• Observe areas for study that are not meeting expectations

• Use data to identify problems/opportunities for improvement

• Needs to be specific – designed to target an identified issue

• Quality Improvement plan needs to be comprehensive and include all departments

Governance and Leadership

• QAPI program must be led from the top

• Foster a culture focused on improvement

• Requires adequate funding/resourcing

• Must get input from all stakeholders including staff, families and residents

• Prioritize areas for improvement

• Balances improvement with resident-centered rights and choice

Feedback, Data Systems and Monitoring

• Systems are in place to monitor care and services

• Data is collected from multiple sources

• Development of indicators and measures

• Need to have an identified baseline so that improvement can be measured and documented

• Improvement can be measured against established benchmarks or against internal baseline measurements

• Tracks and monitors adverse events

Performance Improvement Projects (PIPS)

• Follows the Plan – Do – Study – Act Cycle

• PIPS should concentrate on a specific problem

• Information is systematically collected to clarify the issue and identify opportunities for improvement

• Uses quality improvement tools to perform root cause analysis
