access to at least one internal or external formulary for the entire EHR Reporting Period.

than 100 prescriptions during the EHRreporting period

as structured data.• Stage One Threshold: > 40% of all clinical lab

• Exclusion: An EP who orders no lab tests whose results are either in a +/- or numeric formatduring the EHR reporting period

• Exclusion: EP who has no patients >= 65 or <= 5 years of age with records maintained in the certified EHR technology.

access to their health information (lab results, problems, Rx, allergies) within 4 business days of the information being available to the EP.

• Exclusion: EP who neither transfers a patient to another setting nor refers a patient to another provider during the EHR period.

immunization registries and actual submission according to applicable law and practice
• Stage One Threshold: Perform at least one test of certified EHR’s capacity to submit (submission if registry has capacity to receive)
• Exclusion: EP who administers no immunizations during the EHR reporting period or whereno registry has capacity to receiveelectronically

Electronic Syndromic Electronic Syndromic Surveillance: Menu Set Measure Surveillance: Menu Set Measure #10#10• Capacity to submit ESS data to public health
agencies according to applicable law and practice
• Stage One Threshold: Perform at least on test of certified EHR’s capacity to submit ESS (unless no Public Health Agencies have capacity to receive info electronically
• Exclusion: EP who does not collect/submit any reportable syndromic information on theirpatients
