Registration Overview

Registration opens on February 1st and remains open through May 1st. There are two parts to the registration process:

Part 1: Complete & submit online forms

Part 2: Print and fill out the listed forms, and collect documents. Bring those in person to the Student Services/Early Childhood Office at 40 Fairbank Road

Registration Part OneFill out the three online forms listed under Registration Part One

Registration Part TwoBelow are the Part Two forms & documents required for each student enrolling. Note that some will download a form to be printed, filled out, and returned.

1. Health Record Screening Form- Print & complete for each student2. Parent Information Form- Print & complete for each student3. Military Form- For Military Families only. Print & complete if

applicable4. Preschool Inventory Form- For K Registrations only. This form is to

be filled out by the current preschool teacher and mailed to the Early Childhood Office at 40 Fairbank Road.

5. Copy of your child's birth certificate or adoption decree6. Copy of most recent doctor's physical form (within the last 13

months) and immunization records (birth to current date). These are generated by your pediatrician.

7. Proof of Residency (POR) - See next slide

Proof of ResidencyYou must have at least one document from EACH of the following categories:

1. Evidence of Residency (1 document)

2. Evidence of Occupancy (1 document)

3. Evidence of Identification (1 document)

Evidence of Residency Examples

● Lease and/or Rental Agreement for the year of school enrollment (executed by both parties)

● Mortgage payment and/or property tax bill

● Residency/Occupancy Affidavit - Landlord/Shared Tenancies

● Purchase and Sale (P&S) Agreement along with a signed statement by a Bank Officer or licensed Real Estate Broker certifying financial commitment

● Section 8 Agreement

Wednesday, September 2● Kindergarten Meet and Greet Days are:

Wednesday, September 2 & Thursday, September 3● Families will be given a scheduled appointment time to visit

the kindergarten classroom. ● Kindergarteners will meet their teachers and a few peers

while seeing the classroom, cubbies and more.● Teachers and support staff will visit with families● Teachers will have a chance to make a connection with

each student and play a game/K screening activity

Kindergarten Screening*This is not an entrance or readiness assessment but an overview of

developmental areas per state mandate. It feels like a game to students.

Early ScreeningInventory-Revised (ESI-R)A brief developmental screening instrument that is individually administered.

• Visual motor • Fine motor • Language and cognition• Gross Motor• Speech• Parent Questionnaire• Health and Medical - vision

& hearing screening

Timelines:

• During your child’s first Meet and Greet Day of school

• Screening team reviews each child’s results and data

• More detailed ‘progress data’ shared at parent conferences

• Rescreen (if needed) in January• Continuous progress monitoring

English as a Second Language● 60 languages and dialects in Sudbury!● All proficiency levels welcome!● Home Language Survey process-all families complete online

form● Students who mention any language other than English are

screened for English language proficiency● ESL Program: most of the day with native-English speaking

peers, individual and small group instruction in English
