High risk diabetes management program

Individuals choosing this program must meet the following

eligibility criteria:

Employees

Spouses covered under Eastman health insurance

Diagnosis of diabetes along with any one of the following:

Prior MI, Stroke, Heart Failure, Angina or TIA

HbA1c greater than 8

LDL greater than 130

BP greater than 140/90

Microalbuminuria or GFR under 60

At the end of the first year: Approximately 80% of those engaged had made progress in at

least one category (decrease in HbA1c or FBS, decrease in LDL or

decrease in BP)
