Lifestyle diseases and the stem cellsLifestyle disease are characterize by those diseases which are occurring primarily based on daily habits of people and are the result of inappropriate relationship of people with their surrounding environment.

The onset of these lifestyle diseases is insidious, they take years to develop, and once encountered do not lend themselves easily to cure.

Risk factors for these diseases include tobacco use, unhealthy diets and physical inactivity, wrong body posture, longer working hours, and disturbed biological clock.

These diseases are driven by forces that include ageing, rapid unplanned urbanization, and the globalization of unhealthy lifestyles .For example, globalization of unhealthy lifestyles like unhealthy diets may show up in individuals as raised blood pressure, increased blood glucose, elevated blood lipids, overweight and obesity. These are called ‘intermediate risk factors’ which can lead to cardiovascular disease, a NCD.

If beta cells don’t produce enough insulin, or the body doesn’t respond to the insulin that is present, glucose builds up in the blood instead of being absorbed by cells in the body, leading to diabetes. In diabetes, the body’s cells are starved of energy despite high blood glucose levels.

Over time, high blood glucose damages nerves and blood vessels, leading to complications such as heart disease, stroke, kidney disease, blindness, etc. Other complications of diabetes may include increased susceptibility to other diseases, loss of mobility with aging, depression, and pregnancy problems.

The two main types of diabetes are Type 1 diabetes and Type 2 diabetes.

Type 1 diabetes is caused by a lack of insulin due to the destruction of insulin-producing beta cells in the pancreas. In type 1 diabetes the body’s immune system attacks and destroys the beta cells. In the past, type 1 diabetes was called juvenile diabetes or insulin-dependent diabetes mellitus.

Type 2 diabetes is the most common form of diabetes is caused by a combination of factors, including insulin resistance, a condition in which the body’s muscle, fat, and liver cells do not use insulin effectively. Type 2 diabetes develops most often in middle-aged and older people who are also overweight and obese children and adolescents. Scientists think genetic susceptibility and environmental factors are the most likely triggers of type 2 diabetes.

Hope for the treatment through stem cells

Currently there is no cure for diabetes. Although type 2 diabetes can at least be controlled by healthy diet and regular exercise but in case of type 2 diabetes, patient has to test their blood sugar level, several times in a day and inject insulin when it is required. Over the time, negligence in case of type 1 diabetes leads to high blood sugar levels and can cause serious damage to the heart, eyes, blood vessels, kidneys and nerves, whilst injecting high amount of insulin may lead to low blood sugar level and this can be fatal.

Treatment of Type 1 diabetes is possible by transplanting the whole pancreas or the beta cells producing islet cells. By this body regain the control of blood sugar level and hence insulin will no longer be required.

It is possible to treat Type 1 diabetes by transplanting islet cells or even a whole pancreas into the patient from a donor. Transplants can enable the body to regain control of blood sugar levels so that insulin injections are no longer needed. Islet transplantation is becoming more common because whole pancreas transplants involve major surgery and involves risk.

Lifestyle diseases and the stem cellsHow could stem cells help?Getting a donor for islet transplantation is the biggest problem so researchers are focusing on growing new beta cells from stem cells and use them in replacement therapy. There are several different sources to get the stem cells and using them for treatment:

Embryonic stem cells:

Human Embryonic Stem Cells could be differentiated into immature beta cells which when transplanted inside human body will sense the sugar level in blood and respond by producing insulin.

Researchers have shown that this is possible in mice however precursor cells (transplanted immature beta cells) have the potential to form tumors so this need to be evaluate before transplanting into humans.

Making fully differentiated and functional beta cells for transplantation is another way however more research is required to understand how to make and purify these cells.

Reprogrammed cells (or iPS cells):

IPS’s created by reprogramming of patient’s own stem cells can also be used for the transplantation and creation of insulin producing cells.

This proves true in case of diabetic mouse but the insulin production is low as compared to embryonic stem cells.

However, using iPS cells could solve the problem of transplant rejection – the transplanted cells were taken from the patient’s body so chances or rejection are almost nil. Even so, the body might still attack the cells ,because type 1 diabetes may causes the immune system to attack even the body’s own pancreatic beta cells.

Stem cells in the patient's own body:

As per few researchers, the stem cells which are already present inside the patient’s pancrease can be encouraged to make new beta cells.The first challenge in this approach is to find the stem cells in the pancreas. Beta cell progenitors which are found in the pancreas have some stem cell properties but they cannot renew themselves indefinitely.

Beta cell progenitors can be used to produce new mature beta cells in the lab, but more research is needed to understand how this works. Researchers are working to find drugs that can activate the progenitor cells in the body of a diabetes patient.

Apart from finding the methods of beta cells creation, scientists are busy in finding the ways to make sure that cells survive once gets transplanted in the patient.

Current research is focused on making the cells which efficiently control blood sugar level and preventing the transplanted cells from autoimmune response as it lead to destruction of beta cells .This involves inserting the cells in the form of protective capsule or at a place where immune system is less active.

Stem cells from patients bone marrow can also be used to re educate the immune system to not to attack on the beta cells. This has already been tested in a small number of patients in a phase I clinical trial, with some positive results.

Further research is needed to establish whether the technique can be developed into a safe and effective therapy.
