Offshore Safety Passport

Offshore Safety Passport (OSP)

� PETRONAS Carigali Offshore Safety Passport (OSP) is a MANDATORY document for

all PETRONAS employees and contractors who wish to work on/stay on/visit

offshore facilities i.e. Offshore Platform, FPSO, Vessel and etc.

� This document testify that the applicant has undergone :-

� Mandatory BOSET Training Requirements

� PMU – Minimum Industrial Standard For Basic Offshore Safety & Emergency Training

(MIS-BOSET), Rev 1 Oct 2011.

� PETRONAS Carigali Offshore Safety Passport Guideline, Rev. 4, March 2012.

� Mandatory Offshore Medical Requirements

� PMU – Guidelines On Medical Examination For Offshore Workers, Rev. 2, March 2009.

Introduction to OSP

Introduction to OSP

Employees,

contractors and

visitors shall have

valid OSP to

work/visit/stay at

any of the

PETRONAS

Carigali’s offshore

facilities.

Person without valid

OSP shall not be

allowed to

proceed/work/

visit/stay offshore.

No person shall hold more than one OSP at one time.

OSP Category

Offshore Safety Passport (OSP)

Categories of OSP

� CATEGORY A

� Applicable to PETRONAS Carigali employees and Contractors employees

going offshore to work/stay at any of the PETRONAS Carigali’s and

Contractor offshore facilities (Engaged with Carigali Project).

� CATEGORY C

� Applicable to all permanent Members of PETRONAS CarigaliManagement Committee.

� CATEGORY D

� Seaman or Marine Crew as listed in the ship’s Article of Agreement whosenormal work do not require them to board offshore facilities(Platforms/Rigs/FPSO/FSO) and only travel by boat for crew change.

Categories of OSP

� Visitor

Applicable to visitor as stated below:

• Non PETRONAS Carigali Employees

• Non PETRONAS Carigali Contractor Employee.

� The movement of the Visitor passport holder shall be accompanied by the

facility focal person during the visit at all time.

� The Visitor passport holder is applicable for a day trip and do not stay

overnight. The visitor pass shall be returned to HSE Department upon return

to onshore.

� Prior to the offshore visit, visitor is required to declare his/her health

declaration and submit to the respective HSE Department for approval.

Medical Requirement

� Categories A, C & D

Shall be done at:

� PETRONAS Carigali or PMU Approved Medical Examiners (AMEs); or

� UKOOA, OGUK, ISOS and Registered Offshore Medical Examiners.

� Shall get endorsement from PETRONAS AMEs for Malaysia Operations only.

� Step for Endorsement:

� Bring Original and/or Scan copy of Offshore Medical Assessment Report

(Details Report) to any of PETRONAS AMEs.

� AMEs will endorsed and translate the report to PETRONAS Offshore

Medical Assessment Form (A3 Form).

Note:

• AMEs have a right to decline the endorsement due to incomplete

Offshore Medical Report by applicant.

• AMEs have a right to demand applicant to undergo additional test in

compliance with PMU Offshore Medical Requirement.

Medical Requirement

� Validity of Offshore Medical Assessment is maximum 2 years (AMEs Recommendation)

Note :

• PETRONAS Carigali list of Approved Medical Examiners (AMEs) valid for

PETRONAS Staff only.

• PMU List of Approved Medical Examiners (AMEs) valid for PETRONAS Contractor

Personnel only.

� Category Visitor

Shall complete the PETRONAS Carigali Health Declaration Form – subject to

HSE Management approval.

• For PETRONAS Employee : PETRONAS Email ID and Email Password.

• For Contractor application: Using Company Registration No (ROC) for user id and

given password.

� Online blacklisting of Applicant.

� Auto E-Mail Notification:

� Personnel Enroll Into e-OSP System Successfully.

� 1 Month before Contract Expired.

� 1 Month before Medical Fitness Assessment Expired.
