RCA : RCA is a method of problem solving and tries to identify the root causes of the problem or event

RCA must be performed Systematically. Problem will be having more than one root

cause. Purpose of identifying all solutions to the

problem. Analysis should be in a sequence of events. Root causes identification is depends on the

way in which the problem is defined.
