WELL BEING & SUPPORT PLANNING Seminar Outline

Some lifestyle risks affecting our health and well being can lead to a range of

chronic illnesses such as: Heart disease, Stroke, Vascular dementia , Diabetes.

Increasingly many people past their fourth decade suffer with multiple medical

conditions impacting on their quality of life. Currently, as health and social care

practitioners and academics are challenged to explore creative ways of delivering

suitable care and support plans. Our case study approach will focus on

bariatric/obesity/overweight needs & how this can be implemented/captured on

the Support Plans to meet identified goals/outcomes.

This seminar will help you by:

- Enhancing your assessment intervention using reflective and ethical approaches

to practice.- Developing “well being” strategies, from your first hand experiences, that can

in turn allow you to improve your quality of life and also others.

This course draws on perspectives from Evidence Based Practice, together with

the intellectual and practice experiences of a highly acclaimed and international

field of experts.

This is an event not to be missed. It will provide invaluable continuous
