If We Could Do It Again
1. Collect tally information through a more efficient method such as a google sheet.
2. Expand on the reporting categories in the tally sheet. Have organizations ask consumers: How did you hear about us?
Note: This information would help us to determine the most effective outreach strategies for getting consumers enrolled.
3. Have a big celebration at the end!

• Self-generated leads from proactive patients
• Provides opportunity for patients of busy centers to request assistance if they are not able to work with enrollment staff
• Provides opportunity for patients of less busy centers to request assistance if they are not able to work with enrollment staff because they are part-time or outreach-focused

• Reaches patients who are not identified as uninsured by health center staff and/or are not approached about coverage options
• Reinforces outreach messaging through other mediums (signage, mailings, staff, waiting room videos, text messages, call hold messaging, etc.)
• Helps to regulate and streamline work in times of shifting demand

Implementation Checklist
• Gather the materials needed and create the Commit Cards and drop-boxes
• Announce the strategy to your enrollment staff and medical receptionist staff with respect to their roles, and to the full staff generally
• Provide materials to all staff to support the project (instructions, scripts)
• Plan to track your results in advance
• Ask all staff to help with promotion through emails, staff meetings, and newsletters

Successful Enrollments Resulting from Commit Cards
Monthly Premium of $36.98!
• A Westside patient who is a 37 year old woman first dropped her Commit Card at our Middletown health center when the enrollment staff member was out at an outreach event. Our staff called her back to follow-up and the patient wound up enrolling while she was at the center for an office visit. After applying and receiving a tax credit of $196, she said she was 100% satisfied with her monthly premium of $36.98!
Monthly Premium of $46.21!
• A Commit Card was completed by an immigrant family at one of our health centers. They had been found ineligible for Medicaid because they did not meet the requirement of 5 years of residency, but did qualify for significant tax subsidies of over $450 when assisted with an application. After the subsidy was applied, their premium was $46.21 a month.
