7 IRS Forms-For Small Business Payroll Programs-

Knowing which IRS forms you needfor your small business is no easy task.

This list of IRS forms is specific to small employers, so you

know just the forms you need for your payroll program.

1. Form W-2Form W-2 reports taxesyou withheld fromemployee wages to thegovernment.

Send Form W-2 to eachemployee and the SocialSecurity Administration(SSA).

2. Form W-3Form W-3 is a transmittalform that summarizes allyour form W-2s. Returnthis form with Form W-2 tothe SSA.

Employees don't receiveForm W-3.

File Forms W-2 & W-3 to the SSA, either bymail or electronically.

3. Form 940Form 940 is used to reportan employer's FUTA(Federal UnemploymentTax Act) tax return. You payFUTA taxes quarterly.

File Form 940 annually.Send it to the IRS byJanuary 31, 2017.

4. Form 941Use Form 941 to report federalincome taxes you withheld, andyou and your employees' sharesof FICA taxes every quarter.

You may not report these taxesthe same time you depositthem. Regardless, reportfederal income taxes and FICAtaxes with Form 941 quarterly.

First Form 941 quarterly deadline

Second Form 941 quarterly deadline

Third Form 941 quarterly deadline

Fourth Form 941 quarterly deadline

5. Form 944If you report federal income

taxes and FICA taxes annually

instead of quarterly, use Form

944 instead of Form 941. The

IRS must notify you to use Form

944.

File Form 940 annually. Send it

to the IRS by January 31, 2017.

6. Form 1095-BForm 1095-B is a new form used to

report employee health coverage.

File this form if you offer your

employees a self-insured health plan,

meaning you pay all your employees'

health claims—not just premiums.

File Form 1095-B for each full-time,

covered employee. Send one copy to

the employee and one to the IRS.

7. Form 1094-BForm 1094-B is a transmittal

form that summarizes Form

1095-B. It gives an overview of

the insurance options you offer

to employees.

Return Form 1094-B with Form

1095-B to the IRS. Employees

do not receive form 1094-B.
