• 11,005 patients aged 75 and over– 10% of our population

• Emergency Admissions in over 75s = 2,348 – 28% of all emergency admissions

• Emergency Admissions in over 65s = 3,709 – 44% of all emergency admissions

• >75s = 3,898 A&E attendances • >65s = 7,013 A&E attendances

Frail Elderly Session/Primary Care Session • Patients are lonely • Bypass number (1 number/HUB)• Need to be able to share records - EMIS community• Specialist nurses (Integrated/Standards/Falls)• Enhanced Care Team working in a different way • Explore night sitting• More visits/Admissions earlier in the day – Left Shift• Variation/Info (Emergency/OOH/Care homes OOH/Ambulance/ A & E

presentation)• Demand on appointments• Patient expectation higher• Multiple number of prescriptions• Only 10 minute consultations – not enough time for complex patients • Worried well – anxiety / isolated/ lonely
