Discharge Planning Worksheet
While you are in the hospital talk with your doctor, nurses, case manager, social worker and staff to
plan for your needs when discharged.
Here is a list of questions and reminders to assist you in this process.
Where will you get care after discharge?
If you are looking at rehab or placement, are you working with a social worker or a case manager
here in the hospital? If not, please notify your doctor or nurse.
If you are going home or to stay with a caregiver, write down the name, address and phone number
of where you will be going:

Will you need any assistance or help at home?
Assistance with bathing, dressing, climbing stairs, using the bathroom, cooking, cleaning, physical
therapy, getting your medicines or getting to doctor’s appointments?
Will you need any medical equipment?
Have you and your caregiver learned to change your dressings or handle any other special medical
care you will need once discharged?
Do you know what symptoms to watch out for and whom to call if you notice them?
Do you understand what medications you are taking? Do you have a way to obtain them at
discharge?
If you have questions about any of the above, please make sure you notify your nurse, social worker
or case manager prior to discharge home.
How are you and/or your family coping with this illness/ hospital stay?

• nurses, case managers, doctors, social workers, physical therapists, speech & occupational
therapists, dieticians, and many others.
