Mobile devices have broken the technology barrier we have been suffering too long. They open new possibilities in healthcare but require an efficient integration into the healthcare processes to become a reality in daily practices. This presentation is focus on how mHealth could support proactive care services; by having patient evolution information; improving patient-professional; and integrating the caregiver when needed. A way of getting near a care model centred on chronic needs and on prevention for healthy persons, a model we should be going to in the upcoming years.
