The (Potentially) Violent Patient
• Any patient can be violent.
• Recognize patients at increased risk:
: Psychotic, intoxicated/withdrawal, delirious, past history of violence, schizophrenia, antisocial personality disorder.
• Warning Signs;: Agitated, escalating, paranoid, anxious.
• Trust your intuition

Approaching the potentially violent pt
• At any point, leave immediately and initiate seclusion or restraint if any concerns /destabilization.
• Remove any weapons prior to interview.• Quiet surroundings with “team” assembled.• Assure pt of safety.

• Maintain open exit for pt and physician.
• Never turn your back on patient.
• Maintain distance 6- 8 feet.
• Try to project calmness.
• Adopt non-confrontational posture –avoid prolonged eye contact, not face to face.

• Allow pt to vent/express feelings.
• Try to identify the predominant concern for the pt and attempt to acknowledge their feelings.
• Do not bargain – set clear limits/options.
• Suggest medication as a means to regain control.
Management of the violent patient
• Immediate evaluation: history, physical examination and mental state assessment

Delirium• Delirium or acute confusional state is a transient global
disorder of cognition
• It is a relatively common disorder, esp in older people with physical illness
• has a high morbidity and mortality
• is often under-recognised and undertreated. Early dx and resolution of symptoms are correlated with the most favorable outcomes.
• Therefore, it must be treated as a medical emergency.
Clinical Features
• Disturbance of consciousness: inability to focus, sustain or shift attention
• Changes in cognition: memory impaired, disorientation, language disturbance
• Perceptional disturbances: misinterpretations, illusions and hallucinations
Clinical features
• Disturbance in sleep-wake cycle
• Disturbed psychomotor behaviour
• Impaired judgment
• Emotional disturbance

• Good History esp. behaviour at night
• Find and treat causative factor
• Investigate appropriately
• Restore the normal nutritional and metabolic state
• Reduce environmental noise, ensure adequate lighting

• Sedation: relieves distress and prevents
complications
: Haloperidol 0.5 mg , increase by 0.25 mg to
1mg

Acute psychosis is the presence of the mental state where appreciation of reality is impaired, as evidenced by the presence
of psychotic symptoms
Definition
Symptom of psychosis• Positive symptoms
: delusions : hallucination : formal thought disorder
• Negative symptoms: flat affect: poverty of thought : lack of motivation : social withdrawal
• Cognitive symptoms: impaired working
memory: impaired executive
function
• Mood symptoms: depressed mood: mania
• Anxiety/panic/perplexity• Aggression/ hostility/
suicidal behavior
Psychosis
• a mental state of dysfunction in behavior and thought process.
• The mental capacity is grossly distorted, and thought is disorganized.
• This causes an inability to recognize reality or relate to others in a meaningful ways.
Assessment
• The 1st step is to establish a therapeutic alliance and rapport; interest, trust, and a sincere offer of assistance are helpful.
• Doctor should be calm, friendly, and unhurried, while asking questions clearly.
• It is always helpful to talk to the patient’s family
• There are a number of physical causes of acute psychoses
• Frequently physical examination is not possible and needs to be deferred until psychosis has been controlled.

Features That Point to a Medical Cause of a Mental Disorder
• Acute onset (within hours or minutes, with prevailing symptoms)
• First episode• Geriatric age• Current medical illness or injury• Significant substance abuse• Nonauditory disturbances of perception• Neurological symptoms
: loss of consciousness, seizures, head injury, change in headache pattern, change in vision
• Classic mental status signs
: diminished alertness, disorientation, memory impairment, impairment in concentration and attention, dyscalculia, concreteness
• Other mental status signs
: speech, movement, or gait disorders
• Constructional apraxia
: difficulties in drawing clock, cube, intersecting pentagons, Bender gestalt design

Risk assessment and hospitalisation
• The clinical judgment as to whether the pt constitutes a risk to him/herself or others must be carried out.
• If there is a danger of suicide or violence, inpatient management will usually be needed.
• Sometimes it will be necessary to invoke legal procedures to facilitate involuntary hospitalisation.
Medications
1. Benzodiazepines: diazepam, lorazepam
2. Antipsychotic drug: haloperidol
3. Long acting antipsychotic drug
Antipsychotic side effects• Acute dystonia
: brief or prolonged contractions of muscles that result in obviously abnormal movements or postures
: oculogyric crises, tongue protrusion, torticollis, laryngeal–pharyngeal dystonias,
: Dystonia can be painful and frightening
Rx
: Cogentin 0.5 to 2 mg tid ; IM or IV 1 to 2 mg
: Benadryl 25 mg qid; IM or IV 25 mg
: Diazepam 5-10 mg IM
: Trihexylphenidyl 2-5 mg bid or tid
Antipsychotic side effects
• Akathisia : subjective feelings or objective signs of restlessness: sense of anxiety, jitteriness, pacing, rapid alternation of sitting and standing
Rx: Propranolol 20 to 40 mg t.i.d : Clonazepam 1 mg bid

• Panic attack– Intense fear, tension,
restlessness– Patient overwhelmed,
cannot concentrate– May also cause
anxiety, agitation among family, bystanders
