How to Add a Sign Up Form on a Facebook Page Using
Log in to your MailChimp Account
Enter your Username and Password
Click Log In Button
Dashboard will open
Click the drop down arrow to the right of profile name
This box will open

Click “Account”
The “Account” page will open
Click “Integrations”
The “Integrations” page will open
Click the Facebook icon
If not yet log in to Facebook, it will open to the log in page
Enter log in details then click “Log In” button
Important Note
A Sign Up Form in MailChimp can only be placed on a Facebook Page that is managed through a Facebook Profile and has been integrated with MailChimp account.

To set up the integration if Facebook account is not yet integrated with MailChimp account
This dialog box will open upon logging in to Facebook from MailChimp’s “Integrations” page
Click the Okay button
Another dialog box will open
Click the drop down menu to choose who can see posts
Click the OK button
The pages modal will open
Click the Okay button
The MailChimp page will open displaying the success message

After integration has been set up
Click the drop down menu beside profile name
The drop down menu will open
Click “Account”
The “Account” page will open
Click “Integrations”
The “Integrations” page will open
Click the Facebook icon
Integration details will open
Click the ”Page to use” drop-down menu arrow
Click the Facebook Page where you want the sign up form to appear
In this tutorial, the Facebook Page chosen is “170th Avenue – Terno, Tutu and More Gift Ideas”
Click the ”List to use” drop-down menu arrow
Click the list name where you want to add your subscribers from Facebook
In this tutorial, the list “Special Event #1” is chosen
Click the “yes” option under “Use signup form tab”
Click the “Form theme” drop down menu arrow to choose a form style
To use the design from MailChimp-hosted sign up form, click “My List’s Theme (default)”
To have a form with Facebook's look and design, choose ”Facebook-esque”
“My List’s Theme” is chosen for this tutorial
Assign a name for the tab under “Tab label”. In this tutorial, “Special Offer” is the tab name
The Tab label is where subscribers will click on the Facebook Page to access the form.
Click the “Save” button
When people visits the Facebook Page, they will see the MailChimp sign up tab under the cover photo and
under the Apps section on the page

If the Sign Up tab is not visible on the Facebook Page, click the “More” tab
An option box will pop up
Click “Manage Tabs”
The “Manage Tabs” box will pop up
Drag the Sign Up (Special Offer) tab under the About tab to be visible on the Facebook Page
Sign up tab is now visible (labeled “Special Offer”)
Click the “Special Offer” tab
The MailChimp Sign Up Form will open
