old, it would have taken years for us to be able to afford a down payment.”

Creating a caring environment togetherFacing a fatal illness in 2008, Susan

Sebastian came home to Vermont

to be near her mother. An avid

art collector, Sebastian despaired

over the blank walls of her patient

room. She turned that despair into

inspiration by asking her mother,

Elise Braun, to use the funds from

her estate to ensure no patient in

Vermont ever had to endure their

illness while staring at a blank wall

again. Braun agreed and last year

fulfilled the final piece of the promise

with the donation of 54 works of

original art to SVHC.

notes, “For the past several years we

have been attempting to transform

the ways in which care is delivered

throughout our hospital system. That

this gift should come to us at this time and

aid us in enhancing our environment in such

a powerful and beautiful way was a most

unexpected and wonderful surprise.”

Elise Braun founded the Susan Sebastian Foundation in honor of her daughter.

SVMC is accredited by the Joint Commission and recognized as a Top Performer on Key Quality Measures®.

SVMC received a 4-star rating from the Centers for Medicare and Medicaid Services for quality, patient safety, and the patient experience.

Through partnership with Dartmouth-Hitchcock, our patients have access to one of only three National Cancer Institute-designated comprehensive cancer centers in New England.

Our Cancer Center is accredited by the American College of Surgeons Commission on Cancer.

SVMC’s emergency physicians are certified by the American Board of Emergency Medicine.

SVMC received an “A” ranking for hospital safety from the Leapfrog Group, a national leader and advocate in hospital transparency, for the second consecutive year.
